Why cant we just refine the edges and suture it without going for z plasty? Is there a significant difference in the outcomes of the two methods?

Obviously! The weight of the earring wil be borne by normal tissue... But in a straight line... The weight will be directly over a scar tissue which can get torn again easier than the normal tissues as in z plasty
