Microsoft Forms is a great tool for creating simple input forms for scenarios such as surveys, sign ups, and quizzes. In this video you'll learn how you can take your Microsoft Forms data and put it in Microsoft Lists for easy collaboration, automation and reporting.
I'll show how you can leverage Power Automate to take new forms responses and add them to your Microsoft Lists, as well as how you can take existing forms responses and add those to your list.

Thanks so much for the sub!
I know this video was published in 2020, but I am so glad I found it. My Microsoft Forms & Power Automate screens look very different, BUT, I was able to follow your tutorial and used it to trouble shoot why my list wasn't being populated correctly! Thank you!
It doesn't work for me any tips ?
This was great and EXACTLY what I was looking for! Thank you!

I'm so happy to hear that! Thank you!
@@AprilDunnam Might I also ask if there are videos within Power Automation to send interactive notifications to someone else (& not self) - if you have a video I will be grateful if a link can be shared, please.
Hi, This was very helpful.
I also liked the trimming of the video and that is presented at a steady pace. Thank you.
Glad you enjoyed it!
She-ro! Thank you soo much. You made creating an automatic flow for my event registration so easy! 🥰
Yay! You are so welcome! Glad I could help make it easy! 🙂
Thanks for walking us through that. really helpful
My pleasure! Glad it was helpful!
Great video once again! Thank you. It would have been great if you had covered how to get attachments from Form to List Item. And also different complex column types. It would have taken the vide to next level. Can you make one including different column types?
Thanks for the feedback Ramesh, I'll consider that for a future video
You are awesome! Finally found a video that is high quality and straighfoward.
Thank you!
All your videos about Flows are so much helpful...Thank you so much... looking for more and more

Ok, at around 11:30 I had this great AHA moment what I was looking for the whole day while watching many tutorial videos but could not find the answer...
Amazing! Creative = smart !
Thank you! Glad this helped you find what you were looking for!
Really helpful video, thank you! I've got a question. If I use the spreadsheet created by the Form, if I was to create more columns alongside it and input data, can I create a flow to send that data to the list also?
This is just excellent. I was messing around with crappy Infopath to get some kind of sub-optimal result , but this rules.
Oh InfoPath, I remember those days :)
This works! thank you so much!!!
Awesome! You're welcome!
Awesome.. thanks i will try right away next week... easy to follow up..
Good luck!
Great guide. Do you know if it is possible to share who has filled in the forms, in the list? At work Microsoft registers who we are from our MS-profile, so it would be great if we didn't have to write our name or initials as part of the forms' questions. We share who is attending certain events, and there it would be nice to directly share who has responded to the forms in a list. Does the question make sense?
Thank you for this video.
My pleasure!
Thank you for this great tutorial. Is there a way to create a form to be used by all members of my team so they can send the form to our clients and collect the responses in the same microsoft lists? or each team member will have to have to setup this flow separately?
unbelievable, excellent video. I'm really impressed.
Greetings from Brazil
Thank you! Cheers!
Perfect explanation!!
Thank you ☺
I am very glad and grateful to you, that helped a lot. Thanks.
You are welcome! Glad it helped!
April this is a great video as are all of your videos. Quick Question when wanting to pull in Quiz Results, how can we pull in the Final Score into the list?
I created my first list workflow, thanks!
That's awesome!! Here's to many more workflows to come !
Thank you Just what I was trying to find.
You're welcome!
Really helpful video! Do you have a video on how to get the attachments from Form to List item?
April, this was great! It's going to help a lot. One question... Is there a way I can group by the title field in my list?
Awesome, thankyou so much, I've been stuck trying to get form data already entered into lists
Glad I could help get you unstuck!
Thanks April, Very Helpful
You're welcome. Glad it was helpful!
Thank you so much for the video @April! Question, my company does not have permissions to use PowerAutomate just yet, how can I create the flow manually without using one of the "record responses in sharepoint" template.
So well explained, Thanks April
Thanks so much!
Thank you so much for this great video. Question: what if my MS form has choices? When creating the list, do I add the choices or leave that blank? Thank you!
Ms. Dunham, with all the various platforms to store data, I need to collect standard data from employees i.e. Last Name, First Name, Car Type, License Plate, Make, Model etc. This list is updated annually. Would you recommend as the data input using MS Forms and storing into a MS List or MS Forms into MS Access?
Is there a way that I can get the score from an MS Quiz to go to the SP list?
ILY thanks again April
You're welcome!
