Have been nursing 52 years. Retired from Med/Surg ICU after 20 years. 15 years in home health, taught CNA program at Job Corps, worked in LTC, did hospital and corrections nursing (Loved my inmates)!
Last couple years in nursing home/rehab, but it was such a toxic environment (and I am too old for that "shite") I quit. Didn't want that toxicity to rub off on me!
Now, I am about to begin my new job, Home Health again!
I know you did the math, and yes, I will be 72 this month. I am proud of my age and nursing experience!

This is probably one of the most captivating informative videos I’ve come across. Thank you for your attention to the details. And that nursing brain example!!! I am trying to transition into Home Health after working in the hospital for almost 6 years. Everyone requires at least one year of home health experience. Any advice on how to get my foot in the door without a years Home Health Experience. I did run a COVID testing and treatment site. We had a location in which refugees lived in hotels and I did care for them in that setting, though I don’t know if it would be considered home health even though this is where they lived!?
Hi Tamika! Thanks so much, I’m happy to know you found the video helpful. The first I’m going to say is just apply. Apply to as many HH companies as possible. Go on interviews, and gain tips from the interviews. Most places will interview, and higher you even without HH experience.
What state do you live in? Are you a LPN, or RN?
Another thing you can do is become familiar with the OASIS. Especially if you are a RN.

I am flabbergasted, how do you get thru 4 SOC before noon? We use HCHB (home care home base) and I am unable to do anything ahead of time before opening the visit and starting it. I do use a cheat sheet, but for me, 3 hours for a SOC would be a record, and for 4 visits, even in town, I'm going to spend at least an hour driving. I really think that if the medication list, and diagnoses were entered ahead of time, then I could just reconcile that and make changes as needed. Here we are, all on electronic charting, and none of our computers are communicating with each other, so we nurses have to do the tedious work of data entry along with everything else. I'm hoping to pick up some tips from you.

Do you think it’s possible to work 2 HH jobs? In case one is slower than the other? Trying to make more money, but I already work one FT Home Infusion job. Some days I have zero patients, but still get paid for 8 hours work.

Hi, im also a home health admissions nurse but I do 1-2 admissions per day. I could never do even 3 admissions in one day. How long does each visit take you?
@monikaroseRN Thanks for watching. The max that I will accept is 5. I consider if the admissions are all Medicare, or if some are private insurance. I complete the average SOC in 45 minutes. I would say it takes a lot of practice and being very familiar with the OASIS/Assessment Skills. Do you only complete admissions? How long have you been doing Home Health?
@@AccordingToTawanda I get paid salary so I don’t complain when I only get one admission for the day. It takes me about an hour, sometimes 1h15mins or even up to 2 hours if it’s a complicated patient. I rarely get ortho patients, I have a mixed bag: CHF, COPD, falls, wound care. I also like to start my day early but I rarely get anyone to accept 0800 visit, so I usually start at 0900. Charting takes me about an hour per patient. I don’t manage patients but occasionally will do a routine visit. Thanks for your videos. I find those very informative and interesting to watch from a fellow admission nurse :)

My company does not require vaccines. However, it varies from company to company.

@AccordingToTawanda hi. 🙋🏽♀️ This is the easiest job that I ever had. I spend 89% of the day, sitting down. It can get kinda boring at times, or a day might go slow. I wish that I made more money, but I'll be asking for a raise soon, and taking on a few more days throughout the month. I was supposed to do 2 days a week, but I was asked to do 1 more day, 1 time, and so far it's been my consistent schedule. So, now, I'm up to 3 days a week.

