  • The hip joint is made up of two bones, the pelvis and the femur and is known as a ball and socket joint that allows the upper leg to move front to back and side to side. The ball is actually the head or rounded end of the femur bone that fits snugly in the concave depression known as the socket. The joint is the largest weight bearing joint in the body and is surrounded by strong ligaments and muscle.
    If your hip has been damaged by arthritis, a fracture, or other condition, common activities such as walking or getting in and out of a chair may be painful and difficult. A hip replacement surgery will help by replacing the diseased and damaged portions of the hip with implants designed to restore the function and mobility to the hip joint.     Way back in December 1980, I was involved in a really bad car wreck resulting in several injuries including a dislocated right hip. At the time, I was 19 years old. The hip was "popped" back in place with no surgery required. Fast forward 42 years to May 2022, and I have a complete right hip replacement performed by a very skilled surgeon at the Little Rock Arkansas V.A. Hospital. The surgery went fine and now I live a pain-free life. However, my recovery was longer than most that I view here on CZcams. I returned to my 2-week evaluation using a wheelchair. Then, at my 6-weeks evaluation, I was on a cane. My last appointment was my 3-month evaluation, and I was walking on my own and still a little bit shaky. However, today, almost 5 months later, I'm doing really well. I've been working on building my muscle strength. I'm just providing this personal experience so that others may review. Not all of us are back to playing golf in a month or so. For some of us, it takes a bit longer. I think a lot has to do with the person's age at the time of surgery and physical condition prior to surgery. I hope this information helps others.

    I am 72 years old and have my right hip replaced twice and now it is due again for the third time . Each hip has lasted over 20 years so this should be my last . I am a patient at Wrightington Hospital , UK where the first hip replacement was first invented .

    Thank you, very informative. I'm 67 and I need my left hip replaced. I had open heart surgery this July for aortic valve replacement and aorta repair. Is there any reason I could not have hip surgery this coming year?

    I’m 57 and had anterior approach 4 months ago
    Back in the gym at 6weeks doing light leg extensions, hamstring curls and hacks
    Now I’m back to cross fit
    Doing dead lifts , squats , lunges , box jumps , burpees
    I feel 100% normal , quick string , flexible
    I will need other hip done next year
    Only thing I battle is tight hip flexors and tight low back
    muscles
    Any stretching suggestions with the new hip ?

    Good afternoon, Dr.Adam Wright ,I hope this message finds you well. I came across your CZcams channel while researching hip prosthetics, and I must commend you on the informative content. Congratulations on running such an excellent channel. My name is Israilton Silva, My father, who is 86 years old, underwent hip replacement surgery here in Brazil in 2023. Unfortunately, he had to undergo another surgery six months later to remove the hip prosthesis due to osteomyelitis. He is currently undergoing treatment, which may take over a year. After the treatment, he has decided against having the hip prosthesis reinserted, considering his age despite being strong and healthy.My question is, will he be able to walk without the hip prosthesis? I appreciate your attention to this matter.Thank you.

    Question: I was born with displaced hips. I never knew this prior to my five back surgeries. I only found out with the last back surgery while doing physical therapy. I've been to a surgeon, but he won't do my surgery until I lose weight. For over five years, I've been trying to lose weight, but cannot move enough to get the weight off. I currently weigh 220. I know my hips are worse than ever. My insurance wont' pay for any gastic surgery & my primary physician discouraged me from using the new FDA pills that are available. What would suggest? Thank you.

    Great info Dr. Wright. I'm 64 and just had a second Hip replacement on my right side and was done with the Anterior front. My first one, two years ago was done via the Posterior , side method. I seem a lot sorer from the front approach. Thy and side of thy are numb and sensitive. I'm told that the numbness should go away. Oh, and I'm almost 4 weeks post surgery...... What do I you think ? Thank you

      I had numbness after both of mine. It went away after a few months. Front approach for both. 4 years later and my hips are normal.

    Wow finally somebody talking about Perthes' disease . Thank you doctor for posting this.. I had this disease in 1980 at 10 years old I couldn't walk for 2 years had to stay on my back in a brace 22 hours A-day.. I was Able to be a fireman. But after 2 decades the wear and tear destroyed my hip. The head looks like a softball compared to the golf ball on my good hip.. I'm 52 now and petrified to get this surgery because a friend of mine went to Manhattan for a reshape of his hip Caught a blood clot & died @ 42.. I have suffered My whole life with this. I haven't been able to tie my shoe or put on my sock for over 10 years.. I went to HSS in Manhattan Spoke to a surgeon. Asked him how many of these a year he does. He said over 250 a year but You're hip I do about 3. My question to you is My question to you is. What's the difference between my hip and somebody else that needs hip replacement ?

