Neptune is more of a religious leader, his sea has multiple carved out nations by the creatures inhabiting it and Bikini Bottom is one of them. Bikini Bottom might just be a presidential city state republic, and every city naturally has a mayor to govern the city's internal issues. Not to mention that Bikini Bottom has its own navy since Krabs is a veteran, we dont know anything about the general army though.

Patrick’s relative solves everything about their government:
It was due to Civil War.
Patrick Revere rode not to warn of redcoats, but of rampaging mollusks. Gary’s royal ancestors were mollusks. Also, there was a power struggle between high and low society as noted in The Battle of Bikini Bottom (no not the video game). While the hand washing bit was untrue as pointed out by one of the re-enactors, some parts of it were true including the final battle where the low class won.
I call it “The War of the Mollusks”.
Once King Ameba took power by marrying the queen, the monarchy slowly began to weaken over time. Eventually the People adopted a democratic government, with the blessing of the reigning royal family. However, the Mollusk faction with connections to high society didn’t like this transfer of power. So after careful planning, they enact a plan to overrun the city with wild clams and rush in after to clean it up and become the saviors to be put back into power.
Thanks to Patrick Revere, it didn’t work. While many died, the people learned who was behind the Mollusk attack. Thus, war between the Mollusk Royalists and the Democratic Commons began. After the final battle, the mollusks were executed. The only survivors being those who weren’t sentient. Thus Gary’s line was preserved.

You are wrong in an important aspect, let me explain.
You said that King Amoeba got to power by marrying Queen Mildew. But why do you think that Queen Mildew was the head of state of Bikini Bottom?
Considering that the wife of a king-regnant is called a queen whereas the husband of a queen-regnant is called a prince proves that Queen Mildew became queen by marrying King Amoeba, not the other way around.

The entire Pacific Ocean as a geopolitical entity is governed by a monarch and his royal family. The monarch is currently King Neptune. The king's authority covers all cities, towns, and regions, including Bikini Bottom, in the Pacific Ocean, but because Neptune has more important things to concern himself with than the day-to-day administrative duties of the localities within the Pacific Ocean, he delegates his authority to mayors and other governmental representatives.
Bikini Bottom has both a president and a mayor because it is a city-state, similar to the real-life city-state of Singapore. At the local level, the democratically elected mayor presides over the affairs of the city. (There might also be some sort of city council, comprised of three to seven elected persons, who make up the legislative branch of government.) At the regional/provincial level, the president serves as the head of state. He represents Bikini Bottom in interactions with other localities.
Patrick and Gary are considered royalty in the same way that France still technically has a royal family, but that royal family has had no institutional or governmental power ever since the French Revolution. They're still recognized as royalty due to their ancestry, but they have no governmental power. Perhaps at some point in the past, Bikini Bottom was a princedom or fiefdom, and the family that Patrick and Gary descend from were its rulers, until the residents of Bikini Bottom overthrew the imperial government or until feudalism/monarchism simply faded away.

