Hi. 2 days back i went to bls office to surrender my passport. In emergency details in form i filled name phn no of my husband. But they said there is a option of address which is missing in form. I said there is just one space in front of name and address so how can we fill nane and address both in one given space. They said you can write name of the person and thn comma , thn address of person. So they declared my form incomplete and ask me to come again . But they know very well there was no appointment available. so they said they would charge 45 dollar extra nd fill that form again. I had no option so i let them fill the form . So please fill complete information otherwise this could happen with you also.bcz i noticed most of people filled same like me and were paying extra dollars. Thank you

@@singh.nirmal yes deadline 3 years after you have to pay penalty bro I surrender my Indian passport at Jalandhar passport Seva Kendra it was so easy and I just fill the application form online and took some photo copies and both original passports that’s it
