I'd long heard about Ecco's difficultly, so the version I played for the first time recently was on the Sega-CD. It was still hard, but that version adds mid-level checkpoints which enormously alleviated the difficulty. Still took me over a week to beat that last level, Welcome to the Machine. Brutal level design.
Can’t agree with some of the games on the list. Castlevania, Immortal? At the same time there are games like Alien Soldier or heck even Streets of rage 3 with the western upgraded bs difficulty
The hardest game I played on the Genesis was Battle Squadron. But I guess it's too obscure ... and nobody would recommand such an humiliating pain to their friends!
Pro Tip: For Target Earth grab a second control pad, hold down the start button while starting a game with the first control pad and BAM!!! You are now invincible for the whole game! This was the first 16 bit game I used a cheat code on when I was 11 yrs old! 😎😎😎😎

Good list but you have to make another part : Dark Castle, Shadow Dancer, The Adventures Of Batman & Robin, Sword Of Sodan, Streets Of Rage 3, Fantasia, Tintin In Tibet and many more games are waiting for you.
Games such as Dark Castle, Sword of Sodan and Fantasia shouldn't be on the list because their difficulty doesn't come from challenge but from bad controls, poor programing and (more often than not) hideous level design. Games in this video are difficult, but they're all fair and well made. I agree with the others that you have mentioned.
I played and beat 8 out of 10 of these games, with the two exceptions being Fatal Labyrinth and the Immortal. I'm not going to comment on those two because I haven't played them, but I think I have the right to rank the other eight by difficulty. 8) Ghouls 'n Ghosts. I think this is the easiest of the bunch; and that includes beating it on both loops. It's ridiculously easy and casual in comparison to its NES and SNES counterparts; both of which I've beaten as well. 7) Comix Zone. The game is very short, and is hardly challenging aside from its unforgiving lives/continues system, which isn't even a big deal because the game is so freaking short. It's basically just a button masher, with a few tough trial and error sections that require a bit of thinking. 6) Castlevania Bloodlines. It's a pretty standard Castlevania game with its difficulty, and is not exceptionally challenging like Castlevania III (US) and Dracula X are. It does technically have limited continues, but it also has a password system; so in reality it's the same as having infinite continues. 5) Contra Hard Corps. This is where the games start to get truly difficult in my opinion. Contra Hard Corps requires a lot of trial and error and pattern memorization, so if you want to beat it legitimately without the use of save states, prepare to grind it for over 10 hours starting the game from stage 1 over and over again. With that being said, the game becomes pretty easy once you know all the patterns. It lacks the randomness of Contra III, so in comparison it's a much easier game once you get the hang of it. It's definitely harder in comparison to the NES games. 4) Chakan. This game is not nearly as hectic or grindy as Contra Hard Corps, but it's also much less polished of a game and it's not always clear what you're supposed to do. It's a rather long game. Fortunately, it has unlimited continues. 3) Kid Chameleon. This game is long. The second half of the game is incredibly difficult and tricky to get through. The final boss is no pushover. Limited continues. 2) Ecco the Dolphin. The final stage is absolutely brutal. It probably took me longer to get through it than the rest of the game combined. 1) Target Earth. The only way that I could beat this game is to use a secret code that enables infinite continues. I can't imagine beating it with limited continues. This game is very random, so even when you get good at the game, a lot of times you're just at the mercy of the RNG. This is the main reason why I rank it as harder than all the other games on the list.
Comix zone was stunningly simple. I’m pretty sure I bet it the first the the day I brought it home. Some of the other ones here rent that bad. I beat castlevania over a weekend on a rental. Chakan was a bit of a grind though as I recall.
Revenge of Shinobi is harder than half the games on this list. It took me a year back in the day to master everything about the game but nowadays people just look at CZcams for a walkthrough and think the game is easy.
Contra Hard Corps had the lovely "made harder for Western audiences" feature, as the original Japanese version had a life meter where you took three hits before you died.
Ghouls and Ghosts has no business being on this list. Are you sure you're not confusing it with Ghosts n' Goblins for the NES because that is arguably the hardest NES game.

Chakan's game was extremely difficult than the rest of the games, it was so hard and crazy that I don't think the developer could handle this game.

Target Earth was such a unique game and the npc war happening around you adds so much more to the atmosphere.

Nothing on the Sega Genesis is as brutal as Chakan The Forever Man.
The arcade version of Ghouls 'n Ghosts was released in 1988 but the Sega Genesis version wasn't released until the fall of 1989.
