  • Doctor Andrea Furlan is a physiatrist with specialty in chronic pain management. In this video she explains the cause of spinal stenosis and neurogenic claudication. She shows 4 postures to relieve pain, 10 exercises that can be done at home, and 3 activities to avoid if you have neurogenic claudication.     Dr Furlan, When Spinal Stenosis finally brought me to me knees, I knew I had to do something and it was not going to be surgery. Providence brought me to You Tube and to your series. I have now been using your 10 exercises daily routine for about 6 weeks. Progress initially was slow but has picked up measurably. My initial step average/day was about 4.4K, but this week I raised it to 7.0K steps. I am watching improvement happen in real time. Have been combining your exercise routine with some I learned from PT as well as some gym workout. Results are amazing. While I am not out of the woods yet, I am ecstatic with my progress.       @@DrAndreaFurlan I would also like to add that my spinal stenosis was mainly caused by poor posture. So, in addition to all my new exercise routines, I pay considerable attention to improving the way I walk & sit. Everything is paying off, handsomely. Since I am retired, 40-50 minutes/day is a small price to pay for the improvements I am now seeing. Sure, use my comments as you like.

    I have had degenerative spinal discs L 3 through S1 for 32 years beginning with L 4-5 herniated in 1988. I have seen at least 10 surgeons over the years and many MRI’s to chronicle the gradual deterioration including facet hypertrophy and central snd foraminal stenosis. At 69 after 11 steroid transforaminal injections that helped keep me walking the central canal stenosis is really causing alot of pain spasms in glutes from standing up in the morning to things like caughing. I ham doing many if these exercises and they help. I hike on and off trail in the sierra nevada mountains 5-6 times s week 5 to 8 miles and like today the spasms in the glutes were strong on my hike starting slowly and steeply up a mountain slope. Fascinatingly by mile 3 or 4 the spasms subsided completely and i was able to hike slow and steady fir another 4 miles where i usually get pain waves going down hill. not this time. Moral of the story is do the stretches and exercises for the rest of your life and hike if you can uphill. I have been a seeious mountain hiker now for 50 years and my goal is to stay out of surgery, keep weight down, keep legs and abdominal core and back strong and hike till I drop! Good luck to all who suffer from spine problems and keep moving.

      Yeah I have identical back issues as yours but a little more severe. I also have arthritis growing faster then the doctors would like to see in my lower spine as well. Ive already had back surgery for it and i'm back to square one with pain, except I can at least ride in a vehicle or sit. I finally gave up on stretching and trying to get out of this pain. The pain will always exist. Its just so freaking hard and hopeless feeling. But I shouldnt give up as I think it will get worse. I do absolutely nothing on the weekends because of my back and when monday rolls around Im so stiff and even worse off then I was 3 days before work on friday. But it feels like thats all I want to do is just lay around and not move because of the pain. My range of motion is so bad in the morning it takes every ounce of energy to get my work boots on in the morning. The nerve that is pulling and pinching that shoots down my glutes to. Cheers from Elko Nv... this is the crappiest condition to have at this age but I know there is worse.

      ​@@nateb-runs5053 I'm sorry for all the pain you're suffering. I, too, have stenosis in L2, 3, 4 and 5 and it's so damned debilitating that all I can do is lay down when the pain starts getting too bad. I have Oxycodone that I take with two Tylenol, because acetaminophen boosts the efficacy of the Oxy, but I try to ration it for times when It's that, or jumping off a bridge.
      I am going to watch this video and hope that Dr. Furlan's exercises will help me feel better. I DO NOT want back surgery. I've heard too many stories about people who ended up worse than before the surgery.

      Thank you for that. It makes me hopeful. I am 62 and the spasms in my glutes are unbearable. I am afraid to never hike again, but you make me see it's still possible.

    I had been diagnosed as having lumbar spinal stenosis from MRI. I could barely walk for ten minutes. As I have been doing your exercises from youtube for around two months, I can now walk for around 40 minutes at a stretch. So thank you very much for the exercises Doc.

    Hi Dr. Furman! Last January I was diagnosed with spinal stenosis. I could not walk without pain. I found you on the internet and followed your ten exercises to do. Also you recommended omega 3. I have done these things faithfully and it wasn't too long before my walking got better. One year later I'm back to my usual distance of 3 kms. I am so grateful to have found you. Thank you!

      @@anjaliyadav-vn7yz HI there Anjali ! It was a few weeks before I noticed a good improvement. I also do alot of stretching which helps. I had walking sticks and I had to use them to help me for any distance. I still use my walking sticks because they help to keep my posture straighter. I am very grateful that I can walk and will always do those 10 exercises.😊

    Thank you, Dr. Furlan. Just watched this today (8/04/2021) and I really enjoyed watching and listening to your exercises and advice. I'll start on these exercises tomorrow and will give you feedback after a month. My X-ray showed spinal stenosis but I also think I have piriformis syndrome because my back doesn't hurt, just my right butt down to my ankle. Pain is around 4-5 sitting down but standing up and walking, sometimes pain rises up to 8-10 with shooting pain as well. I'd appreciate your comment. Thank you!!!