Will try this out
Thank you 😊
You're welcome 😊
Dear April, is there a way to create a Microsoft Forms using a dropdown with information from a SQL Server for example taking information from an employees table in order to get the EmployeeNumber and EmployeeName, or what do you suggest to use this, my project is to make a Quiz but i need to get information from a database on SQLServer to show some catalogs.
omg this was soooo good !! thank you very much !!👍
Glad you liked it!!
Very useful content. and easy to follow. Thank you, Thank you Thank you!
Thanks so much. I"m very glad to hear it was easy to follow 🙂
This is a wonderful video! But, I am having trouble getting my existing reports into Lists. When I run the test, I get an error message that the input parameters of my Create Item action. it is saying it needs "to be of type 'String/uri'." Do you have any idea what my issue might be? I followed your steps exactly!
This video was very helpful!
I'm trying to add a another column to calculate the number of times a word (ex: yes) appears within a row. (Important not column but row). In Excel I would use the countif. How can I do this in sharepoint lists?
Thank you for the great video. Can you please guide how we import attachments along with responses data? Thanks
Thank you for your tutorials. They help a lot! I'm stuck trying to get two different forms to complete a record in in a SharePoint list. I know I may not be the only one with this type of problem. Can you help us?
This was so helpful! Thank you!! I want the list to be private to just me and supervisors. How could I do that?
Excellent video
Thank you very much!
Fantastic. Is there a video you have on how to include the attachments from the form in the created list?
Thanks for this awesome content
Thank you for the support!
Great Video. How do I forward or send back the form to the individual for data correction and resubmit?
Great video. RE importing existing Excel data, copy/paste in Grid view would be easier and a lot quicker than using a workflow. Not sure if there are any clipboard limits but I just tested 100 rows with 5 columns and it worked just fine. Hope this helps.
Thanks for the suggestion David
Excellent presentation. I followed the instruction, but my data from the form not populated in the list. with Instant flow as well; though the run was a success.
Thank you for this tutorial! I am now trying to figure out how to get responses in multiple sheets-depending on what info is entered on my form. I can't find a tutorial on how to do that.
Thank you for the Video, may i know how can i create multiple records with separate date on every row in list when Microsoft form response has Start date and end date.
Thanks!
Thank you so much!!
You earned another subscriber
Thanks so much for the sub!
other then the website where the form is filled and results are stored is data somewhere on my onedrive or?
Hello! Does anyone know if it is possible to power automate a microsoft list to a microsoft form that you did NOT originally create but are a collaborator on?
Hi, when you insert a response with multiple answers, in the list SP adds " " to each element, is there a way to remove them? Thanks!!
I LOVE YOU THANK YOU
Thank you!
Very informative. Question. One list is created and populated, is there a means to update an existing record? Or view a response in the Form format?
Can’t thank you enough for this video! Question, now that I have it stored into Lists, I want to view the information in a graphical way. How would I do this ? Power BI? And how would I create a flow that adds additional information after it has been submitted? I know I ask a lot but you have a clear and easy to understand tutorial and would love to see this in action . Thanks again!!!!
You're very welcome! As far as viewing the data, PowerBI is probably the easiest approach. You could also leverage SharePoint list formatting to show custom visuals in the list itself as shown here: github.com/pnp/sp-dev-list-formatting/tree/master/view-samples/column-graph-format
As far as getting additional info there after it's submitted, there's a second flow you would want to create for that. There's actually a template for it that you can search for to setup. I think it's something like "Forms data to SharePoint". That will create an item in the SharePoint list every time a new entry is added in the Forms.
I'll see about creating a video on the scenario at some point too.
Ok question, I followed your tutorial to the t. When it came to finding my form id, my forms wouldn’t populate. I googled what was going on and I tried copying and pasting the form in manually. My dynamic content window still wouldn’t create the different choice I had on the form or lists and I kept getting an error. Any idea what I’m doing wrong here. Is it something within my form? I didn’t create the form and it is one on a network sharepoint channel if that helps.
Thank you for this. Quick question - How would I do this if my questions in MS Forms are radio buttons?
Is there a way to move the columns in the form excel sheet that is generated?
The ability to have a table in forms would be great.
I agree!
Aguardando ansioso por esta melhoria!
Me again - I got the new responses to show up in my list just fine but I can't get any of the data from the Excel spreadsheet. I was having trouble with the dates so I just removed it for testing purposes and while the flow runs and creates records, there is not data in the fields. Any ideas?
What a good video, I have a concern for a problem that I have, I need that when a user has already filled out the form the message that appears is "Your response has already been sent, This form only admits one response per person." My question is if this information can be customized so that a link to another page appears
Very good explantion
Thanks!
