  • Continental 603 was a commercial passenger flight from Los Angeles, California to Honolulu, Hawaii on March 1st, 1978 carrying 200 people.
    Crash:
    Just after the Captain called out "V1" while rolling down the runway, the No.2 tire on the left main landing gear burst, this caused an overload which caused the destruction of tires No.1 and No.4. The Captain decided to abort the takeoff but the plane was going to fast to stop, the left landing gear collapsed backward and the plane caught fire before finally stopping 664 feet past the end of the runway, 4 people died of smoke inhalation.
    Investigation:
    The tire exploded after the recapping tread had separated from it, the Captain's decision to abort the takeoff after V1 speed instead of taking off was the main cause.
    Sriwijaya Air 182 was a commercial passenger flight from Jakarta, Indonesia to Pontianak, Indonesia on January 9th, 2021 carrying 62 people.
    Crash:
    4 minutes after takeoff from Jakarta the plane turned right and began a dive from which it never recovered, the plane crashed 19km off the coast of Laki Island presumably killing all on board instantly.
    Investigation:
    Since this happened so recently it isn't known what brought the plane down, the most likely cause as of now is severe weather or mechanical faliure.
    Dana Air 992 was a commercial passenger flight from Abuja, Nigeria to Lagos, Nigeria on June 3rd, 2012 carrying 153 people.
    Crash:
    17 minutes after take off engine No.1 shut down, the crew failed to check their quick reference handbook and the plane continued to Lagos, 16 minutes later engine No.2 shut down as well leaving the plane with no engine power. The crew attempted an emergency landing at Lagos but crashed short of runway 18R in a residential area killing everyone on board and 6 people on the ground.
    Investigation:
    Both of the fuel feeder lines that give fuel to the engines had breaks in them which caused a fuel leak leading to the engines failing, the Captain's poor airmanship, and refusal to consult the quick reference played large parts in the crash as there were many opportunities to divert to other airports.
    Air China 129 was a commercial passenger flight from Beijing, China to Seoul, South Korea on April 15th, 2002 carrying 166 people.
    Crash:
    During a circling approach to land on runway 18R, the plane descended into a wooded hillside, the wings were completely shredded with the tail separating, the fuselage broke up killing 129 of the 166 people on board.
    Investigation:
    The lack of Crew Resource Management played the largest role, during the circling approach the crew turned too late leading the plane into an area of elevated terrain, the ATC didn't know the plane was a large 767 and gave it an altitude of 700 feet instead of the safer 1,100 feet.
    Adam Air 574 was a commercial passenger flight from Jakarta, Indonesia to Manado, Indonesia with a stopover in Surabaya, Indonesia on January 1st, 2007 carrying 102 people.
    Crash:
    As the plane flew over the ocean the pilots started paying attention to a broken Inertial Reference System the aircraft slowly turned right in low visibility until it dived down and lost altitude, the crew tried to regain control of the plane but it broke up 1,000 feet above the ocean killing all on board.
    Investigation:
    The crew paid so much attention to the broken IRS they lost situational awareness. After the autopilot was inadvertently turned off the plane began slowly rolling until control was lost and it began to dive.
    Iran Air 655 was a commercial passenger flight from Bandar Abbas, Iran to Dubai, the United Arab Emirates on July 3rd, 1988 carrying 290 people.
    Crash:
    The U.S.S Vincennes was fighting gunboats in the Strait of Hormuz in the middle of the Iran-Iraq war, the Warship spotted Iran Air 655 on its radar and assumed it was a bomber aircraft coming to bomb it, after numerous failed attempts to contact the aircraft the U.S.S Vincennes shot 2 missiles at Iran Air 655 causing it to break up in mid-air killing all on board.
    Investigation:
    The aircraft didn't hear the radio calls as it was on a different frequency talking to another aircraft in its vicinity at the time, the Captain of the Vincennes had been unusually aggressive in his tactics and most of the crew assumed the plane was a fighter bomber without double-checking.
    Sudan Airways 139 was a commercial passenger flight from Port Sudan, Sudan to Khartoum, Sudan on July 8th, 2003 carrying 117 people.
    Crash:
    The plane had taken off at 4:00 AM but not even 15 minutes later the pilots turned back to the airport after engine No.1 failed, the plane was cleared to make an ILS landing on runway 35 but after the crew missed the runway and executed a missed approach they continued to descend into the ground with the plane disintegrating killing 116 people on board with the sole survivor being a 2-year-old.
    Investigation:
    The cause of the accident was the pilot's loss of altitude awareness leading to a CFIT.