    The best set of exercises I’ve seen in months for my Spinal Stenosis. I have done several of them but this gives me a planned set that includes walking.. starting these today. So glad I caught your video. I also have arthritis in my lower back. Will watch your videos on Arthritis too.

    My husband had a spinal op on the 4th vertebrae in April 2021 and on January 2022 a return of pain and bad forward posture.The following day after exercises no pain and walking well.These are amazing and thanks for sharing.

    thanks so much doctor for sharing this. the excercises work wonders since my age 63 this year i started getting regular leg pain when walking. your stretches work wonders, especially the prayer stretch, it relieves the nerve pressure very nicely and i can use it when pushed for time and always get a good result. walking capability has increase from 1km to 3km over a month since starting your stretches. thanks heaps.

    Thanx, Doc. While I don’t believe I have spinal stenosis, I do have lower back pain resulting from a stroke during my heart transplant and s subsequent fall. I will work on all the great therapeutic exercises you offer that helps me to walk stronger and better.

    Thank you for your videos! I am familiar with most of the exercises but feel more confident doing it with you. I find other fitness type apps or videos intimidating because they focus on reps within time limits. But this is more encouraging and no pressure. It is nice to feel that personal connection with small steps to reach simple goals that are totally achievable and results are therefore guaranteed :)

    I'm just coming across your video and what you described is exactly my situation...Hip osteoarthritis, lumbar spondylitis or stenosis with severe low backache and pains down to the legs especially on walking,. I'm most times sitting or lying down and scared of trying to walk except I take pain killers.. I will try and start up these exercises and see how far I can go. Thanks.

    Thank you again for this useful video. I have been doing these exercises for many months and they help my walks. One additional technique I recently adopted greatly helped me walk without getting a sharp pain in my back. This is nordic walking with sticks. I can walk longer, with better posture, and get exercise for my upper body as well. I encourage others with spinal stenosis to try it, as well as Dr. Furlan to comment. Best of luck to all.

    Thank you so much! MRI showed severe stenosis at L3-L4, mild at L2-3, L4-5 and disc bulge at L4-L5. I like the relief positions you showed as well as the exercises. I get pain into my legs upon bending back up from picking something up off the floor, for example. I’m hoping your strengthening exercises will rid me of that leg pain. Thank you again.

    I've found that I can do the pelvic tilt even when I'm driving in my car. I do deliveries, and if my back starts hurting while I'm working, I'll do them while I'm driving. It really helps!

    I have had back trouble as long as I can remember. I was formally diagnosed with spinal stenosis in 2008 but I believe I had it plus cervical stenosis as early as 17 years old.Currently I’m dealing with osteoporosis and the results of bad posture, getting much worse. I’ve had 7 vertebral fractures In the last seven months. You can imagine how many doctors I have consulted. And none of them have explained spinal stenosis or the spinal column and easy to understand and visual way that you do. What is wrong with them! So many people like me, need a hand to hold and visuals. I have done so much reading on the Internet but I am most impressed WITH YOU And will follow your program diligently and share it with my sister who also has similar problems. 2023 will be different! Thanks . I have just subscribed to the channel and I am in the process of sending this particular video to several people

    I crawled to this life-giving video after an Indiana Medicaid treatment for spinal stenosis left me in agony at home alone for more than 2 weeks. (COBRA POSE and many repetitions of backwards flexion were instructed by a Physical Therapist who claimed in writing to relieve my back pain.) When I started Dr. Furlan's session, I could barely do the motions but I followed along to the end of the video. After one session, I am able to sit up in a chair. I will return daily to Dr. Furlan's generous share of genuine therapy for spinal stenosis and skip the unbelievable IU Health cure for intentional pain - Duloxetine 90 tablets x 7 refills. THANK YOU, DR. FURLAN for this common sense work-out for a debilitating condition.

    I have doing this exercise for about ten days and I can walk better and more distance and I am so happy. I talked to my doctor and he told me beside stenosis I have sciatica as well, now should I continue with this exercise or I have to combine with sciatica as well.       It is better to ask the orthopaedic surgeon who did the surgery. They know which approach they used (anterior, posteror or lateral).

    Hearing people speak and share their stories on battling chronic back pain, I thought I shared my story...
    I started out dianosed with Congenital Spinal Stenosis, Loss of Lumbar Lordosis(flatback syndrome) and as time progresses, my pain and other painful issues started arising...
    *herniated disc
    *disc bulging
    *degenerated disc disease
    *foraminal stenosis
    *spinal spondyolitis
    Just to name a few....
    Throughout the years, of suffering and fighting the chronic back PAIN... it finally gave in on me(borderline paralysis)...so I had an emergency L4-L5 decompression, done to my lower back. After surgery, I had bladder issues and also alot of dead nerves... had to spend 4 years to rehabilitate...
    And after 9 great years(38 yr. now), with slight and manageable pain....it has now returned to the max... and became a nightmare all over again. With depression to the max, and unbearable pains at times... some family/friend truly doesn't understand what I am going through. 🥺😮‍💨
    I was always told that my back isnt really fixable, but can only be patched up(decrompression).
    Anywho... to all the people suffering from chronic backpain... and depression... I FEEL YOU... keep fighting on 💪BECAUSE I AM.

