In your video you were talking about Air med being on or off while at a scene. In my area here in south Texas we leave it on at all times just incase we have a critical patient and we need to move asap. Sometimes we leave it on because when they get on scene the patient is already intubated by medics and trage and ready to go. In my area if we have a MVA that is 15 min or more to a level 1 trauma center and there is more than 3 patients who are critical they will dispatch 2 med flights with a pilot, Copilot (paramedic) flight medic and flight nurse. And they can be in the air within 3 min of getting the initial call for medical. The main pilot will leave the helicopter on and will drop the medics and they get the patient and go back. Or sometimes the nurse and medic in the back will rappel with a basket if there is power lines, weather or other situations.
In my area of North Texas (Dallas,Fort Worth) we land all of ours in the middle of the road (running) no matter what Power Lines,Traffic,Weather or a grass field next to the scene perfectly capable of holding an AugustaWestland 109 GRAND helicopter 😃

Engine running/engine off: coming from someone who has actually flown in air medical helicopters; it can vary from time to time, patient weight is a HUGE variable. If the patient weighs a lot the helicopter must burn fuel before taking off due to weight capacity. Also if you’re inside of a medical facility talking to previous medical staff of patient you might want to shut the bird off if you might take a while due to saving fuel. Like I said it varies from flight to flight.

To comment on your shutting down the helicopter as a firefighter my self ive experinced both. The most reason is backwash where it throws debri from the feilds tgey shut thwm down because tgey are not stable or ready to be transported . the second is they keep them running because sometimes we have them ready to go when they land

Excellent flying ocean but I personally would've used the curve in the road versus where you landed it was clear of trees and looked clear of lines.... looks good though
Also ocean another reason they would keep the engine running on the chopper is if they want a quick lift off

By what im studying. Ocean is correct but also it takes up to 2 hours for the heli engine to cooldown before they can start it again even if u were only flyin for 10 minutes. I might be wrong but this is what I am bein informed by the guide's I am studyin.
Yeah, that's gotta be wrong. In my area, medical choppers shut down depending on the call, and about 20 or 30 minutes later they have no trouble starting up.

No real disagreement, it's all up to procedure depending on the helicopter company/department and how they operate their aircraft.

A med evac chopper will only shut down its engine if it is a new chopper equipped with quick start up engine
And again, also depends on your area. Lots of factors go into it, not just the age of the aircraft.

Can't do that. It creates desync in the server and causes havoc with traffic.
I would love to see this as well but as Ocean said it is hard... I can barely watch his videos even with HD can not imagine with the game. So I just take it as a volunteer station (that does staff one or two) plus I am sure there are some departments that do have only one or maybe two people in vehicles.

If you have to do another MedEvac, have State Police set up an LZ that's not right at the scene, due to some of the locations of the calls. Let's say a water rescue, pick an LZ in an empty parking lot and have EMS drive the PT to the LZ to Evac them.
Depends on the situation. Plenty of MEDEVACS will land right on the highway if needed. Depends on the area and policy.
I think the Dev of the rigs should update them to have the Traffic Stick to work.
