    Ive passed out maybe 10 times unexpectedly in my lifetime Not sure if that meams I have very mild POTS or what

      This is not individualized medical advice or an opinion about your specific situation, but in general, passing out without warning is an uncommon manifestation of POTS. If I was seeing a patient and the only symptom they reported to me was unexpectedly passing out on occasion, POTS would not be a concern of mine.

    Nice video. House of God, good book. Don't forget the bed height index... LOL social admits --> treat'em, (feed 'em) and street'em "GI Rounds"--> time to eat!!! we also used to keep track of patients on index cards (3x5's) and trade patients depending on complexity and difficulty. This was in the late 90's.

      In the US, there are mechanisms by which a medical team could theoretically place a DNR order on an incapacitated patient over a family's objection, but it's such a legal and PR quagmire that it's virtually never done. In 20 years, I've never personally seen it on a patient to whom I had a connection. Hospitals have even had issues stopping "life support" in patients who were declared brain dead (which in the US is legally considered a form of death).

    I presented with an incidentaloma and a fascinoma one time! CC was severe R side pain and they found a 13mm kidney stone on my left side. I was damn near handing out autographs.

    More slang: FLKs (flecks) - funny little kids High Five - HIV "Did it for the 'gram" - When your teenage/young adult patient just needs GC prophylaxis and discharge (probably deprecated now that the guidelines changed) Status Hispanicus - Either a large, panicked family, or an overanxious, typically Latina mother accompanying a patient with an innocuous problem that is easily defused despite the drama and cacophony of despair surrounding it. The Bakery - the area of your ER holding all the baker acts because inpatient psych is full and transfer/transport is taking forever. Basically, the psych rock garden.

    Dr. Strong, thank you for this video and I love how understanding, accepting and respectful you are in this video when discussing what intrsex is (which is a sex as sex is a bimodal and infinite [in its own ways] spectrum of its own), what transgender is, and what sexuality and gender expression are. This is helping me with unpacking my internalised transphobia (as I'm a transmasculine man of his own genders who's struggling with himself).

    Fun watch coming from a prehospital medicine setting. Heard some of these but bot most of them Dumping i have definitely heard as frequently directed towards EMS. At least in the US it's tricky. Many of our patients do not warrant an ER visit, but medicare and medicaid generally as of yet do not pay iut for treatment in place by paramedics or transport to alternative destinations like urgent care/FSED, pt's PCP office, acute mentalmealrh or addiction crisis centers, etc, when many of our patients would benefit significantly more from these services than being "dumped" on emergency. Zebras, especially "Horses not Zebras" is one we get told about, mainly in the context of prioritizing looking for immediate life threats in medical patients before considering non-emergent causes of their symptoms. As with all good things, it can be misused though, and medics who have what i like to call a "knowledge defecit awareness defecit" could very well write off a patient's symptoms, simply because a paramedic's education is so heavily skewed toward acute care

    Now with all the upcoming chatbots and Ai systems like " Claude " which can analyze pictures now and even ECGs ( and I'm talking about the free version not even the paid one - now there's Claude -haiku and other versions) … it's developing too rapidly right now and with the right prompting you really can specialize the chatbot and get it to solve clinical cases efficiently. so GPT 3.5 is something from the past now compared to recent more sophisticated AI. - Now medical students can even use this to not only solve cases but to make the AI come up with cases so it's a sort of a training for them - Would be great to see a comparison between gpt 3.5, 4, and Claude using the same clinical cases !

      I'm part of a large multiinstitutional research group looking at questions like this. As just one example, last year we compared GPT3.5 vs, GPT4 on cases similar to the one discussed in this video. I talk about the results with some of me fellow authors here: czcams.com/video/eB4gxT2ciTM/video.html (The tl;dr: GPT3.5 < students < GPT4) We've since moved on to comparing doctors without access to AI vs. doctosr with access to chatbot vs. chatbot alone to "solve" real life cases and make complex medical decisions based on real cases. So far, somewhat disturbingly, regarding making diagnoses, chatbot alone outperforms doctors using a chatbot as a tool: www.medrxiv.org/content/10.1101/2024.03.12.24303785v1.full.pdf I think the takeaway is that simply encouraging doctors to use these tools is not a great strategy. Doctors need to be trained to use them effectively. However, specifically regarding the interpretation of ECGs, that's lagging behind the interpretation of text.     "Supratentorium" can be ruled out because it's probably not technically correct (the worst kind of correct). Cerebellar lesions have increasingly been correlated with obsession, anxiety, and illogical thought processes-Schmahmann 2010 has a great review. Everyone thinks of it as a motor structure, but its functions are pretty diverse, and motor symptoms are merely especially conspicuous. So since it's one of the perhaps minorly offensive or dismissive ones, maybe it can be reappropriated for an opportunity to lecture about cerebrocerebellar connectivity and all its subtlety.

    I’ve spent a lot of time in hospitals in the last 5 years because of the need to bring my elderly parents in for care. This video explained some of the (I thought non-medical) questions we would hear during triage in the ER. I was lucky to have lots of siblings and my parents had good insurance. I cannot imagine how hard it would be if I were alone. Elder care at home is physically and mentally demanding and a 24-hour job. The people doing it are often, themselves, over 60 and chronically sleep deprived. The “dad dump” is absolutely understandable as something someone driven by exhaustion might feel they have to do. I’m grateful I never had to do it, but now I see that, at times when I had to send one parent to the ER in an ambulance but stay home to care for the other-who could not be left alone-while I arranged care so I could go to the hospital, that ER visit might look like dumping. COVID also changed our sense of urgency about accompanying our parents to the ER. For at least a year, and then during surges, we simply were not allowed to stay with them. I do believe, because of hearing difficulties and my mom’s positive attitude (that is, she’d answer medical questions based on what she wished was true rather than what was actually true) they both got better care when they were accompanied.     I was a fascinoma once. It was fun! I had severe reactions to poison ivy as a kid. Age 18, I was having one. My whole face was swollen and bloody. In admitting, the nurse asked what the other guy looked like. I said it wasn’t a fight, it’s poison ivy. While in ER, everyone on the floor came by to see such a violent presentation. I explained to like two dozen students that this was the worst reaction I’d ever had, but not uncommon.

    I’m in uk 🇬🇧 at my wits end ! Had explosive uncontrolled loose stools for a year now! With episodes of incontinence! It’s horrific it’s like a complete blow out! Seems mucus to me is present. I just have lost about 2 litres in volume over 1.5 hours on my last attack. I was shaking and cold. It’s horrific I’m worried about going out or doing anything I’m 34 and brought nappies ! The healthier I eat the worse it seems to be! I’m not lactose intolerant- had tests. Not under any more stress than usual! I’ve been more stressed and had no symptoms. It is utterly miserable 😭
