  • This latest video is a surgical demonstration of me setting up a Titanium Trochanteric Fixation Nail (TFN). This surgical procedure is done to repair proximal femur fractures. I wanted to take the opportunity to show you all my back table set up, surgical instrumentation and supplies used in the procedure. It’s been a few weeks since I posted a video but I’ve got a lot of content recorded and I’ll be releasing content consistently to kick off the New Year.       Our educator told us we’re allowed to wear them to set up in.

    Thank you again! It is always interesting to see how other places are doing it...
    But....
    Seems you guys still keep flipping things and disposables - totally against standards.
    I still see battery casings open for insertion - not in this particular case, but I observed that in your other videos as a circulator doing it right in the bowl - absolutely no protection shielding on it... Why are you having them together for battery insertion? It is very unsafe and highly likely will be a reason to break sterility as this is not the one-hand technique. Totally unacceptable.
    Also no towel under instruments during initial setup - the drape is often poked through and it a sterility hazard. While you are assemble long metal instruments from the TFN trays - where are you doing it? in the air or on the table? If on the table - there is not much space on it and also heavy TFN parts may punch the drape.... You placed the TFN instrumentarium directly on the table drape with sharp ends on it...
    Perhaps this can be taken to the educator or/and the Ortho charge nurse...

      Thank you for watching and your feedback. I spoke to my educator and we checked the AORN standards and it says in “Guidelines for Perioperatice Practice: Sterile Technique 5.5 - Present sterile items directly to the scrubbed team member or place securely on the sterile field. Items tossed onto a sterile field may roll off the edge, create a hole in the sterile drape.” So I do understand what you’re saying as far as “flipping things.” I think that with newer nurses it can be difficult to open packages but I agree that they shouldn’t be “flipped” but should be “placed” on with sterile technique if they can’t hand it directly to the scrub.
      As far as the protective sleeve for the batteries to be honest we’ve never used that here at this is facility but I will definitely mention that to the staff and see what everyone thinks about it.
      Lastly with my towels I definitely agree that with these bigger trays it’s a great habit to put towels down in case of micro tears and just to have that extra layer of protection just in case.
      Thank you again for your feedback. It’s easy to get caught up in what you do everyday or a culture at a facility and I appreciate you making me double check what I’m doing and ensuring that I’m doing the best possible job to maintain sterility for our patients.

      Hi my friend.. thank you for reply.
      In my facility we do not flip neither do not place items as it is unsafe. We made a policy about that and regardless the seniority no one is allowed to do both. Our practice is to open sterile packages directly to the circulator hands.... The only exception will be real trauma case...
      I'm surprised hearing you have never had any habit of shielding a battery casing. Actually all Synthes drills are coming with it and it is a matter of getting it from your vendor.
      I am gladly viewing your environment and appreciate your work... Keep going!

      You seem to be very knowledgeable. It is nice you sent the corrections. You spent a lot of time to respond several times. Maybe you can use that time to do videos of what you would do at your facility.
