    I had them both and it was a pain I would not wish on anyone. Brutal. Healing took a long time but finally!!! I had the botox shot for the fissure, I screamed for a week ( I think it affected the hemorrhoids ) but in the long run, it did help heal the fissure. Thanks Dr. for your video's, I know they can help a lot of people.

    These videos have been so helpful! I am thankful I found these. I'm going to try some of these options. I've been trying to get more fiber lately, and I had read that you basically can't get enough. I'm glad you clarified that not everyone needs the same amount of fiber. I'm relieved. No pun intended. Lol.

    Hi Dr- so sweet of you to post all these videos. I have dealt with hems since my first baby 10 years ago and a fissure (and hems of course) since my second 7 years ago. My fissure got so bad last November I finally saw a doc who prescribed me nitro and I started taking stool softener, however I still have monthly flare ups. I am currently having a flare up with 5 days of soft poops but still daily bleeding and pain so just ordered Pranicura. Praying this will work -otherwise surgery!???

    Soft stool is the key guys, yes it hurts even more even if it is soft but gradually it will heal. And yes take your meds.

    Thank you for all your information. My hemorrhoid started to bleed profusely at work after wiping & i had to use tp as a "female pad" down there until it stopped. I was mortified!!

      The most important thing that helped me was baths. At least two a day.
      None of those sitz baths that fit inside the toilet (lots of pressure on the butt), a real bath stretched out in the tub.
      Witch hazel helps a LOT. Tucks and Prep H pads are nice at work but I would really suggest a large bottle of it at home.
      Also, watch your food. Diarrhea and watery stools can be just as bad as hard stools.
      When the fissures were painful I found it best to keep covered in Neosporin, but this isn't advised to do long term. So when the pain lessened in a week or two, less creams to let dry and heal. Treat it as you would a cut or wound on your finger

I developed hemorrhoids and a chronic anal fissure after get destroyed by Bali Belly diarrhea 2 days relentless days in a row. I’m now fasting, applying Auqaphor and Witch Hazel, and have full confidence that everything will heal completely after watching your videos❤The hemorrhoid and fissure are getting MUCH better 6 days into the fast.       @@nrtnrt6676 The fasting worked A LOT. It took 3 rounds of 1 week on, 1 week off. I’m still not 100% healed but it doesn’t hurt to poo anymore and there’s no hemorrhoids. I’m confident that at some point next year I’ll be 100% healed. Fasting has also positively impacted my life in a lot of ways as well. I highly recommend it!

      @gyanaranjan3268 Hi there! Pretty much. By the end of this year there will be no evidence of it ever happening. I'm now a huge advocate of water fasting and intermittent fasting. These videos really helped me too during a very challenging and vulnerable time in my life. I am actually grateful for having the experience ❤️💯🙏🏼

    Thank you so much Dr. Chung for the information! Your videos have helped me understand more about the issues I'm having. I thought I was suffereing from severe external thrombosed hemmorhoid pain. It was so bad I couldn't function and had to call out from work and I didn't eat for a couple days due to fear of having a bowel movement. I got the Hemorrhoid to shrink with lidocaine cream and stool softener, but I was still in excruciating pain. I think I also have an anal fissure. I am using the anal fissure oil from Healing Natural Oils and that has reduced the pain significantly and (hopefully) started the healing process. Time will tell.

    I switched to a diet of mainly fruit and vegetables. It caused the fissure 😔 making sure I drink lots of water now

    Metamucil powder saved me and fiber gummies .. try to avoid taking so much laxatives or stool softeners it worsened the ibs for me

    Honestly it's soo painful every single day, it feels like the pain increases with every stool, despite having a soft stool, so frustrating, but your videos made me feel so much better, gave me some hope that it will be better one day.

    Exactly,external hemerroids and anal fissures and a few,you have to look in the mirror,and open up the the hemerroids and you can see red/pink and these little holes and bleed sometimes but the painful to open up the hemerroids soo feel for you

    I have bad hemorrhoids after giving birth to huge babies (last one was 11 pounds😭😭😭) ive been having bleeding on and off for 2 years now 😭😭😭 had a colonoscopy my doctor couldn't see anything apparently. I need to find a new doctor. Some times it goes away for a while. Then the bleeding will come back. Currently nack for couple days. Ruining my quality of life I'm still so young 😭😭😭

I have a question : Do you know why even if we do not really feel the inside fissure anymore, or much much less than before, there is a burning sensation which stays next to the skin tag that is left (but it is like on the outside part of the anus near the skin tag)? I hope that you will understand my question. And still feel like there are cracks (micro fissures) outside. Thank you.

    I just don't understand one thing. sometimes it hurts so much that it is almost impossible to endure and it is impossible even to make elementary movements. But after a few days everything passes, and there are no sensations for months, as if nothing had ever happened ..

    Due to IBS and pelvic floor disorder, I've experienced all of the above. I've lived with hemorrhoids, fissures, abscesses, and fistulas. Aren't I a lucky guy? I've tolerated setons and a fistulotomy, but what I never want is hemorrhoidectomy or THD. Both of those sound horrible. And some of these docs even outright refuse to give pain medication after these procedures! The meds don't bind me like they do to some people, and I don't use more than I need to. But certain doctors don't even care. Luckily, mine were decent enough to allow me to have some.

