@Dr. B Addiction Recovery I noticed the stigma around this drug when I was first prescribed Subutex for chronic pain. Even the pharmacists would take a second look at me when I had it filled. There still seems to be a misunderstanding about opiate addicts, in some medical communities. This medication is a very strong opioid, but with opiate addicts, we have a strong tolerance. What would probably be too strong for a normal person, may be fine for an opiate addict. In time, as more and more people are being prescribed it, the stigma should decrease. Maybe one day, it could be completely diminished. Keep up the great work!!

Cindy’s Journey, I agree. I take Suboxone for pain related to MS. When I go to the hospital, I feel like there is a switch in the mood of the doctor and nurses, when I bring up the Suboxone. Even when I state it’s prescribed for pain, not for addiction purposes. Not that it should matter. It’s sad.

I’ve been on suboxone for 5 years. 2 years I was doing heroin and selling it for the most part and the past 3 years I have been using it for the right reasons. It saved my life I wouldn’t be where I am without it.. it’s helped with my bad anxiety and depression very well..the reason I was relapsing was from the way i felt mentally and the guilt. Once I learned how to deal with the guilt is once I was able to stay clean.

I’m doing exactly what you’re talking about: micro-doses (500-700 microgram per day) of Suboxone for Pain.
A little unorthodox but increasingly the drug of choice for chronic pain. And my mood is much, much better as the decrease in pain restores functioning.
My main concern is tolerance. I started at 300-400mcg daily but now I’m at roughly 700 mcg. I’m worried that this may be too good to be true, because my depression, anxiety and pain are all being relieved by a drug that’s designed to curb opioid cravings.
The literature, as you mentioned, is very upbeat, but I’m paranoid about withdrawal and addiction.
Dose that make sense - especially when I’m taking one third to one half of the dose my doctor prescribed per day. Should I stop worrying so much and just take the amount that two, yes two, independent pain management doctors consider appropriate for my chronic pain? Both are also glad my depression is easing.
I have mini-withdrawals that are basically rebounds when the drug begins to wear off - more pain and more depression. I’ve been trying to “tough it out” - taking less then prescribed, trying to prove to myself that I won’t get addicted and miserable.
      Am I being irrational?

From my perspective it was similar. I started taking suboxone 1,5 year ago for my addiction treatment. What I discovered was, that I could finally be normal! I was much more calm and happy even comparing to the time that I hadn't been envolved in opiats. Unfortunately for me this effect started to decrease after 1 year of the treatment and now it's not that vibrant, but still I feel like I've never been better. But it has negative effects too. I feel like I've been procrastinating much and daydreaming (still it is an opiate). And I feel like I should be off it, because of a possible pregnancy in a future (now I cannot plan things like that). Thanks for the job you're doing here! I'm learning much more than from my daily doctors :)

IMHO tolerance will occur and a need for more to get the desired effect will happen. I suggest that you use the minimal amount to lift your mood so that you can attack the cause of the depression. Exercise, healthy diet and good sleep to get the basics out of the way then professional psychological help to get to the root cause of the depression and anxiety. If you've been cleared of any chemical imbalances I would give that a look into. I'm no professional and best of luck.

Hi Dr. B, I have just found this vlog from you and was soo happy bc I am on methadone 50 ml /54 yo., not ruptured brain aneurysm op in 1995, with born high blood pressure but taking meds, cocaine, benzo and alcohol in the past, ended 2003, but always used something. But I bipolar with loong deep deep depression period/ I am on methadone for 3.5 yrs and it works Ive not drank since then. Methadone has 3 main function for me> 1. Antidepressive> gives me energy and warm, 2. Painkiller, for my bones, muscles, joints. 3. Stopped my cravings. All. Even alcohol. So it will be a life-long plan for me. But if buprenorphine gives me that main 3 lifesaver effect for me, I could switch bc it gives me more freedom and now it would be easier to change it when I am on 50 mg. It will be more difficult as times goes by and my daily dose will be higher like 60, or 70 mg bc with methadone the tolerance grows and not for Bupi. I know u r craaazy busy, Im in Europe, both of them are available so doable but I would love to hear your advise, and everybody,s advice, experience etc. Hugs. Susan

Same goes for methadone with me. I went to treatment 10 times and my mood just would not stabilize and I relapsed every time. Couldnt stand that low almost depressed feeling. Once I got on methadone the depressed feeling stopped my mood leveled out (completely after one year) over time. The important thing for me wasnt that it happened all at once but that I was always getting better. 2 yrs in and I have my old self back after a 20 yr long battle of opioids and heroin. No i do not and never have gotten high off my methadone dose. I expected to though. The way they dose you doesnt allow for getting high they build your dose up do slow that unless you have the lowest tolerance ever (not to common when you land in the clinic) then you arent getting high. Yes some do (I'm not oblivious) but most do not.

