This all looks like a solution that was in search of a problem.
Why does either Visa or the store merchant care where this money is refunded to? To which card it is refunded to? Isn't that the cardholders responsibility? He/she threw away the gift card it was charged to........ that was stupid.
Does it make any sense for a vendor to say to a customer, "... you've met all of our store requirements for a refund, hold on and let me see if I can get Visa's permission to give you back your money."?
Looks to me like another way to disappoint a customer at the POS/return counter at a time where people are avoiding the counter all together and buying on-line where this is all done with the click of a mouse.
I would be curios as to what brought all this on.

Let me know in what matter bank authorization code and merchant refund authorization code may differ from each other
