The refs are too busy following the action and are no longer counting players and even the opposing sideline would miss it unless someone happened to be looking right where the player comes on. Now obviously with film and cameras it would get busted so fast but what if it was only done on one crucial play a year to get incredible field position or a score and never again?

Also if this were to be a consistent strategy like you said then rules would be put in place to prevent it, NFL rules actually change more often then you think, it might go on for a season or 2, but it would be eventually brought down( also who would want to watch a system where a team is rewarded by cheating? That would be a new low for the NFL). But otherwise, yes, if a ref doesn’t see the penalty, then you can’t review it, but you could make that argument for ANY Penalty( Also your head coach could just challenge a play too) 3.
