    After 9 years of fighting mdd
    I finally built the confidence to seek medical health im supposed to go on therapy etc etc and i really hope i get the help i need😮‍💨😭

    As a mdd pt with psyctoic episodes. I feel this is accurate and I feel like this helped me digest some features I was unaware of. I stay on my meds and have a few safety net strategies. But that is how I cope.

    I knew i was treated for MDD but at one point my psychiatrist told me that i need more drugs cause i developed psychotic features.
    I didn't understand at first but then i realised how scared i was to be outside,feeling that people looking and laughing at me
    It is so strange how i ended up here
    Worst part is that most of the drugs she gave me ,makes me feel numbed and jombie like😢
    I just want to feel normal again.
    Just normal.....

      Hang in there. Maybe your doctor just hasn't found the right medication for you yet. Someday soon, You and your therapist may have a breakthrough realization soon and you could finally begin to heal from trauma You've hidden; buried deep in your mind. Recovery is possible, and you will get through this and heal those wounds within yourself so you can finally feel like you again. Understand how much strength you have within you, that has enabled you to endure what youve gone through up to this point. You're a bad ass, and you're going to get through this. I believe in you

    when my first major depressive episode happened i was having auditory hallucinations. and when i told a counselor at my school, he didn’t really take me seriously. so now i’m only just now re-thinking my diagnosis, b/c i have the MDD diagnosis but not with psychotic features and it’s mostly bc that made me feel like i shouldn’t tell my providers. that counselor i talked to really messed with me

    I have been taking anti depressive medication for twenty five years with a mental health diagnosis of Borderline Personality Disorder emotionally unstable type. However, it is not uncommon for me to relate to symptoms from a broad spectrum of other diagnosis

    Just curious, I understand that psychosis and depression can occur together, but if psychosis tends to be based on hyperdopaminergic states in the striatum how can you have anhedonia at the same time? Wouldn't the antipsychotic tend to further contribute to the anhedonia? I know what I'm saying is gross oversimplification but I would like your the thoughts on the matter. I know antidepressants with antipsychotics are effective I just would like to understand better how it fits together. Thank you.

    I have been on Abilify and Zoloft, as well as trazadone and klonopin for over a year. My auditory hallucinations have almost completely disappeared. I’m afraid to even bring up changing any med because it seems to be working. I’m still alive. Is there a point I need to reduce meds? I’m sure the voice will return if I stop Abilify. I don’t want to mess with what’s working.

    I have family members who are high on the narcissistic spectrum, who are clearly also depressed. They’re all delusional by nature, anyway, and with their unrealized goals (which are rooted in their false selves, and thus, don’t reflect their ACTUAL abilities, personalities, etc.), that leads them to disillusionment & subsequently, depression.
    They exhibit all of the delusional tendencies mentioned in this video, which makes complete sense for them to do it, since their sense of self-importance is high, and they’re thoroughly convinced that people would spend their time & energy stalking someone completely inconsequential to them. 😂
    In case someone reading this has people in their life who fit this description, hope it provides some insight. 🙏
    BTW, they’ll never change, since the narcissism is hard-wired. Those of you with narc family members, who have observed & experienced them for decades, some until death, can attest to this.

    I am diagnosed with gad ptsd and suicidal ideation and depression my psychosis lasted for months i don't believe my delusions were mood congruent but they are considering this

    My therapist (and previous docs) have kind of given up on figuring out which psychotic disorder I have... he puts it down as this since I've never really left a depressive episode and the mild to moderate psychotic symptoms have been there since I was like, a very young child. But it's not really mood congruent really.

      If the psychotic symptoms are not in the context of a depressive episode this does not really apply. I would say using an antidepressant in those cases might make things worse. Also when can think of primary psychosis as existing on a spectrum. People can have very mild manageable symptom while others have severe debilitating symptoms. Hope this helps

    I have recently diagnosed with MDD after my Fistula Surgery after seeing my first physiatrist he advised me to go for Psychometry, soon after the result I was told I have psychosis with x%0.5 Schizophonia with Schizotypal personality basically a personality disorder I have been on Peroxin HCL since 14 weeks with my second Physiatrist as per her advice i was diagnosed with General Anxiety Disorder with Major Depression but the medicine is not affecting at all. I am still very depressed and i don't hear or see things but I talked to myself and shout or scream to make my self-calm, I am very inspired by Josh Nash and Dr. Peter Bergin taking an antipsychotic medicine as it only harms rather than doing good. sometimes i feel i should try an antipsychotic like Brexpreprizole my life is hell every day with out any joy please help thanks Michael.

