All About the Troponin Blood Test

  • In this video, we talk about a commonly performed blood test that is used to measure a protein called Troponin. This is an integral part of how our heart muscle works and is released from the heart at times of stress or damage to the heart tissue. The released protein can be detected by a simple blood test and is a useful way of helping to differentiate the causes of particular cardiac symptoms e.g. chest pain, shortness of breath, palpitations.     I went to the ER with sudden severe chest pains on a Saturday. They did an ECG and blood tests. My Troponin was 9 and the ECG didn't indicate anything unusual. They determined that I was not urgent, and my wait to see a doctor would be 8 hours or more. They didn't offer any additional testing during that time. I went home. The pain continued for the next 24 hours. I scheduled an appointment with my personal doctor the following Monday. My doctor told me that I had had a heart attack and sent me back to the ER. This time my Troponin was 11,355. That's not a typo. I finally got treated for the heart attack. I wonder how much additional damage was done from the 2 day delay in treatment.

    Good explanation I had test when I had my heart attacks so that was it I suppose then they put my stent in thanks for imfo

    Some Emergency Rooms do not keep the monitor with the ekg stickers on you. No wonder some doctors miss diagnose, and because no evidence at the basic protocal wasn't achieved you discharge patient without correct diagnosis.

    That was clear even for a non-specialist like me.
    I have a question: does the release of triponin and the consequent high triponon level serve any purpose in terms of health, in the same way as say a high temperature is an attempt by the body to make things uncomfortable for bacteria, or the way diorrehea can be the body's expulsion of toxins? Or is an elevated triponin level just a measure of the triponin which the damaged muscles are discarding, a kind of debris, so to speak?
    I hope my question makes sense.

    Excellent, troponin read 500 last Tuesday, had stent installed, some blockage of another artery, told to reduce LDL to level of 1.4, last Christmas LDL read 2.4 and my total cholesterol 4.9
    So eliminating fats n transfats, salt , bmi was25.6 at Christmas , already dropped 3kg in a week, i don't want to risk another heart attack. I wonder does heated olive oil break down to saturated?

      Do not heat over 180 degrees Celsius...HDL SHOULD BE UNDER 0.7....THE MEDICATION TO TAKE IS LIPTRUZET 10/10.
      .ONE PER DAY...WITH THE DIET AGAINST CHOLESTÉROL AND ABOVE ALL 30 MINUTES OF RAPID WALKING PER DAY

    I went A&E yesterday and they said mine was 6. They said this above the normal 3 level but sent me home. So I am confused

    Considering there are many ramping issues in Vic ambulances. Do you think the POC troponin test can be used in waiting ambulance pt who has chest pain and potential ACS.

      thank you for the comment. The issue becomes that troponin does take some hours e.g. 3-6 hours to become elevated in the setting of an ACS. There may be a role but I feel the focus on assessing patients with chest pain too often relies primarily on a troponin test rather than a good history and the ECG.

    I exercise at a very high level and couldn’t this also elevate my triponin? Why don’t doctors every listen to you! Had Angiogram and all normal!

    Mine was '6'. Just one over the safety limit of 5. A 2nd test an hour later was the same. What does that mean? And six what?

      6 ng/L. 6 is as safe as 5. 5 and below would mean they could discharge without a 2nd test (if ecg is fine and your risk profile is low). Only 30% of all humans have values below 5 ng/L, so you're fine. Even 50% of people that have values over 14 ng/L (99th Percentile) actually have an MI. So there is nothing to worry about specially since they did a second test.

      I had the triponon test exactly 13 hours after the fact of the event ...it was 18....whats that mean?????

      I had a speeding up of 95 at 1000 pm at night then followed by heart beats skipping crazily and lower throat pain took my nitro pill....waited then took bp......209/114 pulse 94

    Can you detect Angina with troponin?
    I have chest pain that comes and goes without any shortness of breath though, I’ve to the emergency twice within a month and all the tests done came back unremarkable.
    Should I be worried?

      Thanks for the comment, in general, troponin is a blood test only used to detect features of a heart attack. Angina itself does not necessarily make a troponin test abnormal so it should not be relied upon. There are other ways that your doctor can assess symptoms including stress tests and dye tests such as angiograms that may be required in some individuals. Please always follow up any symptoms with your health care professional.

      Thank you for your question, usually troponin is the more sensitive marker that remains elevated for a longer period of time compared to the CKMb that usually goes down within a few days and therefore can be negative when the troponin is positive.

      In my case, troponin i was positive 6 hour after pressure on the chest, after 10 hours in repeated, test, it was weakly positive and after that ckmb was done that was negative,... No other physical symptoms, no shortness of breathe, no sweating, no weakness, etc... Echo was normal ,anxiety problem,.. First Ecg, abnormal ,after 10 days Ecg was normal.. Sir please clear the doubt 🙏 age 22

    I went to the ER with palpitations. The ECG showed PVC’s and sinus arrhythmia. All bloods were normal except Troponin which was at 24. Doc had me do another blood test couple hours later to see if levels were rising. They had went down to 16. So he sent me home. Said the arrhythmias were quite common and not worrisome. Now I’m at home googling and read that event slight elevation of Troponin might be indicative of damage to the heart. Explain to me please why the doctor paid it no mind.

      Mine was 85 and I was sent home day 2 because there wasn't a known blockage found. I'm still worried

      Get a grip. Mine were 65 after a fall. Levels can drop after 7 to 10 days. Of course I am concerned but I had them high in 2015 during sepsis episode and they went down.
      These levels can taper off. Obviously.
      Troponin levels diagnosis are a point of contention between medical colleagues .
      Stress or anxiety can also raise your levels. Go figure that nothing is concrete when it comes to Troponin.

      ​@@stephanied6451just rest and get a retest in a few days... they do decrease over days.
      But if you feel dizzy or alot of pain then just go back to the ER. But your ekg has to be abnormal.
      I was not hospitalized..and sent home..but I suffered a bad fall and landed on my chest. 😢
      I just feel tired. You take care

      Different laboratories report troponin levels in different ways, depending on the types of assays with some reporting less than the lower range that is within normal limits
