    Dr. Goldring is telling the truth. Progesterone is needed for health. I used progesterone cream to heal my bipolar disorder and clinical depression. As he mentioned in his video PCOS is a cause of low progesterone. I was diagnosed with PCOS before bipolar disorder. I told my obgyn and psychiatrist the two conditions were related, but they assured me they weren't. This was in my teens. By the time I was in my 29 I was completely disabled. That year, God led me to the late Dr. John Lee's work on estrogen dominance. I restored my progesterone levels and was healed.

I have high blood pressure a lot of the time though I am really healthy, exercise moderately every day , practice intermittent fasting, and eat a well balanced diet. I love your videos! such great info. I have a question. Listening to menopause doctors on youtube one doctor was explaining that you can take micronized progesterone every night or you really only have to have it 12 days per month.. • @SimpleHormones
      @SimpleHormones  Před rokem +2

      Carla - Thanks for watching. Glad the videos are helpful. There are different progesterone approaches for sure. One approach is "cyclical," which is most often used for women who are still perimenopausal and are still having at least some periods. The idea behind that is that progesterone levels are naturally high for the 2nd half (roughly 12-14 days) of the month, also called the "luteal phase" and low for the 1st half, the "follicular phase." Cyclical progesterone mimics the way progesterone normally cycles throughout the month in younger women. Some providers use that concept after menopause. Most doctors have women take progesterone every night to help with sleep . . . every night! The disadvantage to cycling progesterone is that it won't help with sleep when you're not taking it. Whether you use 100MG or 200MG is dependent on your individual response. Many women try 100MG and it's not enough, especially after menopause when they're taking estradiol and they get breakthrough bleeding. In many cases, upping the progesterone to 200MG or even higher takes care of that. My wife takes 400MG. It's a good idea to "titrate" the progesterone dose - start lower and move up to find the right dose that allows for good sleep, low anxiety, good mood, and no vaginal bleeding. Daytime drowsiness may also be a problem if the progesterone dose gets too high.

    • @SimpleHormones
      @SimpleHormones  Před 2 lety

      The specific tests (called "assays") that labs use to determine progesterone levels vary substantially. It's not really fair to compare a progesterone test from Quest Diagnostics to another progesterone test from LabCorps. They use different methods and their numbers and references ranges can be completely different. That's why I recommend you have an experienced hormone specialist who has a lab they know and trust. That way, the provider can help interpret lab results and help you get the big picture of what's going on.

      @jaykaira9939 If you're in menopause, you'd want to have your estradiol level looked at, even if you're not experiencing estradiol-related symptoms. Optimal estradiol is extremely important for strong bones and heart risk, regardless of symptoms. If you're not in menopause yet, then progesterone is OK by itself.

      @liztowers2058 Sounds like stories I've heard from women in perimenopause. It's a complicated time that's different for everyone. Heavy periods can definitely be stopped with a progestin-only birth control. That's not actual progesterone but a chemically altered, synthetic progestin. Unfortunately, progestin-only birth control is unlikely to help with sleep, depression, anxiety, belly fat, brain fog, or hot flashes.

    Good video. I will say that over-the-counter topical progesterone has always helped me to sleep like a baby. I don't like the oral progesterone as much because it has to pass through the liver and can be one more thing the liver has to deal with. The topical progesterone will bypass the liver and go straight to where it is needed. I started using progesterone cream when I was 33 years old for bad cramps during my monthly cycle. After taking it daily for 2 months, I never had any more cramping as long as I used the topical progesterone cream on a regular basis. Some doctors will tell you not to take after menopause....BUT I totally disagree. I am 66 years old and I stopped using it for the past few months and I felt like I had a lot more irritability & anxiety and also started having low libido and low sex drive. After feeling like I was going a bit crazy, I started researching progesterone cream and learned that low progesterone will cause those issues in the body. As of today I started putting the cream on again. This time I am going to be using it daily to see if that stops all the issues I am currently having.

    I had breakthrough bleeding when I stopped 14 days straight tho, so now I take 100 mg oral bio identical progesterone the first two nights after I put on a new patch. For the other two nights before I re-patch, I use 1 click (25 mg) topical cream from a compounding pharmacy, plus 1 vaginal cap (from Amazon, (bizweike brand) to make almost 50 mg total on those nights.
    Because I am on as low progesterone as possible, I have a transvaginal ultrasound scheduled to be sure my endometrial lining is okay.