    Doc I saw a new GI dr this week and he prescribed me nitrous glycerine oxide in a cream with lidocaine. Are you familiar with this treatment?? He didn't do an exam just based on my history of pelvic floor dysfunction, hemmheroids and chronic pain :(

    I had hemorrhoid banding in 2th March then 10 minutes later I was feeling strange thinks in my bump. Like after the gym you feel muscles pain. That time my doctor did 3 hemorrhoid banding in same time then 3 weeks later 2 again.
    When I had to go to toilet I always feel something bad along my left leg as well like a nervous pain and I had a plenty blood as well. I got rectogesic cream after first banding because my doctor said maybe I have fissure as well, but definitely I never had before and I think I got it when he did first branding.
    5 days before I did a big change for my eating and daily habit and since 2th of Marc last 4 days my bleeding almost gone and the pain after the toilet not the same like before. I hope I will be again the same person before the banding which normally wasn't very important and made my life worst.
    My question is can I go to the gym and do any exercise before this fissure will dissappear? I normally do walking only, but because I reduced my food a lost 5kg in the last 3 months and I lost a lot muscles 🙃

    Hi Dr. Chung! Could you provide any suggestion on the order of applying diltiazem and nitroglycerin? I have both cream (am also suffering from fissure and hemorrhoid) but I am not sure about when to use which cream. Before BM or after BM? Also, if I am no longer bleeding, should I continue using both or just one or the other? Does nitroglycerin help with hemorrhoid? Will appreciate any suggestion! Thank you!

    Im so glad i watched this video. I was wondering why I've had such a massive headache since taking the nitroglycerin ointment for my hemorrhoid flair up and anal fissure. They said apply 3 times a day but im applying 4-6. Would that be too much??

    One of the things that your channel has made me think of is the low-fat diet. If you don't have some fat in with your bowel movements that's a lot of friction on them hemorrhoids and everything.

    What about surgical options for treatment? All those creme's just mask the problem and don't resolve then. Not to mention with all those cremes, your underwear will stick to that area because cremes are sticky and then you have a mess changing you underwear two times a day or rushing into the shower to wash all of that stuff off because cremes can cause itching. I know all of this from experience and that's why I would have loved to hear surgical options to permanently fix fissures and hemorrhoids together.

    Itching is not a problem co existing with the conditions like hemorrhoids, fissure etc. Itching is a side effect of the conditions mentioned.

    I had internal hemorrhoids, external hemorrhoids, fissures, and anal fistulas. I'm 2 weeks post op from my hemorrhoidectomy surgery. Fun stuff! Great channel. :)

      @@enesalimadhi5998 me too 6 weeks post operation - grade 3 haemoroid + chronic fissure. After pooping in the morning I am getting severe pain for the next 6 -8 hours still now. Not healing at all

    Dr.Chung I was diagnosed with an anal fissure about 4 weeks ago. Now my pain have Ceased since I been using the diltilzam cream. But I am still having anal bleeding after every couple of bowl movements. I was wondering if that is normal?

    What is the procedure called when they go in and surgically repair a perianal abcess cavity??...4cm deep...Been 3 months of slow healing.Been packing it and keeping clean but is healing soooo slow. Was 6cm at first..Wondering the medical term of this procedure.

    I had internal hems at stage 4 and a anal fissure very close to the anus that would not heal at all....and would split back open regularly after a heavy movement followed by flare ups that caused me to have to have time off work. It was some time before i realised myself (after researching) that i had an anal fissure also. My doc diagnosed me with hems only and so that is all i was treating.....for over 7 weeks....i thought i was healing until it split open again after a heavy movement...worse than before.....pure agony. So personally (through experience) i'd recommend stool softeners first for anyone tackling both of these issues. Then train yourself, like I had to, to poop without over stretching your anus. This is only really possible when using stool softeners. Cold compress after movements also helped. It only takes one bad stool to undo weeks of progress so stool softeners are a must. I use Lactulose.

      I agree. Softening the stool is key, but not diarrhea. And trying for once a day BMs. . The only laxative that has really allowed for little to no pain for me was Restoralax which is made with Polethylene Glycol 3350. Also Dr. Cheung’s advice to not keep wiping over and over w toilet paper has helped. Treating the area as the sensitive area that it is. Gentle wiping once or twice, then maybe a gently flush of warm water from a cleansing bottle and then only blotting with tissue. Also using gentle unscented no alcohol baby wipes. The key word is gentle. Extra advice: be careful not to accidentally burn yourself in the desperate measures taken to relieve pain with hot water or microwaved hot packs!

      My case is so similar to yours. I have been dealing with a fissure for the past 2 months after getting botox injected. I've been on a softner called Movicol - it makes the movement soft but the quantity that comes out during a BM is way too big for me and it often rips me open. It's also terrible to have to go anywhere else but at home because of the mess😒
      I have lactulose syrup - will be trying that instead of the Movicol for the next few days to see how it works.
      Would you mind expanding on how you trained yourself to go without overstretching?

