The RBC with and without casts distinction was something that I always wondered about and meant to investigate, but never did. Having just looked into it, it doesn't seem to be true at all! I generally double check (or triple check) everything that goes into these videos, but I wanted to publish this one ASAP to keep it timely with Harold Ramis' death from vasculitis, and this one point fell through the cracks. I was at a loss but my cardiologist suggested vasculitis and referred me to a Rheumatologist. Finally. My appt is still six months away but I feel better knowing what I’m looking at. Thank you!

I had a vitreous hamerrage in right eye, I was treated for 2 years and now Im fine. But how can I keep myself health and get rid of vasculitis? I dono what is root cause all test were done?? Im hardly 30 and this causes a lot of anxiety, I dont drink smoke anything. Will this also cause problems if we plan for babies?
I'm surprised to see erythema nodosum listed as a common clinical feature in vasculitis. I'm a med student in Finland and our guidelines on erythema nodosum make no mention of vasculitis as an etiological factor. I made a quick search on the topic and found a paper explicitly stating that erythema nodosum is not associated with vasculitis (Schwartz et a; American family physician 2007, 75:695-700. PubMed ID 17375516). I didn't find any articles supporting an association between the two. Could you clarify?

As noted in another comment below, I was attempting to publish this video ASAP after Harold Ramis' death, and didn't double check everything as thoroughly as I normally do (which I now know to do for future celebrity deaths...) However, erythema nodosum has been described in vasculitis, just not as commonly as the video implies, and not specifically in medium vessel vasculitis as the chart incorrect indicates. First, it's seen in ~1/3 of cases of Behcet's disease (dozens of papers, here is just one: Chajek T, Fainaru M. Behcet's disease. Report of 41 cases and a review of the literature. Medicine (Baltimore). 1975 May;54(3):179-96. PMID: 1095889). It has also been uncommonly reported in granulomatosis with polyangiitis (Barksdale SK, et al. Cutaneous pathology in Wegener's granulomatosis. A clinicopathologic study of 75 biopsies in 46 patients. Am J Surg Pathol. 1995 Feb;19(2):161-72. PMID: 7832276). And has been reported in Takayasu's arteritis (Frances C, et al. Cutaneous manifestations of Takayasu arteritis. A retrospective study of 80 cases.Dermatologica. 1990;181(4):266-72. PMID: 1981565). An interesting case report of recurrent erythema nodosum being ultimately attributed to an initial presentation of Takayasu is here: Gupta M, et al Recurrent erythema nodosum: a red flag sign of hidden systemic vasculitis. BMJ Case Rep. 2013 Apr 9;2013. PMID: 23576669). Regarding the article you've cited, I can only speculate about the contradiction. First, the authors may not consider Behcet's disease to be a vasculitis. Although there is some clinically irrelevant disagreement as to the categorization of the disease as either a primary vasculitis, or as a disease associated with a secondary vasculitis, it doesn't show up at all on some classic lists of the vasculitides that you may come across in general med textbooks and review books. Another potential source of confusion, and this is with the caveat that I am far removed from being a dermatopathologist, but the authors of the Am Fam Phys paper seem to consider the presence of vasculitis on a skin biopsy as an exclusion criteria for diagnosing EN. Interestingly, some of the aforementioned papers refer to the skin lesions seen in some of these vasculitis patients as "erythema nodosum-like" or the patients having been given "a clinical diagnosis of erythema nodosum". This suggests some distinction between EN diagnosed on clinical grounds, from EN diagnosed from biopsy. But adding to the confusion, some papers that I've just been skimming about the pathology of EN suggest disagreement on whether vasculitis on biopsy even excludes EN. The bottom line, legit EN appears quite common in Behcet's disease, and EN-like lesions (i.e. clinically indistinguishable from "true EN" on biopsy) are occasionally reported in Takayasu's and granulomatosis with polyangiits. In retrospect, I should not have listed EN as "a clinical feature commonly seen in vasculitis". I'm adding an annotation to clarify for future viewers.

Can you please explain to me what the difference in appearance of a"hot water bottle/heating pad rash " is compared to livedo reticularis? I have it on front of my thighs bad While I do have pots and persistent low blood pressure my whole life (ever sense I was a teen my normal BP is about 80/40 nothing brings it up we've tried salt now considering vasopressin ) Either way I HAVE used a heating pad while I sleep on my legs and this rash look is in every way like the livedo as pictured BUT it also looks in every way like a hot wattwe bottle rash Even my dr is stumped I should as I haven't used a hot pad on legs in few weeks and it's still there Along with worsening BP symptoms FYI I'm a 30 year old female of normal weight never been over weight never had hypertension it's always hypo and we don't know why
I started getting itchy 5 days ago around the stomach. Next day I had red marks that I thought were animal bed bug bites. Next days got more itchy and spots spread and looked more red. Headaches started coming in to play too. Finally saw a doctor today and he said he thinks small blood vessel vasculitis. Prescribed me prednisone. Today was first day I took the pills. 4 more days to go.

also if possible .... organ involvement in systemic diseases .... like cardiac or renal involvement in different systemic disease .... ufff medicine is really vast ...
It's very subjective. if specific criteria exist for grading the severity of a specific form of vasculitis in regards to treatment, I think most doctors wouldn't adhere to it too strictly, and would instead use their "clinical experience" (i.e. use their gut).
Hi I’m 15 years old and I’ve had the livedo reticularis pattern on my legs, arms, and sometimes on my stomach(not as noticeable) and I’ve had it for about 5 years. It makes me very self conscious and I haven’t worn shorts in forever! Do u know if there’s anything I can do about this?
I'm sorry to hear that. To the best of my knowledge, there is no specific treatment for idiopathic livdeo reticularis ("idiopathic" means cases in which the cause is unknown and it's not associated with another disease).

