Kyphoscoliosis, a real challenge for PMBC.

30y old female patient with kyphoscoliosis and severe MS.
trans Septal puncture and PMBC was difficult due to distorted anatomy.
Septal puncture was done in lateral projection and needle was tracked down at 6 o clock position quite unusual than in normal anatomy and punture was done between 6 and 9 o'clock.
reverse loop used to cross the inoue across the Mitral valve and two inflations were given.
balloon size was 22mm according to pt height and weight.
successful result at end.
