    7:10 Hi, pharmacist here! Just wanted anyone reading this to know that an EpiPen is NOT the only thing needed during an anaphylactic reaction. While it does save lives by addressing the initial reaction, other treatments are needed afterward to ensure the patient makes it through the reaction okay. If an EpiPen is needed, please immediately call emergency services so they can provide the other measures needed. 😊

      I was about to comment the same thing. I have only ever had to use an epipen once, and the school called an ambulance. Thank god I am not severely allergic to anything anymore.

    Obligatory PSA for when I see anything about EpiPens: Remember to still get the person in anaphylactic shock to a hospital! EpiPens treat the symptoms of anaphylaxis for a short period of time, after that the person will need further medical treatment.

      It is also important to note that it is dangerous to take an epipen, nomatter the reason, a 911 call needs to be placed or at a minimum you need to be taken to a hospital ASAP after taking an epipen.

    There are 2 reasons why the needle retracts. 1) so you don't break it off inside you. As Dan's mum said, you're panicked because you feel like you are going to die and the careful insertion and removal of needles, as is done with other meds, is not possible. 2) it's a medical sharp and the withdrawal into the shaft of the pen prevents it from poking someone else.       There are other options in pens like these instead of retracting.
      My medical pens usually have a shroud come out when you are done, to block access to the needle. I then also fit the protective cap back before disposing of them in a medical waste container, but the cap is optional.

    I appreciate the explanation of where the bubbles were coming from after the first insertion of the EpiPen. A lot of people probably got frightened if they were aware of how dangerous it is to have air bubbles enter your blood stream, so I'm pretty sure you diverted a bunch of worried peoples' concerns. Solid call on doing such!!!

      That small amount of air would be harmless anyways. It's take a rather significant amount of air to cause issues. Hollywood has made the entire world afraid of a few small harmless air bubbles.

      It also helps that epipens are intramuscular or subcutaneous (in the muscle or under the skin). Not directly into the bloodstream.

      Epipens are NOT anti-allergy. (There's probably a name for that but I don't know it.) They just keep the heart going so a person has time to get treatment for the allergy reaction.

      Epinephrine is adrenaline. It works by increasing the blood pressure and heart rate to keep the heart pumping since the blood is likely less oxygenated than usual. It also works to relax the airways to allow for easier breathing. Yes, the effective time span of 1 shot is 15-20 minutes on average and should always be followed by immediate medical treatment. Yes, epipens are not anti histamines and don’t stop the allergic reaction.

    With how the needle retracted afterwards, it almost reminds me of the needle on a sewing machine. And Dan's mum sounds amazing, enough so that, in my opinion, she could easily be one of the Doctor's travelling Companions. I'd like to imagine the length it took to have her actually respond was due to Tardis-level wibbly-wobbly shenanigans.

    I experienced an epee-pen shot without being in anaphylactic shock during a medical study and I can confirm that your heart rate increases whether you're panicked or not. It doesn't feel very good but you can feel your body's metabolism speed up. Your body temperature raises and you feel like you should move around. Unfortunately I couldn't get up because they had to do other procedures on me.

      Many injected dental anesthetics (like lidocaine) include adrenaline/epinephrine as a vasoconstrictor, and you can definitely feel its effects even in those relatively low doses of probably less than ⅙ of an Epipen's dose. (~50mcg in a whole single-use vial of lidocaine vs 300mcg in the EpiPen.)

And it's still an SI measurement; it's just the symbol that's swapped out, because it's much harder to mistake "mcg" than "μg" for "mg" (especially in handwriting) and accidentally have someone be given 1000 times the dose. The "mcg" symbol is definitely used in the UK, too. That said, the Epipen's dose in the US is listed as 0.3mg, which I suspect is pretty common for integral multiples of 100mcg, as it's even less difficult to confuse. But I wanted to use the same unit in comparison, so I converted.

    So cool to see up close and in detail. My son was allergic to nuts, milk, gluten and during a provocation test in the hospital had an anaphylactic reaction. I was told by the nurse to use a pen like this (call it a form of training for the parent). The cool thing is you can even use it through clothing, like jeans (the ones we have at least). The reaction subsided fairly fast (just minutes) and after about 30 minutes he was completely fine again. It is a fantastic invention and lifesaver and he always carries one with him. We never did any provocation test after that though, as we did not want to put him through this ever again...

    Dan's mum is right, you get a rush of adrenaline (metabolized epinephrine), and then you get the let down afterwards. I ate a dinner roll on a British Airways flight from NYC (JFK) to Birmingham UK (BHX) 29 years ago and had to use my epi-pen due to ground walnuts in the roll. When I used mine, I did not feel the needle in my thigh. I pulled it back a little to see the needle and shoved it back in my leg. After being seated by myself, my pulse rate just sitting was above 120 per minute. It lasted about 30 minutes, then I fell asleep. After waking up, I had to assist the crew chief to fill out an incident report since it had been reported to the airline. My epi-pen did not retract the needle. The doctor at BHX had never seen one and asked if he could have it. I do believe it saved my life on that flight.

      I had pens like these to inject a prophylactic medicine against migrane. It only hurts as much as you can basically expect when a needle is pushed into your skin & flesh, but the thing with these is that you are actually supposed to hold them down for a while (like 20s or 30s or so) after it started to make sure they are all empty and the needle retracted as supposed...probably also that less material squirts out of the puncture hole if you were to pull it out right away. But there is a retraint in your body that after the pain starts wants to pull it out immedeately which would be the normal reaction, so you have to overcome that feeling and force yourself to keep pushing it in, even though it hurts. Challenging thing to do each time anew.

    Experiencing migraines all my life, I’ve done 1,000’s of these stat-dose pens of Imitrex. Eventually you can find spots on your body where you don’t even feel the needle. The older pens had so much force, they actually bruised the skin at the injection site. Thanks for the video. Now I know what’s happening inside the body!

    I have to take a CPR / First aid class every 2 years for my job, and it's getting more and more common for them to have "dummy" epi pens, this classic style and a really cool newer one called Auvi-Q. They let you practice the feel of using one without needles involved. I call the Auvi-Q one really cool because it has spoken instructions that will guide you through using it, like how long to hold at against the persons leg or wherever, and they sent our First Aid instructor a whole pile of them for free (he had to buy the other ones) and I think their actual epi-pens are a good bit cheaper than the old school ones too.

      IIRC Tom Scott did a video with one of the defibrillators, the new equipment that walks you through the steps is a great idea. Aside from the fact that plenty of people won't know how to use it, even if you do, when someone has just collapsed/is in shock/having a seizure etc. having the thing you're using just go "ok person, do this, then this, then this..." is super helpful.
