Calcium Channel Blockers, as the name suggests, prevents or reduces the opening of these channels. There are different classes of Calcium channel blockers, but almost of all of them acts on the L-type voltage-gated calcium channel. Therefore, blocking or reducing calcium entry into these cells, means inhibiting calcium effects, and thus causes the following reaction:
Vasodilation - by acting on vascular smooth muscle, Calcium channel blockers reduces contraction of the arteries which causes an increase in arterial diameter. This drugs primarily affect arteries, with minimal effects on venous vessels.
Calcium Channel Blockers also have a Negative Inotropic effect - by acting on cardiac muscle, Calcium channel blockers therefore can reduce the force of the contraction in the heart.
Calcium channel blockers also have a Negative Chrono-tropic - effect - by slowing down the conduction of electrical activity within the heart, therefore Calcium channel blockers may affect the rate of the heartbeat.
They also can exhibit a Negative Dromo-tropic effect - by slowing down the conduction of the electrical activity of the heart, thus the conduction of velocity also decreases, particularly at the atrioventricular node. Which can ultimately slow the heart rate.
Therapeutic Use
Calcium Channel Blockers are used to control a variety of medical condition such as high blood pressure, chest pain, and tachyarrhythmia.
As anti-hypertensive drug - The effects of calcium in the heart muscles, is that it causes the muscle to act aggressively, by contracting more forcefully. Calcium also stimulates vascular smooth muscle contraction resulting in narrow blood vessels. This series of events many times can result in high blood pressure. By preventing the entry of calcium into the heart muscle and vascular smooth muscles (particularly in the arteries), the heart muscle contraction will not be too strong and arterial vessels are able to relax and dilate, leading to lower blood pressure.
As anti-anginal drug - The word angina is one that you will hear a lot as a nursing student and nurse. This is a fancy word for “chest pain.”
Nice. I'm a pharmacist and this explanation is very clear and organized.
Betty Huang Thanks dear! Thanks for letting me know. I really appreciate it. Looking forward on hearing from you again soon. -xoxo
Pls send me no. Help me
There no way a nursing student could get the questions on a test wrong after watching this video. Very nice. Broke it down in a very nursing student friendly way 👍
Rina Contreraz Hi Rina! Thanks so much for letting me know! Unfortunately, the questions are always surprisingly difficult... believe it or not... this is why I'm passionate about helping you guys... unless you have help, nursing school (I believe) is impossible without additional resources :). So when you come to this topic in nursing school, make sure you review the additional questions on my website: EmpoweRN.com and also go through a good NCLEX review book :) Keep up the good work!!
xoxo

Hi, Caroline.
Your videos are truly informative for every nurses specially to those who aspire to be one of us.
Btw, Ive watched all your vids regarding anti hypertensive medications.
-Ace Inhibitos, -ARBS, -BetaBlockers -CaChannelBlockers. However, Im still looking for other classifications of anti hypertnsives, such as Alpha Blockers, Central Acting Symptholitc/Adrenergic Blockers and Direct Acting Vasodilators.
Im hoping you could do educatioonal vids for these drugs as well!
Thank you and more power!
Love your videos - filled with such great information, easy to follow

Got a pal in ICU getting Cardizem..good bp good rhythm ,bad breathing problems..shortness of breath. I hope the hospital picked the right stuff to give her. I am going to grill them on this choice..but politely. Thanks for arming me.
Hi Tom, I’m very sorry to hear about your friend! I will say a prayer for him!! It is always good to be a patient advocate 😊 your friend is lucky to have you! Wishing him a speedy recovery!!
This is awesome. We are learning more stuff about the heart this quarter. My request for a new video is the hemodynamic monitoring. Cardiac monitoring can be so complicated and so much in detail to learn. Thank you so much.

ABSOLUTELY AMAZING!!! Thank you thank you thank you!!
Nice and informative. Thank you so much for shinning the light. i enjoyed it all.
This is one of the best video you've done so far. I like how you broke down the different groups. I love the animation of action sites. The background music is a bit loud. It takes attention away from the important information you are presenting. LOVE the NCLEX style questions. Request: elaborate on LAB values. (I know you did a short video before). Thanks!
Moxie Girl Hi Moxie!
Thanks so much for the comment & also for for letting me know that you liked the video!! Make sure you go to the website & you can download the audio of the video with & without music. When I watch the videos without music... it just needs a little something... I will try to lower the audio though!

