How do you ready the nade and not have to hold mouse button?
Start sprinting with the nade readied and let go of the mouse button while sprinting. There might be other ways, but that's how I've always done it.

Hold the mouse button down and ready the grenade throw. Then press escape to go into menu. While in menu, let go of mouse button and then go back into game. You'll now be holding the grenade without holding mouse button. Left click to throw.
