The real reason that Captain can't lift the Mjolnir (Thor's hammer) in the age of Ultron he was feeling guilty all the time for the death of tony stark (ironman) father and mother that he didn't not tell tony that who killed his father whereas caption knows that the killer was his friend Bucky Barnes(winter soldier) that the reason he can't lift in the movie. In the movie CIVIL WAR in the end Zemo told tony that winter soldier was the one who killed your parents .after that caption America was free from the guilt and he becomes worthy to lift the Thor's hammer and he lifted the hammer in the Avengers End game

Well one more reason could be that he felt mjolnir's grip in his hand but chose not to lift it in AoU to save Thor from embarrassment. Just saying.
At the end of the movie it was not the Captain America, he was just Steve Rogers. After he went back in time to return all infinity stones, I think he disabled his quantum realm tracker and decided to live life like a normal guy but with all his memories. So he went in past and he stayed there with Peggy and at the end, he came as 100 years old Steve Rogers who lived his life like he wanted. In other words, in Peggy's reality, he came out of the ice safely and never died.☺️

The real reason that Captain America was not able to lift the hammer in age of Ultron was that -:
He knew that Bucky Had killed Tony Stark's parents but he didn't tell him and kept it to himself all these years and this made him unworthy! (But Cap does tell Tony in the movie Civil war that happened after Age of Ultron and therefore he becomes worthy again)!