I would like to see a whiteboard or else a
digital representation of the particular words as you use them. Your pronunciation is perfect. I learned a lot; I’m not a nursing student; I’m a gerontologist. I try to advise older people, especially minorities, about the heart and different drugs, and their doctors don’t tell them anything. So I think there are many mistakes I’ve seen calcium channels administered with the beta for angina. You opened my eyes to many drug reactions that I had no idea how the calcium channels worked. I got a lot out of that video that stuck in my mind, like gorilla glue. You know how to fire a lot of arrows at once and hit a lot of targets_ amazing--Successfully. And I know it shouldn’t matter but the little animation being so well dressed in an appropriate color made all the difference to me. Thank you
This video is really helpfull .....it helps me in my studies.....thank u so much for this video...

I was put on 480mg a day of diltiazem. I had no idea how massive a dose I was put on. Why was I placed on that much? An incompetent cardiologist. I should have been on about 60 mg per day because I just had a case of atrial fibrillation but now I have lost about 15 pounds of muscle, have been debilitated with fatigue every since (Feb 2014) and I weaned myself off this drug around April of 2014 and the effects never cleared up.
This is a great drug for what it does but if you give someone 480mg a day, it will be a slow death sentence. If I don't find out what that drug has burned out in my body soon, I won't be around much longer. My legs are so thin now and I was in good shape b4. I've been to numerous clinics and doctors and they all deny it's the diltiazem and refuse to follow my hunch and test some things that involve what a CCB overdose would show (like acidosis and other things). My heart no longer speeds up or responds like it did. It beats slower now and I cannot exercise and I sit all day.
Anyone know a compassionate place that would actually do some blood tests and maybe muscle biopsy/blood-comparison to save my life? I live near Memphis and it's completely nothing but people who suspect you of being an idiot long before you walk through the door.
Ever since Feb 2014 I have had low heart rate, low BP and flushing and weakness and all the side effects of diltiazem. I have contacted a local law firm about this also since the cardiologist who put me on that dosage is monitoring ALL of the medical review websites to be sure he doesn't get any negative reviews and he had someone call me at home to complain that I wrote a negative review of him. ! Everyone I've asked said 480mg a day is too high for someone with a-fib. My life is gone and I need to probably try one last time and get a plane ticket somewhere. Thanks.
+TruthSurge.. there's a process to leave a drug.. immediately stopping a drug too has effects.. i too went through the same.. i took the blocker and after years even have this fatigue
zeejoyful Did you not pay attention to what I wrote? Here is the relevant part:
"ever since (Feb 2014) and I weaned myself off this drug around April of 2014"
I'd say 2 months is long enough to wean yourself off of 480 mg down to 0.
I got my growth hormone checked and it was 0. I think the CCB can mess up your hypothalamus gland and maybe pituitary also. You could get your human growth hormone levels checked and if you can find some really knowledgable endocrine person to do some hypothalamus function tests, those might turn up something. I have been injecting HGH now for about 1 month (no thanks to any of the dozen doctors I've begged for basic testing, I found out on my own). Not sure if it's helping much but I'm using about .6 mg a day. My HGH level after 3 days was showing .22 which is normal for my age so I know it's working but I think my hypothalamus gland has been damaged somewhat because I have a lot of other symptoms that are all related to hypothalamus functions. Look up hypothalamus on wikipedia and see if you have anything related like your heart not speeding up enough when you move around, blood pressure too low during any exercise, low appetite, less fear, very slow digestion, etc. I have all those.
thnkyou so much i realy like your videoa and appreciate your work
thanks!!! great info in a succinct presentation. just what i was looking for!

isn't amplodipine an exception. as it's the only dihydropyridine that doesn't cause reflex tachycardia?
+Terrell McNasty Hi Terrell!
Amlodipine is a second generation dihydropyridine... it is less likely to cause reflex tachycardia.... compared to the other dihydropyridines. But it can still occur. I hope this helps!
xoxo!
- Caroline
Thank you so much.
Rose H Hi Rose!
You are welcome :) Nice to see you on here again! See you soon!
xoxo!!
I love all of your videos. They have gotten better and better. I love how thorough they are, and how you explain the pathophysiology. One question I cannot find the answer to...If we were taught excess Ca usually acts as a "sedative" causing reduced neurological response, and even coma, as well as muscle weakness..why does a calcium channel blocker, which reduces the entry of calcium into the cell...cause relaxation/dialation? In other words, if too much Calcium causes weakness and relaxation, why does blocking calcium cause vessel relaxation?
hunger4jsutice
If I could guess it would be due to vasodilation in the brain? but don't take me on that one! Haha
I got all my answers right yay !!!
This video was great! I love how you break down the classes of the CCBs and when they should and shouldn't be used. I also love the NCLEX questions at the end. I hope I have CCBs when I pass meds next week with my instructor! Could you do some videos on phosphate binders, diuretics, antibiotics, seizure medication, etc. I'll keep watching! Thanks!