      After talking with Dr. Wright here is his response to you Jim: "Sorry to hear you’ve gone through Perthes, and to hear the tragic story about your friend. Blood clots after surgery are unusual, less than 1% but still a risk.
      Perthes causes the hip to take a very unusual shape, both the ball and the socket are abnormal. The surgery is harder for a surgeon who is not very familiar with hip replacements, as the anatomy is unusual, but the recovery for the patient is still excellent. Patients who have suffered from this often have almost no motion at all in their hip. A hip replacement is a powerful surgery for them though as it restores motion and normal joint mechanics. The procedure often isn’t all that much different for people that do hip replacements routinely like I do. If your anatomy is severely altered by the disease, I have the ability to use other techniques like robotics to ensure the implants are placed in the perfect spot for you. I would not recommend hip resurfacing though, only places like HSS do this and for Perthes I do not think it would be a good idea. Plus, resurfacing risks metal on metal complications as well. Come on in for an appointment and we can look at your Xrays together"

Right now getting this surgery is extremely psychological for me.. Not being able to walk as a little kid devastated Me throughout my whole life... No one really speaks about this disease..

      Your story is exactly the same as mine , I have this disease and have had it since I was 10 , I had a replacement 5 weeks ago and am already walking on it , My other hip will be done within 6 months, carnt wait till it’s all over and there is no pain .

    Do you need to do any different or more exact calculations for ceramic on ceramic ...I've talked with surgeons who don't do it on principle or have only done one or two...one surgeon said the squeaking comes from incorrect placement in angles, etc...how do you insure yours work so effectively to the point that it has become your preference? and what do you think of dual-mobility installments? they say they only use them on people with tight backs...I wouldn't say I have a flexible back, but I do have osteoporosis...would dual-mobility be suited for me and does it come in ceramic also?? thanx

    Hello! Thank you for this informative video. A couple of questions with regards to activities following THR in a healthy, typically-active 40-something. Should a patient be able to sit cross-legged on the floor safely? Also, are activities such as jet-skiing, horseback riding, and dance fitness within the realm of possibility 3-months post surgery?       Hello! Sitting cross-legged on the floor should be possible with stretching, depending on the patients flexibility and approach. Higher impact activities like you mentioned can be done, but I stress patients be ready before going back 100%. 6months to a year is probably a wiser timeframe so the patient is really ready. I usually let people go back to practicing golf and tennis by about 6 weeks to 3 months, but have kept my martial arts patients out a little longer, sometimes up to a year before full fighting.

    Hello doctor - I live in Vancouver, BC, Canada and I had a right hip replacement exactly 30 days ago. I'm back at work and walking and exercising with light weights. However, where the incision is I have swelling but it is hard and hurts to push on. I also think because that area is swollen my surgical leg feels longer. My surgeon said he measured my leg twice and it should be fine. Will the swollen bump eventually subside? I'm 49 and had the Superpath method. Thank you for your time.

      Jon, these are all great questions for your surgeon. It is not unusual for your hip to feel long at first. Arthritis shortens the hip but we fix it during surgery. The raised areas are probably sutures that the body is trying to dissolve and almost always does without issue, it can just take several months

    I am 46 years old now. At the age of 9 I had surgery for perthes on my right hip and used crutches for 2 years for my left without surgery . Now pain is getting worse on my right hip. 2 questions I have. First one is how often femur fractures after surgery? That is my biggest fear if I get hip replacement. Second concern is would that last lifetime if I get it now?

    Great info. I had a right THR last October. I was progressing very well. Once I was given the OK to start working out and getting a little more physical I developed bursitis. I am now just a year over my surgery and I still have bursitis pain. I am 48 years old and I would say active before the surgery. I just feel like I am taking forever to be pain free. I have had two injection. One trochanteric bursa and the other in the psoas tendon. I did get some relief and if I take an Aleve I am pretty good. I don't want to take Aleve like a daily vitamin. Can this take this long to feel great again? I can swim, ride bike, sit, sleep, ARC trainer with no issues. I just seem to feel the pain with walking contact. I also have lost strength and can't carry heavy loads up stairs. Any thoughts?

    I was pretty into boxing and basketball until Dec last year when i started having groin pain. I scaled it back an cut most of my minutes in a coed basketball league until i found it was hip arthritis. Im 44 with a 4yr old at home. I just want to know if its possible to play the occasional "old man" pick up game and some light sparring after replacement? I may have to have them both done in the future. I dont want to be the guy who cant play with his kids. Is it possible to play ball after a dual mobility hip replacement?