Hi April is there a function to digitally sign a form with one's signature?
great video, but i cant find the power automate template, is there a link to this template? tried to create, but couldnt
Do you by any chance do one on ones? I am having an issue with Forms, Lists, Sharepoint
Hello April,
Thank so much for this video.
I created my a Group MS Form, which responses are loaded on SharePoint Lib and my SharePoint list was also created.
But some responses on my form are not showing up in the dynamic contents for me to match the responses to the list columns. Please How do I resolve this?
Thank you.
How do you take other fields that aren't single line text (e.g. Numbers, and currency) currency in SPO Lists?
April, I'm having problems getting financial values and dates to work with the flow and populate the List, can you help?
I'm triggering approvals from my MS Form. Is there a way to store the approval decision in a list? For example, does this address qualify for service (yes/no), stores to list.
can you instaed doing workaround create a sharepoint list that will have form inside?
Hello April, Your knowledge and sessions are really very effective. I would like to take your support on one of my Queries. Could you please let me know, how to set autofill option in MS Forms while taking details of an employee? Like, Just enter the UIN Number and rest his information gets autofilled, like, Name, Email Address etc.. Looking forward your help on it. Thanks
Hi,
great video.
Is it also possible to fill the form with multiple people based on certain entries within the form?
So as example the form should go through a whole department consisting of 6 groups and each group needs to fill in information in the form and then the form is send to the next group until all 6 groups have entered their information. It could also be that based on certain form entries that only lets say 4 out of the 6 groups need to enter information, so there should be also different branches to steer the form through the needed groups.
Thanks in advance!
Hi April. I see that the Responder Email is available as dynamic content. If the form is submitted by a user in my organization, how do I get the users name as dynamic content ro write to the sharepoint list?
Has anyone found a resource for examples with different data types? (Picklists in particular)
FAN GIRL! Thank you! Women in TECH ROCK!!
Thanks Denise! I agree, women in tech rock, we need more :)
Another great video, thank you April.
I've got a Forms shared in a Teams and can't find it in Power Automate, only Forms that is mine. Am I missing something or isn't it possible?
Hey Ola - Great question and probably something I should have mentioned in the video come to think of it... By default only forms you've created show in the dropdown in Power Automate BUT you can reference Teams forms by clicking on the "Enter manual value" dropdown option and copying and pasting in the Form ID in. You can get the form ID by opening the form and copying what's after the "ID" parameter
When in the 'Record Form Responses in SharePoint", will the Form ID dropdown include/show Group Forms? It doesn't show the one that I created in Group Forms. Any suggestion?
Hey Clay - To use group forms in Power Automate you'll need to select the "Enter manual value" option in the flow action and copy/paste the form ID in there. It can be found in the form URL, should see a formID= and a big long number which is the ID
does anyone know if i can power automate results from a MS Form to drop into MS Access?
Hi April, I have run into a issue hoping you can help me with.
Someone shared the Microsoft form with edit access (which they created) with me. However when I try to use the form in my flow trigger dropdown, I cant seem to find the form. I understand this could be standard behaviour of shared forms. (If I created the form I can see it though). However requirement is that I need to use ONLY that shared form in my flow.
I then tried to implement a workaround I saw in the forum discussions. The suggestion was to use the form id part (the portion after form id - xxxx) to use that ID as a custom value in the trigger. However when I do that way, I am unable to set the dynamic values to the the sharepoint list columns. All I see is only one dynamic item "list of response notification". Can you please suggest as I am stuck
Hi April, how do I do this for multiple choices, some with 'multiple selection allowed' and some don't. With multiple selection allowed, responder also could also enter free text.
How's the connection between a list with columns that are for instance formatted as currency or Boolean reconciled with forms which you don't have that type of formatting or data type if you will. These columns don't show in the dynamic fields in the template you use in flows.
Hi,
When I want to setup the 'Record the Responses in Sharepoint' (7:30) and am choosing the Form IDs, the last box (Create List) only gives me one Sharepoint option for Web address connected to my Uni. There are no options under List Name. Can't continue! Help please!
If we have to take survey of employees every month and in second month we have to prepopulate the values to form from data submitted in first month from datasource how to do it.
Great video. My flow didn't work, I'm getting a message error: The 'inputs.parameters' of workflow operation 'Create_item' of type 'OpenApiConnection' is not valid. Error details: Input parameter 'item/Anyfeedbackfromtheclient_x003f_' is required to be of type 'Boolean'. The runtime value '"No"' to be converted doesn't have the expected format 'Boolean'. Does anyone know how to fix it?