Hi Bart! We provide skilled nursing care in the home. Some patients have Home Health Aides to assist with ADL care. You may run into a patient that needs some type of personal care while you are there. However, the nurses main goal is to provide a skilled need. Great question, thanks for watching 😊

Sometimes 2 hours if the patient has a wound. We use this app that measures the wound but the downside of it is if there’s poor connectivity it slows us down big time

OMG! How did you get an employer who can give you 3 ortho, an PIV,? Judging by your referrals before I know your assessments: the last two sound like a possibility of complications, but otherwise you (well, I) might be done in 8 hrs. Is this your avg acuity? I heard you say "I'm doing 4 admissions & will be done for family time" & I thought you must negligent! people considering home health SHOULD NOT think this is a typical caseload. Ortho & IV patients don't really need any considerable case management. They're the simplest Dxs you'll see and 2 hrs is a reasonable time to complete them. Almost any other Dx requires more time assessing mobility, knowledge deficits, planning teaching needs for Dz process mgmt & prime pt/CG for each topic, family/social issues, I'd probably spend more time on the head to toe, drug reconciliation (likely to not be long list for ortho pts.), much more likely to require interdisciplinary communication (waiting on HOLD), more time thinking & writing narrative, much more time on entire OASIS, communication problems (from language to lonely & wants to chat), Wound care is ALMOST as easy as ortho, but the actual wound care & data entry time is increased.
I'd still be doing HH if I had your job. do you work for an orthopedics organization?
Hi, about 95% of our census is Ortho patients. Hips and knees replacement makes up the majority of our case load. I only complete SOCs, a different nurse does the follow up visits. When you are only doing one thing you really get a chance to craft your skill. Thanks for watching ☺️

I've never worked somewhere that had a dedicated admissions RN, but on-call nurses would transfer some SOCs if they had a full case load. I do not like doing this: As the admitting RN, having audited visit notes, I know that many nurses are task-oriented & do not understand the responsibilities of RN case manager so they will not follow my POC, Esp education. I don't like knowing that a good portion of my time spent organizing my pt's POC will be totally wasted. As the nurse accepting an admitted patient, you need to do A LOT of unpaid work just reviewing the SOC (IF RN has even finished their documentation) & P&P so you can actually follow the POC. Regarding quality of care, your understanding of the POC will never be as through as if you had done the assessment, you miss the opportunity to really introduce yourself the the pt & build rapport, & rapport is further damaged by the inconsistency of sending out a different person as if they are interchangable widgets. IMO, SOC is criminally underpaid - The only reason it's worth doing a SOC (excepting ortho & IV) is that the extra time you spend in the visit and on the computer is compensated by the shorter and more focused re-visits. *I say criminal because I know we are billing $500+ per visit yet corner cutting runs rampant and many clinicians just ignore work that isn't clearly measured in our documentation because we simply are not paid to do our job per medicare guidelines.
I'm just starting to explore the world of HH Vlogging and I am surprised I'm not hearing more assessments of systemic problems. I'm surprised to see so many videos like this one that only talk about the skills & strategies for the individual clinician to be successful but with no mention of systemic and fixable issues that increase HH turn-over and impact overall HH quality of care. Participating in the change of health care at the systems level is part of our scope of practice, yet we seem to have internalized the idea that industry obstacles are entirely the responsibility of the individual to overcome. The implication seems to be that you're just incompetent if you can't do a FT case load in 40 hr/wk while the reality is that it is just not possible to meet our CMS dictated responsibilities in 40 hours if you're doing 25+ visits/week.
Your acuity level is highly unusual & I think you're doing a disservice & contributing to our high staff turn-over by failing to include a prominent disclaimer. I do not mean to attack you, I think this is an insidious & wide spread cultural problem. In addition to a high turn over rate and high industry proportion of new grad clinicians, this cultural attitude of "just work harder" leads to harming pts and real policy implications like the recent billing changes akin to the austerity measures of No Child Left Behind where we are treated like lazy children and punished financially for poor performance on arbitrary metrics instead of making obvious changes to to our working conditions that would enable us do our job AND participate in family time.
@@cartermclaughlin2908 I was approached by a former DON for this specific role. There is only two of these positions that exist in my company. Both of us were solicited for the position by a prior manager. We both have a very different background. I was a ADON and Clinical Educator. I won’t even go in all the experience I have with the MDS and the OASIS. They needed RNs that could handle the volume of SOCs the company has. We literally get 25-50 post-op Orthopedic referrals a day. The rest of the load is handled by Case Managers. That’s the company design. We specialize in Orthopedic Aftercare. I actually would never accept a job as a Case Manager in a typical Home Health organization.
