Yes, there is a SERIOUS bug with the game. When you get iver a certain number of entities on any given map, the game starts to have problems with pathing and collision. People start to get stuck constantly. They're getting stuck in the walls, because they're trying to get through them, ghosting, and the game thinks they can't be reached or they can't reach their destination, so they get perma-stuck. Guards and staff you can tell to move by individually clicking them and forcing them to move, or the guards you can use the Guard Response button. The problem though, is the wall-ghosting. If a prisoner gets stuck, you have to remove the blasted wall, or they keep getting getting classified as unreachable by the game. Turn on Spectral staff and it may fix this issue temporarily. The main cause of this bug though, is either your computer isn't bestly enough to handle all the calculations of the map size and number of entities you are running... or more likely, the devs haven't gotten off their backside to fix the coding issue with the same said calculations. The developer is basically cramming in features, but isn't fixing LONG STANDING bugs that everyone knows are problems with the game, and have been since day one of development. Introversion have either bitten off more than they can chew, or they got cheap and lazy, one or the other.
