Positive Antinuclear antibodies (ANA) is the marker for patients with systemic autoimmune disease. These antibodies can help establish a diagnosis in a patient with appropriate clinical features.
Positive ANA indicates the greater chances of the presence of any of the following disorders
• SLE
• Scleroderma
• Mixed connective tissue disease
• Polymyositis/dermatomyositis
• Rheumatoid arthritis
• Rheumatoid vasculitis
• Sjögren's syndrome
• Drug-induced lupus
• Discoid lupus
However the test is more sensitive for the SLE , Drug induced lupus, Mixed connective tissue disorders. There is a minute possibility for the normal person to be having a positive ANA.
Different types of ANA’s are
• Anti dsDNA
• Anti ssDNA
• Anti Histone
• Anti Nucleoprotein
• Anti Sm
• Anti Ro
• Anti La
Positive ANA titers indicates the further testing the specific type of ANA
Indirect Fluorescent Antibody method is the gold standard method.
Patterns observed in cellular fluorescence
1. Homogenous (diffuse)-SLE, mixed connective tissue disease, and drug-induced lupus
2. Speckled-SLE, Sjögren syndrome, scleroderma, polymyositis, rheumatoid arthritis, and mixed connective tissue disease
• Nucleolar: Scleroderma and Polymyositis
• Centromere pattern : Scleroderma and CREST Syndrome

Currently in the stretch of figuring out that I don’t have SLE… however I have had consistent +ANA tests along with positive compliment panels and other + inflammation markers. But SLE is ruled out. Many many test need to be done to rule it in. One Sr will diagnose you with it and another won’t but it being some other autoimmune diseases. Very tricky.

what does it mean if you get 1.80 titer but none in Patterns
UpToDate: “The ANA test is positive in virtually all patients with SLE at some time in the course of their disease. If the ANA is positive, one should test for other specific antibodies, such as anti-dsDNA, anti-Smith (anti-Sm), Ro/SSA, La/SSB, and U1 ribonucleoprotein (RNP), which are described further below. In some labs, a positive ANA test by indirect immunofluorescence will automatically result in testing for such additional ANA that are often present in patients with SLE. However, a positive ANA must also be interpreted in the setting of other clinical and laboratory findings.
Almost 15 percent of the population in the United States has been found to have a positive ANA of at least 1:80 by indirect immunofluorescence, but only 10 percent have a true autoimmune disorder.”

1: 40 indicates that antinuclear antibodies are absent .
Titre values for ANA are represented as ratios . ( 1:20, 1:40,1:320 etc)
Titres means, The sample will be diluted to check for the presence of antibodies .
Interpretation of ratios : the higher the titer the lower the levels of antibodies
1: 20 or 1: 40 --- indicates no antibodies
While 1: 320 indicates presence of antibodies
1: 640 has even higher concentration os antibodies .
