  • This video explains how to use a nerve (ankle) block for painless bunion surgery (bunionectomy), without general anesthesia. Dr Hadzic points out that a partial ankle block, with tibial and deep and superficial peroneal nerve blocks, are all that is needed for complete anesthesia for this operation. Additional clinical tips and cognitive aids from NYSORA’s fabled Nerve Block app simplify the understanding of the procedure.

You are pretty much talking ankle block, but I personaly prefer popliteal block. One does fewer stabs with less pacient discomfort and gets longer anesthesia and analgesia. You get foot drop with popliteal block but benefits outweight the drawbacks IMO.

Popliteal block is fine as well. However, it results in a foot drop (common peroneal nerve) for the duration of the block. This is why we prefer a more distal approach, wherever feasible. If the surgeons are using distal tourniquet, or Esmarch, and are reasonably efficient (under 60 minutes), partial ankle block is all you need. Greetings

@@DRBLUESNYC what would you add to cover discomfort/pain from a mid-high calf tourniquet? Had such a case recently

See if you can negotiate with your surgeons to use above ankle Esmarch - instead of Tourniquet - works just the same. If they must use longer calf tourniquet - Partial Ankle Block + Sedation - Light GA. Greetings.
