I've had it happen to a cheap power supply before. Accidentally left my screwdriver inside there while repairing it, and it was touching one of the leads on the capacitor. There's a few large capacitors in there that store all the power going through the power supply. Those are capable of storing a surprising amount of energy for their size. What happened was that a metal piece of the CPU fan fastener broke loose and the spring ejected it onto the GPU, shorting it.
