    One key concern is related to the hemodynamic effects of nitroglycerin.
    Nitroglycerin is a vasodilator commonly used to relieve angina and improve blood flow to the heart. However, in the context of right-sided MIs, its use can exacerbate hypotension. This is because nitroglycerin primarily dilates veins and decreases preload, which can further reduce right ventricular preload and cardiac output.
    In right-sided MIs, the right ventricle is often affected, and its function may already be compromised due to the infarction. Right ventricular infarction can lead to decreased preload, impairing the ability of the heart to maintain an adequate cardiac output. Administering nitroglycerin, which further reduces preload, can worsen this situation and potentially lead to profound hypotension and cardiogenic shock.
