Dr. Rabiul, How does the lipid goes into the Tunica Intima? Is it because of Endothelial Dysfunction thus increase Vascular Permeability?

+Andrew Chew Thank you for your question. According to the “Response to Injury” hypothesis endothelial dysfunction is the most common reason behind initiating atherosclerosis. It can occur due to hypercholesterolemia, turbulence, toxins from cigarette smoke, homocysteine, infectious agents etc. Endothelium dysfunction will result in loosening of the tight junctions between the endothelial cells and result in increased permeability of fluid, lipid and leukocytes into tunica intima. This is why more lipids will enter the tunica intima from the lumen of the blood vessel. Note that in the blood lipids are carried in lipoproteins. Elevated level of cholesterol in the LDL can directly impair endothelial cell function. These LDL can also get oxidized by free radicals generated by inflammatory cells. These modified LDL will be taken up by macrophages by scavenger receptors and accumulate inside the macrophages converting them into foam cells. Binding and uptake of these modified LDL will also stimulate release of various growth factors and cytokines that will attract more inflammatory cells and the vicious cycle of “inflammation---> oxidation of LDL---> uptake into macrophage” will continue.