Does low dosages of buprenorphine weaken painkillers. I’m taking belbuca 450 twice a day for pain and I noticed my 50mg nucynta I take twice a day I get less pain relief. I’m thinking of maybe going up in nucynta or switching painkillers. It depends on what my pain doctor wants to do. Also insurance only covers 2 belbuca a day but they only last about 6 hours for pain, is it safe to get the belbuca 900, and just take half of it 3-4 times a day? I like buprenorphine for pain but doc didn’t feel comfortable prescribing suboxone for pain and insurance only covers 2 belbuca a day and it’s so expensive out of pocket I can’t afford it. Sorry for all the questions, you’re amazing!

@@DrBAddictionRecovery i live in San Marcos Ca, my pain doctor is up in Orange County (I used to live up there), but he doesn't get very involved with me. I have only actually seen him once over 3 years (my first visit) All visits after that are with nurse practioner. I go into appointment, she writes out new prescription and thats it. Visit lasts 30 seconds or less..basically as fast as she can write our prescription is length of visit. I do remember my first visit with doctor and he said he doesn't like prescribing suboxone, but said he would either write suboxone for me or three 10/325 norco per day, I said suboxone. His office is all full of pro-opiate medication literature, (is just something I noticed) I'm not saying its wrong as I'm sure it is helpful for people, but maybe its just in the wrong place for me. When I was in hospital though, he did not want to get involved at all. Feel very much on my own. I don't know what to do anymore.

I have a spinal injury, while in hospital doctor came in took out his pen and started stabbing me in my feet pretty hard and asked if that felt sharp.. I said yes. He then started stabbing me in my forehead with sharp part of pen, when I went to push his hand away i felt my forehead and I was bleeding.. thats how hard he was hitting me. He left, said there was nothing wrong with him take him off all pain medication. I was in hospital for issues after an epidural procedure gone bad. They hit a nerve in my back with needle and it paralyzed me.. I ended up in hospital for about a month, including an acute rehabilitation hospital to learn how to walk again. Sorry for long comment.. just sharing how poorly I was treated when doctor saw i had been on suboxone. I felt absolutely humiliated.. they cold turkey me and discharged me. I have been explaining to my doctor for years that I am not in “treatment”, I take this because it acts as an antidepressant and profoundly affects my mood. It’s black and white. But my point is that I get drug tested and this is put on my electronic medical record and I feel stigmatized by forced “addiction treatment”. I only used opioids for 3 months so I needed help for the discomfort physically to get off it because your body gets used to it so fast. The problem is….I still use another substance that is tested that I get flak and frowns and the “failed” attitude on the required urine test to get my prescription. I feel like I have to give up my privacy just to get depression treatment (bc of 20+ years Rx resistant MMD). Stigma, stigma stigma. This is exactly my problem and I have to fight to gain any respect and get comments such as. “Partied last week, didn’t you?”. No, no good mental health providers on Medi-cal, no other medication works with few side effects except one that even my doctor feels is for treating drug addicts and I must accept this treatment in my medical record to get relief. Any doctors just prescribe it for depression?

    • @user-ni1bh6zb8i
      @user-ni1bh6zb8i Před 8 měsíci

In exactlythe same boat as you. I totally sympathise with your situation - your comments are a mirror image of the treatment I receive. I refuse to be treated as a drug addict so remain depressed after taking 2mg for 20 years (for depression). If I register as a drug addict the irony is that they would prescribe them for as long as a piece of string. It's a hard choice, to abstain from a drug that you know helps depression, but the alternaitve is I register as a drug addcit and as you say is 'forced addiction treatment with no respect for accurate medical records or privacy'. Most of the UK by now would know the content of my medical records, it's total abuse. I'm not prepared to go down that road anymore. As you say, it's black and white, nothing to do with drugs or addiction. I'm glad we agree, it's what brought me here. Best of luck finding a GP prescriber.

