its a belt that increases your flask effect and makes your magic flasks active at all times. This is really strong with the magic flask affix increased flask effect. You can almost cap resists with just one flask.
It's a belt that is used in very high end builds. Currently they are going for about 330 Divine.

You can't see it in this clip cuz it's cropped but if you hover over an item, even unidentified it'll show the image of the item. Every unique looks different so he saw the mageblood picture on it and then realised. The other heavy belts look very different

He keeps the unique heavy belt in the filter and he checks all of them. Maybe one day they will skip oene by mistake.
If you hover over the belt you can see the picture without picking it up.

he basically "juiced the map out to the maxx" meaning he invested ALOT of money for map to get sooooooooooooooooooo many items + they use group play = 5 players = almost 5x monsters and loot + they have a magic finder in group for even more loot
POE is a good game but its combat and rainbow puke visuals are terrible.
