I had this problem. A weird way to stop the gag reflex is to put one of your hands on the corner of the countertop and press down putting the corner pressure right in the center of your hand and hold it there while brushing. It truly keeps you from gagging and it is the weirdest thing that it actually works. It doesn't require enough pressure to hurt, but just enough pressure right before it is painful. You'll be surprised.