Doctors have no idea what they are doing with depression.. depression is caused by metabolic dysfunction, but they treat it with opiates or prozac; because they have no other tools in the their tool box.. you are right, opiates can and sometimes are used for depression, but you get accustomed to them far too quickly.. with prozac and other SSRI's you can get a craving and it makes you need it when addicted.. so opiates stop working (which, may or may not be addiction) and prozac makes you need it.. the problem is fixing the underlying depression.. that is what can easily be mixed up with addiction.. if only the medical profession were professional, they might actually understand biology rather than beating it over the head with a hammer..

I recently had a relapse on alcohol and went to Lion's Gate rehab in Utah and got detoxed from both alcohol and the second week Buprenorphine. I am now going on my 7th week off the bup and have had very depressing days where I lay down to take morning and afternoon naps between AA meetings. I am wondering if it takes two months or more to get back to normal on this low of dose for such a long time or if the bup was helping with my depression, or as you say could be confusing because of old age and change of work habits, etc. I have never had this low of energy and almost a feeling of nutting up when it comes to daily routine. So after two years of trying to taper or cold turkey I checked myself into treatment. It’s been about two months off and I get worse not better. My first 3 years of recovery I had my days sure, but for most part loved my life and recovery. Now I care about nothing. I relapsed after 3 years cus of a toxic relationship on kratom. Sounds ridiculous but it the liquid form got me high, caused euphoria and when I tried to stop I went into
Acute withdrawal. So what was prescribed instead of just trying to help me detox medically under supervision? Suboxone. I typed in depression after ceasing suboxone but this came up. Every comment is positive. All I could think is how are all these addicts taking as prescribed. This addict could not. Anytime I had a negative emotion I’d eat more. Subs had the absolute worst side effects I’d ever experienced. I just went cold turkey instead.

I just randomly found this video so I don't have any context. However I have vast experience with suboxone as a user on and off over the course of a decade and a half. I'm currently 2 years into this current suboxone journey. Suboxone gives me very bad depression and even worse anxiety. Yes, it helps both when you first get on it. For a few months you'll be feeling much better. However long term it has always made me very antisocial. I usually hole up in my house and only leave when going to work or doing chores. On the flip side, I'm a pretty social person when not on suboxone, and cannot stay home for too long or I get antsy. I have also consistently abandoned my hobbies (being out in nature in doing different activities) while on suboxone. I'm in a reddit group for suboxone users and the atmosphere is pretty bleak. I've shared this experience many times and always get several people saying "Wow, you just described how I feel exactly, are you me?"

In general, opiates/opioids of any kind tend to positively affect mood (this is a no-brainer). however, Suboxone is unique in that it's antidepressive effects come in part via the Kappa receptor. Other popular opiates achieve mood elevation largely via the Mu receptor (this is the primary receptor responsible for the sense of euphoria you experience when taking the more commonly abused opiates/opioids). The fact that Suboxone achieves mood elevation with significantly less euphoria and side effect profile is what sets it apart . I would even argue it doesn't create any real "euphoria". For individuals convinced that it give them euphoria, I tend to think its simply treating undiagnosed depression or anxiety that is then being perceive that as "euphoric". Believe me, if you have true depression and it is relieved instantaneously (which tends to happen when you take Suboxone) it will be perceived as euphoric. Also, another key here is "low dose".. Brain imaging suggest that doses of 2 mg or less you still maintains some degree of natural endorphin production. I would contend that at doses higher than 2mg you run the risk of numbing natural emotions to point real benefits could be outweighed. Then you get into Suboxone relief of pain (which seems to be largely ignored in favor of traditional opiates/opioids) that are truly profound given its side profile. A lot of people in chronic pain are also significantly depressed. Yet, there is still a significant stigma buprenorphine/Suboxone.... or maybe it's just a Suboxone brand that tends to be viewed negatively. Either way, buprenorphine/Suboxone could be argued to be a true "wonder drug".

It can save your life but doesn't help depression .