I have that! I’ve had a “rash” since August 1 2021. I went to the doctor they wouldn’t come in my room. I was miserable. I’ve dealt with it since on my own. Getting steroids is like buying crack. I’m still miserable. No insurance No help
+tracieamoore Once the diagnosis has been confirmed, a rheumatologist is the only person typically qualified to help patients manage most forms of vasculitis over a long time (with a few exceptions). However, if the diagnosis is just suspected and the patient needs to undergo a work-up to confirm or refute it, either a good primary care doctor, hospitalist, or relevant medical specialist should be able to do so.
Very sorry about the omission. For my videos that are intended to provide a broad overview of a general category of diseases to medical trainees, I need to balance thoroughness with "digestability" (i.e. is it a compact enough amount of information for students to click on, watch, and remember) - which I know doesn't help if you're a patient looking for info on a rare disease. Urticarial vasculitis is a immune-complex associated small vessel vasculitis (same category as cryoglobulinemic vasculitis) categorized by urticaria (which unlike most other causes of urticaria, can be painful in additional to itchy) and often (though not always) low complement levels (complement is a collection of proteins used by the immune system). In addition to the urticaria, joint pain/arthritis is a particularly common manifestation. Patients can also have ocular complications, recurrent abdominal pain, nausea/vomiting, cough, and shortness of breath (among other symptoms). I just did a search here on CZcams hoping to find a good, reliable video on urticarial vasculitis to share with you, but unfortunately came up empty.
Just to mention that we give steroids only when the cause is immune mediated , but if the cause was infection , so the steroid will make it worse . So steroid should be avoided when the cause is infection

No, not usually. Only a small percentage of cutaneous small vessel vasculitis is caused by cancer. It's more commonly associated with infections or autoimmune disease, or is a medication side effect. About half of cases are idiopathic (i.e. no specific cause can be identified).

After seeing this video I’m sure I have vasculitis and it runs in my family, unfortunately. I’ve been reading for two months and this was a very succinct video. Very helpful. FYI I'm a 30 year old female of normal weight never been over weight never had hypertension it's always hypo and we don't know why
I started getting itchy 5 days ago around the stomach. Next day I had red marks that I thought were animal bed bug bites. Next days got more itchy and spots spread and looked more red. Headaches started coming in to play too. Finally saw a doctor today and he said he thinks small blood vessel vasculitis. Prescribed me prednisone. Today was first day I took the pills. 4 more days to go.
I can't give specific, individualized medical advice here. But no form of vasculitis is cured with 5 days of prednisone.

How are you doing now, though one year later, did you develop any such problems later?
@@elchinn87 I haven't been to the doctor since then, so I don't know if any other problems developed internally, but from the outside there are no other problems.
also if possible .... organ involvement in systemic diseases .... like cardiac or renal involvement in different systemic disease .... ufff medicine is really vast ...

It's very subjective. if specific criteria exist for grading the severity of a specific form of vasculitis in regards to treatment, I think most doctors wouldn't adhere to it too strictly, and would instead use their "clinical experience" (i.e. use their gut).

Hi I’m 15 years old and I’ve had the livedo reticularis pattern on my legs, arms, and sometimes on my stomach(not as noticeable) and I’ve had it for about 5 years. It makes me very self conscious and I haven’t worn shorts in forever! Do u know if there’s anything I can do about this?
I'm sorry to hear that. To the best of my knowledge, there is no specific treatment for idiopathic livdeo reticularis ("idiopathic" means cases in which the cause is unknown and it's not associated with another disease).

I have that! I’ve had a “rash” since August 1 2021. I went to the doctor they wouldn’t come in my room. I was miserable. I’ve dealt with it since on my own. Getting steroids is like buying crack. I’m still miserable. No insurance No help

Urticarial vasculitis is a immune-complex associated small vessel vasculitis (same category as cryoglobulinemic vasculitis) categorized by urticaria (which unlike most other causes of urticaria, can be painful in additional to itchy) and often (though not always) low complement levels (complement is a collection of proteins used by the immune system). In addition to the urticaria, joint pain/arthritis is a particularly common manifestation. Patients can also have ocular complications, recurrent abdominal pain, nausea/vomiting, cough, and shortness of breath (among other symptoms).
I just did a search here on CZcams hoping to find a good, reliable video on urticarial vasculitis to share with you, but unfortunately came up empty.
Just to mention that we give steroids only when the cause is immune mediated , but if the cause was infection , so the steroid will make it worse .
So steroid should be avoided when the cause is infection

No, not usually. Only a small percentage of cutaneous small vessel vasculitis is caused by cancer. It's more commonly associated with infections or autoimmune disease, or is a medication side effect. About half of cases are idiopathic (i.e. no specific cause can be identified).

I disagree. I’m waiting for months to see a Rheumatologist. In the meantime, my cardiologist has suggested vasculitis. After seeing this video I’m sure I have vasculitis and it runs in my family, unfortunately. I’ve been reading for two months and this was a very succinct video. Very helpful. The more I learn, the less fearful I am. Having to wait six more months for the Rheumatologist! That is the worst part.