      @@MrKassie78 The way I deal with my BM is to wait until the point of no return, if you catch my drift, sit down at the last moment, now, don't push on top of the natural muscle movement because that allows too much through into the area you don't want stretched which can cause more damage...like it did for me. Instead, now I squeeze in at the moment of no return, but only enough to reduce the girth of the stool that is naturally coming out of its own accord...remember, no pushing....this is where lactose helps at the beginning. That and a decent diet. I've had to reduce the amount I eat...not by loads but, I ate too much anyway lol. Once the natural muscle movement has stopped (even if you feel there is more inside that decides it doesn't want to come out of its own accord)...do not push it out. When I get to that point I stop and finish. Doing it this way might mean more movements for you but at much lower risk of re-ripping something that needs to heal.     May I ask : Can dosage of diosmin and Hesperidin.(E.g daflon 500) help to treat acute / chronic haemorrhoid condition e.g internal piles or external pile(including thrombosed pile) ( heard it worked by reducing swelling (inflammation) & restoring normal vein function) .
    Can we take diosmin & Hesperidin daily and used for long term for prevention of haemorrhoid? Appreciate your advice thanks .

    Sir I had LIS and skin tag removal a month ago…there were frequent leaks in beginning now they have started to get less once or twice in a day or two…and I feel the area loose…sometimes I get scared if it’s a fart or poop as I am not able to judge like I was able to before… I am taking sitz bath twice a day after poop…
    I want to understand if this is all normal and part of recovery and when this all will stop and will I be able to have my normal life back and by when…I am scared to go out as I am scared I might poop in my pants… there is no pain for last 2 weeks at all but I am having big time anxiety and panic attacks…

    I was diagnosed with 3 internal hemorrhoids and rectal tear. I completed my first banding and was prescribed nifedipine ointment for the tear. I am finding a lot of conflicting info. on using nifedepine/lidocaine ointment. The ointment was dropped off by a deliery service and there were no instructions inside. Should the ointment be applied on the outside as well as just inside the anus?

      Nifedipine is a vasodilator to increase healing blood flow to fissure; Preparation H is vasoconstrictor to decrease blood swelling of hemorrhoids.

    Dr. Chung, as a Proctologist, when someone who has active fissures for 6 weeks internally (currently taking diltiazem for 4 weeks now) and doing BM and they feel the internal anal being so ITCHY and BURNING while releasing poop happening for a week now and itchyness and burning will disappear when finish pooping, why do you think that is? Is it because it's healing and it keeps opening while pooping?

    When l am feeling the pain for me to get better l will be feeling so cold even when it's not cold, so l put a warm blanket tight make sure l m real warm, the moment l sweat l also feel sleepy so that when the pain is gone

    Hey doc ! I had a abscess near my anus a couple of months ago it was drained but I’m thinking I might have fissures every now and then that bleed when I poop 😿 the abscess is still moist so I’m not really sure if it’s the abscess or the fissure but yeah that’s been my life for the past 3 months haha

    I healed after 5 days with your Pranicura cream,but the fissures came back again after a week beacuse of medium hard stool. Thats always the problem,it comes back😪 ! Do y have tips to keep the stool soft every day. I drink a lot of water.

    I had my anal fistula surgery last march 18, 2023 now is almost 4months and I will have next surgery this coming July 17,2023 since i have this first surgery until now I feel so much pain specially after poops, I have discomfort because of my wound there is liquid poops discharge and sometimes bleeding,if I take medicine it will helps me a lot to lessen the pain, sometimes I don’t know what the food I will take because I don’t want to feel worst pain after poops, theres a time I can’t walk properly because I feel like burning my anus and my intestines pulling down, Dr. what the best way to feel better specially after poops, please give me advice what I will do to deal with my condition, hoping you read this comment 🙏 and please more content video about anal fistula, hemorrhoid

    Dr. Chung, thank you for these videos. I just had a baby 2 months ago. First I had terrible coccyx pain for 2 weeks, then my perineal stitches from a 2nd degree tear started to come apart. After both of those healed-took a month, I began having terrible anal fissure pain. I am a week into using Rectiv, trying my best to have a lighter diet so I only go once a day, but it’s tricky bc I have to eat enough to keep up my breast milk supply for nursing. 🤦‍♀️ Basically, I eat a lot of nuts-high calorie/ low volume. It’s still incredibly painful when I use the bathroom, but I take a warm shower afterwards every time. I’ve watched several of your videos and I have a question about the “angles” of the rectum and anus. For whatever reason, it is more comfortable for me to stand to empty my bowels. If I sit or use the squatty potty, I feel like I’m stretching my anus further apart. Standing with my tailbone tucked under slightly is the only way. I also grip a few hair brushes in both hands to short circuit the pain receptors in my brain so the pain signals from my bottom aren’t as intense. Got it down to a real crazy science over here. What are your thoughts on why it’s more comfortable to stand?