    Hello Dr, watching your video, very good I was diagnosed with severe bilateral hip osteoarthritis with surgery criteria, the doctor says he does not know the reason why my hips are so bad at 48 years old, but since I am 40 I have symptoms that were misdiagnosed, terrible

    Iam 52 @6'2... I have had hip issues since I was a baby. Got told when I was a baby I was supposed to have a metal plate but mom voted against it.. later have seen a few doctor over it , most give pain pills but mainly got told it's all in my head. Now I can barely walk. I am not after pills or lies just the truth and way to fix it.

    Dr. Wright, what about snowboarding? the reason I ask specifically about snowboarding is because your feet are locked onto the snowboard and you use your hip to steer your movements.

      Here is Dr. Wright's response: "I usually recommend that patients wait about a year before returning to these type of sports. This is still possible, however you have to be ready. The hip motion shouldn’t be a problem, it’s the fall that I worry about."

    I had both of mine done in 2018 3 months apart. Bone death of the ball. No known cause seeing I don't drink etc.. I'm an avid surfer and hiker. I went into the first surgery in great shape as I could still ride my bike but walking was bad. My right hip would often lock up and I'd be stuck. I waited 3 months before surfing and mnt biking again. My first big test was hitting the reef 3 months later and the hip survived. My issues were all of my original older body parts now had to keep up with these new hips. That led to some groin pain issues but I was able to work that away.
    Honestly the surgery wasn't a big deal. No pain pills at all. Not even in the hospital. I even jog again but never downhill. Jog uphill and walk back down.

      @@kandacepatterson7965 It's really not a big deal but I guess it all depends on people's pain threshold. Also what kind of shape you are in will dictate your recovery time. You can walk right away and you notice how much better you feel right out of surgery.

    hello, so with the lateral verses anterior approach would the activity level be not as much too soon? Im headed in age 50, nurse getting a lateral total left hip. I was told 3-month of recovery. Is it because of my job?

    I am 74. 6ft 4 inches tall and 180 lbs. In good shape except for a right hip bone on bone. I was all set to get a hip replacement last January - anterior approach. Settled on an experienced local surgeon but on the way out of the appointment he said I should think about selling my Miata to help avoid a possible hip dislocation down the road. I canceled going ahead with the surgery and walked out. Yes, the car sits very low and I am a very tight fit, but I have loved 2 seaters since I was 16. In your practice would that also be your recommendation? As much as I would love to have less daily hip pain, I just do not want to give up my roadster!

    Dose core decompression really help in early stage like stage I or II

      From Dr. Wright - Yes it can! Core decompression can help stimulate blood flow to the involved area of the hip which can promote healing. This is more likely to work if there are reversible causes of AVN and if the area of the hip involved is smaller. The recovery from a core decompression is longer than a hip replacement in some ways, so in certain patients it is possibly a better decision to just replace a hip even with early AVN

      Here is Dr. Wright's response: "You can jog and run, or ski or even participate in martial arts after you are fully recovered. However, lower impact exercises are healthier for the joint and will make it last longer."

      Here is Dr. Wright's response: "A resurfacing has several unique problems and complications. There are only a few surgeons who perform that procedure and I have found a total hip to be more reliable.     Why does any doctor continually do posterior/back/side surgeries anymore when moving/cutting muscle and/or nerves could be damaged? A doctor crippled my sister's right leg by cutting her sciatic nerve back in 2012.

    I don't have Perthes' disease but my hips looked like this before double THR surgery in June 2022 due to abuse from hockey. Google "Kreider hit on Price" but in my case the player landed on my torso with my legs splayed out in a butterfly stance. I'm almost 100% without pain now except for bursa pain in one hip due to the trochanter sticking quite far out. 2 PTs have mentioned that the bone shouldn't be that prominent; the surgeon said it does look a little far out but to keep up the PT until the pain goes away. After 12 weeks of PT the pain has subsided but not gone away (always about level 2-3). My question is this: is there something the surgeon can do for this issue? Is there some kind of shaving that can be done, or some other adjustment? Thanks in advance for your reply!

      Here is Dr. Wright's response: "Bursal pain can be an issue before or after hip replacement, or even in people without arthritis. I don’t recommend shaving type procedures, typically anti-inflammatories, PT, and an occasional injection. This is a problem that can be bothersome to patients, even those who have not had surgery, and there sadly is not a great surgery for it but these other modalities are helpful."