    I have 4 other mental health diagnosis but this diagnosis MDD with psychotic features is 100x worse than the others. It just doesn’t feel like you can even identify with people with just MDD. The severity is so alarming. I’ve been in this episode 14 days depressed and 7 days psychotic 22 days so far I’ve had 2 other psychotic episodes this year. The hallucinations get worse and worse the delusions get way more believable with each episode.

      It ended up being 3 months straight of psychosis I’m finally out of the episode and it had an uptick of getting worse.

    Hello Doc,
    I have psychotic depression since 2015,.my psychotic symptom is delusion and no halluc.
    I tried many medications wihtout succes.combination: ssri neuroleptika , trcycl antidepressants and neuroleptika...antiepileptika. i also have tried ect only 1 time but they must stopped because of my blood pressure increases under anesthesia...i tired also tms but no effect...what should i do? Can you recommend me something?

    would you lean towards a more activating SGA, like aripiprazole, vs a more sedating one, like olanzapine, given that we're dealing with depression here?

      That is a possibility, medications like aripiprazole and Seroquel do treat depression and are often used as adjunctive treatment. The issue I usually have with them is the efficacy is a little lower and they may not adequately address the psychosis. Hope this helps

    Doc- ⚠️URGENT⚠️Please explain how Buspirone along with Hydroxyzine would work for generalized+separation anxiety. This summer, I’m taking a one week trip away from home, then will be back home briefly (about a week) before going on another 3 week trip (the longest I’ve been away from my family). For most, that’s no big concern, but as someone who experiences anxiety when away from home and familiar locations, I’m worried about what will happen. Currently take Hydroxyzine when needed as well as an Olly supplement for stabilizing mood. Need to find a medication soon so it has time to come into effect before these trips. I experience occasional anxiety at home as well, but much more when away. Thank you!

    Hello, my son was hospitalized for observation. Dr diagnosed him with mdd with psychotic symptoms .. he gave him zyrpexia.. I’m freaking out about this as I don’t want him on meds but feel like I have no other choice at this point…

    Iv had social phobia since i was a kid and i have these issues my doc labeled perfect aparently .......but clonazepam and prestiq seems to maks the paranoia and dillusions go away

    Oh yeah I was diagnosed with this when I was 18. Now I am 25, still struggling with it but I think there are no psychotic features there at least. Unless I get drunk 😅😅😅

    I have mdd & my psychiatrist only has me on aririprazole & you stated it should be 2 medications?

      Possibly most people will be on an SSRI/SNRI and a dopamine blocker. There is some debate about use of antidepressants. In my experience I’ve found ECT to also be very effective and fast but this is something that has a lot of stigma surrounding it.     I’m suffering a nervous breakdown at 69, and haven’t slept in a week. The cause is my adult son’s abuse of me and my having to get a PPO against him. He’s terrorized my dog and stolen my clonopin. Not only have I been traumatized, but can’t get to the hospital because of my dog and the fact that he wrecked the engine in my car and I’m afraid to drive.

      We can cover MAOIs in general in video. This is a great topic that we need to talk about as I believe these might be the most effective of the antidepressants but come with a difficult side effect profile to tolerate and dietary restrictions.

    i recently got diagnosed with mdd with severe psychotic features & bipolar unspecified. and i didn’t fully understand why they did the severe psychotic features when i just told them i can hear my self conscious i guess in the back ground when i’m in my panic mode. i thought everyone could hear their self conscious. i couldn’t find anything or anyone that can relate to that. & now that i watched this video i don’t understand why it’s severe

      I usually don't consider an internal dialogue like ones own thoughts of voice a psychotic symptom. We all have this. I would not give a diagnosis of MDD with psychotic features and unspecified bipolar disorder together. It's probably one or the other and people with bipolar disorder can have psychotic features. I'm not sure where they were going with this but I would try to clarify the diagnosis. Hope this helps

      These questions are difficult to answer because I can’t provide any specific thoughts with respect to your case. What I can say in general decreased energy and loss of interest would lead me to select a more activating antidepressant like the SNRIs or bupropion.       ​​@@ShrinksInSneakersHey why are snri's a go to for you?. I don't think you have ever been on one yourself so you wouldn't know unless you listen to your patient's. Why would you prescribe a drug so molecularly closely related to methamphetamine ?
      I would like to know why you think patient's need to develop severe insomnia and balance issues that my sleep specialist diagnosed as a noradrenalin mediated syndrome causing 123 awakeni gs in 4 hour of sleep. Remember your oath to not kill people with your lack of experience. You can't bring people off snri's and tell them they need them for life because they can't endure the withdrawl. Doctors prescribe snri's with a benzo for that reason.