      @SimpleHormones  Před rokem +1

      My recommendation is to keep going on both estradiol and progesterone. They are absolutely safe. A week is far too short a time for your system to fully incorporate those hormones. The combination of estradiol and progesterone will do 2 things: 1) eliminate all your symptoms, including insomnia and 2) protect you from long-term health risks. It's possible that 100MG of progesterone may be too low for sleep but give it at least 4-6 weeks, then ask your provider to re-evaluate.

    Can too high levels of oral progesterone also cause pelvic cramping and headaches. I'm experiencing both 3 weeks in. My doctor said give it a little time to see if symptoms go away.

    • @SimpleHormones
      @SimpleHormones  Před 2 lety +3

      Those can be side effects of progesterone, but they can also indicate low progesterone levels in relation to high estradiol. I've always counseled patients that progesterone takes quite a bit of time to settle down after you start taking it. Your doctor is right to have you give it longer.

    Hi, Dr
    Im 45 yrs old, had a partial hysterectomy 15 yrs ago and have always had a light period since. In the last couple of years they've been irregular and sometimes skipping a month. I've been dealing with insomnia, joint pains, mood swings anxiety, pulsating ears, low libido ect. I also have hashimotos and thought this was the cause of my symptoms but my tests came back normal plus with hypothyroidism I've always slept more now I'm waking up in the night and not able to go back to sleep grrrr!! My symptoms with hashimoto's have never been this bad. My indocrinologist thought it sounds more like perimenopause. So went to gyno did hormone blood work for perimenopause. I couldn't do it between 3rd and 5th day of period because they are irregular and I skipped again. Tests came within range so she put me on 200mg of synthetic progesterone to help with sleep 14 days on 14 off. On day 7 of starting prog my period came and lasted 3 days. I'm maybe sleeping 1 hr more but it's given me nausea, anxiety and leg pain. Is this normal? Will it eventually ease off or could the dose be too high? She said she gave me synthetic because the bioidentical would be too strong for me. Sorry for the long comment I really hope you answer. Thanks for your great informative videos. Wish there were more doctors like you here in Europe.

      Thanks for watching! So progesterone in pregnancy is a bit different than in menopause or perimenopause. Your progesterone levels go through the roof in a normal pregnancy, way higher than you’d have when you’re not pregnant. High progesterone helps you maintain that pregnancy and women with low progesterone often have multiple miscarriages. So it’s a great thing to get your progesterone supplemented during your pregnancy if you’ve had trouble in the past.

I would suggest you talk with the doctor who prescribed that and ask for a dosage increase. I'm not so sure the progesterone is not working, it's just that you're not getting enough. Medroxyprogesterone (Provera®) does work, but it has some major drawbacks when compared to progesterone. It won't help at all with insomnia, depression, or anxiety. I recommend against Provera®/medroxyprogesterone.

      That's a low level . . . but it needs to be looked at in an overall context of all your hormones. That single test of one hormone is interesting, but you should really have a qualified specialist take a close look at your estradiol, testosterone, progesterone, insulin, and thyroid to get an idea of what might really be going on. they'll probably also want to take a look at cortisol and DHEA to make sure your adrenal glands are working right.

      That sounds like a somewhat low progesterone level, for sure. The important part is understanding the overall context of that level. When was that drawn? Are you still having periods? Was it a few days after a period? 2 weeks? Do you know if you're ovulating? Are you having other symptoms that correspond with low progesterone? Are other hormones (insulin, testosterone, thyroid) optimized or are they high or low? These are all questions a hormone optimization specialist will want to dig into in order to truly understand what's going on.

    But that menstrual chart that is in all the medical text books has a HUGE flaw. There is no unit of measure on the y-axis! The authors of the chart combined an estrogen chart in pg/mL with a chart of progesterone in ng/mL and removed the unit of measure. This is absurd because if that chart wanted to compare progesterone to estrogen, that progesterone line should be 1,000 times higher on the chart to be realistic.
    Look at it another way: if we use Steve Goldring's optimal level of progesterone of 10 mg /mL and convert it into pg/mL by multiplying 10 by 1,000, we would get 10,000 pg/mL. Now this value can be compared to estrogen because estrogen is measured in pg/mL. Estrogen's optimal level is 75-100 pg/mL. That makes progesterone roughly 100-133x more abundant in the body than estrogen! And it means bHRT therapies should be dosing much higher progesterone than what is commonly done in practice.
    This has been a COLOSSAL "aha" moment for me and it shocks me just how many misconceptions are out there when discussing HRT.

