2) Move the ball and stop dribbling so much.
3) Play inside-out.
4) Pass and cut ALWAYS. (The technical term for this is Pressure Release)
What the Nuggets have been doing:
1) Rushing their offense
2) Iso ball and overdribbling (heck as soon as a player catches the ball they dribble, they don't even look to see who is open)