*My take. I humbly ask you to consider it*
I agree with you that the ancestors of Mr. Krabs propably used to rule Bikini Bottom long time ago but later lost their throne and became empoverished royalty, which forced them to become pirates (similar to Raubritter and other bandite-like nobles in the Middle Ages).
Some time later, King Amoeba became the new ruler of Bikini Bottom, establishing a brand new royal family. Because King Amoeba was the only member of his royal family to actually hold the title of king, the monarchy must have been dissolved during or after his reign, with neither his daughter nor his son ever wearing the crown. If we take a closer look at the royal family tree, we might get to see two of the possible reasons for the abolition of the monarchy:
*1stly,* the second born child of King Amoeba and Queen Mildew, Prince Callous, would propably have been the original choice to inherit the Kingdom of Bikini Bottom, because traditonally, the first born son of the king would be his heir. But Prince Callous “married” Prince Dingus, which makes him gay and excludes hin from the line of succession. But from this alone we learn that there must be or must have been other kingdoms neighboring Bikini Bottom, whose kings must have had royal sons like Prince Dingus. Both princesʼ son Carl might be adopted or-considering that Prince Callousʼ parents are literal primitive organisms-they arenʼt developed fish yet but hermaphrodite organisms instead, opening the possibility of them really being Carlʼs parents.
*2ndly,* Princess Tulsa, who would have normally inherited the throne after her brother came out as homosexual, seemingly fell in love with the jester Yorick and possibly married him despite her parentsʼ approval. This marriage was inappropriate to her royal status and therefore excluded her from the line of succession, too.
Those two reasons alone explain why King Amoeba and Queen Mildew ended up without a suitable heir, dooming the monarchy.
Because amoeba and mildew are such little and primitive forms of life, I always suspected this new royal dynasty to have come into power following the disatrous nuclear bombing of the Bikini Atoll by the United States. This would have explained the shift from complex life forms like the crab King Krabs and the whale Princess Pearl ruling the city versus low life forms like amoeba and mildew. The latter could have rebuilt Bikini Bottom after the Second World War and the US nuclear bombing tests, explaining why nost houses in Bikini Bottom are literal human trash rather than the small traditional houses with adorable architecture as seen in Dunces and Dragons or SpongeBobʼs grandmotherʼs home. In this process of rebuilding the city, Amoeba may have become a great leader of the people of Bikini Bottom and ultimately he might have been crowned.
You on the other hand date King Amoebaʼs reign before the Wild West and mainly cite the clothes of his granddaughter Maw as evidence. But to be honest, I think this is not that good of an argument, because in the countryside, especially in the aftermath of nuclear distruction, fishfolks might have still worn very old-fashioned dresses. This might have been especially the case for Maw, because she is very likely to have grown up in poverty, because her mother Princess Tulsa, after she eloped with her lover and left the royal court, wore old ragged dresses as seen in the royal family tree. Later in her life Maw might have becomee satisfied with her very old-school almost Western-like dresses and lived happily with her husband, Billy Bob, from the famous Star family, tracing their line all back to Patrick Revere.
Now we fast forward to the episode Rule of Dumb, where a starfish, dressed like a high-ranking servant of a king, shows up and erroneously declares Patrick to be the King of Bikini Bottom, only to later see his mistake and declare that Patrickʼs cousin, Gary the Snail, is the true king.
But that is most likely untrue again. Given that this starfish is confused enough to have already made a mistake in the first place by saying Patrickʼs the king, he might have made another error regarding Gary:
Garyʼs father Sluggo Star was the first-born son of Billy Bob and Maw Star. Their second son is our dear Herbert Star, father of Patrick Star. While the wife of Herb is noted in the family tree, no wife of Sluggo is mentioned. Now let us remember the full name of Gary the Snail: Garold Wilson, Jr. Do you already notice something off? Let me show it to you: Garyʼs surname is Wilson, not Star, meaning that his parents werenʼt married, making Gary a child out of wedlock, propably the product of an affair of Sluggo, which may be the reason that Miss Wilson, Garyʼs mother, isnʼt shown. It is possible that Miss Wilson could no longer bear the pressures and demands of motherhood after Sluggo Star left her with their son and therefore gave Gary up for adoption or even abandoned him. This would explain how SpongeBob became the owner of this snail. The father of Miss Wilson propably had the name Garold, too. All of this reveals that Gary, being a literal bastard child (no offense), is excluded from the line of succession.
    A quick summary so far:
If the monarchy of Bikini Bottom ever was to be restored, Gary and with him the most senior line of the royal family through Sluggo Star is exluded from the line of succession. To identify the legitimate king, we must search for the next most senior branch of the royal family, which we quickly find with Sluggoʼs brother Herbert Star, who obviously is still alive and the rightful King of Bikini Bottom, hooray!
There is another interesting observation with the royal family tree I want to share: When Maw married Billy Bob, she married into the House of Star. All of this coupleʼs consequently in wedlock born decendants are members of the House of Star, which was replaced the House of Amoeba as the rightful royal dynasty of Bikini Bottom. One if many parallels from history is how the Royal Family of the United Kingdom and Ireland changed from the House of Hanover to the House of Saxe-Coburg-Gotha with Queen Victoriaʼs marriage to Prince Albert.
Long live the House of Star!
    Hail King Herbert!

umm...Krab's family was never royalty, it was all a dream made up by Spongebob and Patrick when they were knocked out. Basically, this would eliminate the entirety of the Dunces and Dragons plot. So mid-way through Patrick's royal family tree, the monarchy dissolved and became Democratic, yet they still kept record of the family, incase they were to return to monarchy.

If you notice, Mayor Green has a grey moustache and the Food Judge has a blond one. Could he have dyed it? Perhaps, but because he doesn't recognize the Krusty Krew or the Krabby Patty he could be a doppelganger or Mayor Green's twin brother. Maybe that's why his rule is so inconsistent. Sometimes his brother takes his place.

So, maybe in the old days, the Monarchy ruled with power, while using the parlimentary system as a voice, until modern day where parliament uses the monarchy as a voice. In the early episodes including the film, The King had brutal punishments, unfair system and was completely insane about his pride, but the later episodes, his power diminished. Somewhere along the lines, things changed, as it became more democratic. Not only that, the whole mayor thing, there will be more than one and each one, has power and the voice to oversee different things. But yes, you never see the president, like most countries, you dont know they are there.