      This is a complex topic - one I may need to post some videos about. I can understand some hesitancy about compounding pharmacies. For the Endocrine Society (and other physician groups) to claim that compounding pharmacy is "unregulated" is completely false. Compounding isn't regulated by the FDA, but by state boards of pharmacy. It's exactly the same regulatory framework your doctor is under. No one claims that doctors are "unregulated" because they answer to their state board of medicine instead of FDA. Federal law and the FDA's own rules allow compounding of prescription medications, based on the right of a physician to practice medicine in a way that's best for the patient. There are lots of instances where a drug is available in some form in an FDA-approved product, but that form isn't right for a particular patient. For example, Prometrium® peanut oil-filled progesterone capsules. If the manufactured drug is not right for the patient, a physician has the right to prescribe a compounded version of that drug, as long as it's substantially different than the commercially manufactured product. I've been in charge of the compounding lab at a very large compounding pharmacy that is licensed in all 50 states. My pharmacy was inspected by the FDA (the WORST week of my life, by the way!) and passed their audit with flying colors. There are excellent compounding pharmacies and a small number that don't know what they're doing. My recommendation is to work with a hormone optimization specialist who has a relationship of trust with an accredited compounding pharmacy. At some point, I'll make a video all about finding a compounding pharmacy you can trust.

    I have a dumb question...why would nature have women go into menopause in the first place? If it were going to give women all these diseases without estrogen? I know women lose hormones as they get older because the childbearing stages are gone, so isn't it only natural for women NOT to have estrogen/progesterone as they hit these non-childbearing years? For decades, if not hundred years, most women have lived long healthy lives without HRT? ESPECIALLY in nations other than North America. All of the older women in my family have either gone through menopause or are going through it now, but their heart health, bones, etc are all perfectly fine! They don't have HRT, they are mostly healthy eaters, and definitely move a lot, and I don't mean exercise...they're just always out and about or doing something in the house or work. So is it fair to say that NOT all women even need HRT in menopause? If their heart, bones, cholesterol, etc are good.

    • @SimpleHormones
      @SimpleHormones  Před 7 měsíci

      @Blueskies7775 Not a dumb question at all.
      One perspective is that "evolution" favors reproduction and once reproduction isn't an option, there's no real advantage to having humans live a long or healthy life.
      It IS natural for women to lose their estradiol and progesterone after menopause. It's also natural for most of those women to experience the physiologic consequences of losing those crucial hormones - hormones that helped them stay healthy and strong up until menopause.
      Congratulations! Sounds like you come from a long line of healthy, strong women. That's great and let's hope it continues into your generation and beyond. But it's also the exception.
      I'm not so sure that women have lived long healthy lives without HRT. It's a bit difficult to reconstruct the data, but lots of women didn't make it to 80 or 90 in the past. And lots of women in underdeveloped countries also struggle with major health consequences of menopause.
      There are women who don't experience the major health risks associated with the loss of hormones. But statistically, the risks are MUCH higher without hormones than they are with them. Your family is very blessed to have healthy women. But the overall population isn't always that lucky.
      My wife tells people all the time about her mother, Catherine. Catherine lived to be 101. She died about 6 years ago. She was one of those women who didn't seem to suffer from the long-term health risks of going without hormones after menopause. She never really had much in the way of HRT.
      On closer examination of Catherine's life, though, we realized that she spent the last 10 years of her life in a nursing home. She had been unable to walk since about age 90. She suffered from severe osteoporosis and her vertebrae were basically crumbling. She was in constant pain and had to have a lot of pain treatments, all because of her crumbling spine. I'm convinced that she would have had a much better quality of life if she had known about optimal hormones, which she did not.
      My own Mom is another case like Catherine's. She seemed to sail through menopause with no problems at all. But now, in her mid-80s, she's definitely experiencing some of the health risks that come with losing estradiol at menopause . . . especially cognitively.
      I like to say that hormone optimization isn't right for every woman . . . but the number of women who would not benefit from optimal hormone levels is WAY smaller than mainstream medicine would have you believe. The risks are exceedingly small and the upside is so great that it is the best pathway for the vast majority of women.
      Like I say in the video (8:34) "Whether you optimize your hormones or not . . . you face risks either way!"