      Yeah I have identical back issues as yours and I'm only 32, been dealing with this on a severe level since 27. I also have arthritis growing faster then the doctors would like to see in my lower spine as well. Ive already had back surgery for it and I'm back to square one with pain and range of motion, except I can at least ride in a vehicle or sit now, as before it was so unbearable to go to work every morning it was unreal. I finally gave up on stretching and trying to get out of this pain because it feels absolutely hopeless with no end in site. My hamstrings are so freaking tight its unreal, cant stretch anymore because of that. The pain will always exist. Its just so freaking hard and tiresome mentally and physically. But I shouldn't give up as I think it will get worse if I do. I do absolutely nothing on the weekends because of my back and when monday rolls around Im so stiff and even worse off then I was 3 days before work on Friday. But it feels like that's all I want to do is just lay around and not move because of the pain. My range of motion is so bad in the morning it takes every ounce of energy to get my work boots on in the morning. The nerve that is pulling and pinching that shoots down my gluts to. Cheers from Elko Nv... this is the crappiest condition to have at this age but I know there is worse.

      I had spinal stenosis to L4 and L5, six years ago, along with major surgery to take care of the problem I was having. I had a laminectomy, and they placed a cage and two rods along with screws in my back. I had relief for six years from my pain but now I have stenosis in the L2 and three. I have had several blocks done, but they were not effective except for about half an hour afterwards, which indicates that the stenosis can be helped. I had six weeks of physical therapy with no help whatsoever from that. I felt it was a waste of time and money. I am a nurse, so I do know what is going on with my body. I am currently scheduled to have the procedure done to cauterize the nerves on one side of my spine and then do the other side later on. But I realize I will not get very much result because of all the hardware in my back. The first surgeon I went to. He wanted to do the same surgery I had before, and I refused to have anymore hardware put in my back, I know that if this fails I can have the indwelling stimulator placed to relieve my pain. I am 82 years old now and I still work five mornings a week. The only time I have pain is when I stand or sit. I used to walk 3 miles a day but now I can’t even walk a block without the pain getting as high as eight, so it limits my ambulation, but I do walk frequently, I am excited to see the demonstration of exercises that I could use because even with PT I never was given any sort of exercises as these to help with the lumbar stenosis. I feel like I am 60 years old except for my back and people also think I am much younger, so I know I am not close to being dead yet, and I refuse to let my back make an invalid of me. Thank you so much for the videos and I am definitely going to start on them today because all I do right now is put ice on my back when I get in pain and that does relieve me for a while I have learned how to pace myself to keep the pain around a five when I am walking or standing. Thank you again.

    Thankyou so much for such a clear, concise series of videos to assist with the many & varying pain inducing conditions. I was diagnosed with fibromyalgia many years ago and had most probably had suffered for many years prior to diagnosis, possibly from childhood. I have done most of the exercises you showed in this video and at all times I have found them to be very helpful.
    Whilst you covered stress triggers and illness or physical trauma increasing pain symptoms, would it be possible for you to comment on whether major stress or loss or other major psychological trauma could be a causative factor, or occasionally a single cause of fibromyalgia. Also would it be possible to address crepitice causing pain in multiple joints, particularly in the spine in the context of a fibromyalgia diagnosis.

    To my delight -I do all these exercises, but it appears Holding them is also beneficial. Thank-you.
    I was plagued with Spinal Stenosis to the point I couldn't walk to the mailbox - that was almost 1year ago.
    I like to think these exercises got me back to my active life style - I am 75. I am back to hiking in the mountains
    weekly plus a bit of biking

    Thank you so much for this information! I can’t report success yet, because I have only recently become well enough from other medical problems to be able to remedy years-long deconditioning. I intend to do the exercises, and I will say that the relief positions, and shopping cart idea have helped me over the last few years.
    I watched this video because of the diagnosis in the title. While I do have pain from my spinal stenosis, it actually eases as I warm up, and is not really what limits my walking. My limitations come from an overall numbness that begins in my left buttock, progresses all the way down the leg until the whole thing is numb, and then travels across to my right buttock and leg. My leg muscle control and balance are affected. Relief positions work. But, to find somewhere to sit can take me a few minutes, and the risk of my left leg giving way and causing me to fall without warning is pretty high. When it’s both legs, I have managed to avoid falling, usually by just leaning on a wall. I know a walker will help, I just haven’t made the time to get one and learn to use it. The end result of these episodes is severe delayed onset muscle soreness in my calf muscles on both sides. Mainly the soleus. That pain is actually more disabling than the pain from the stenosis itself. I am a lifelong stomach sleeper, but fall asleep on my left side. Do these neurological symptoms require any alteration to your overall relief routine?

    This is a great video, Dr. Furlan. I have Stenosis, and have been doing some of these very exercises, but now have more work to do. Thanks much. One question, though. You said not to wear a back brace. I have been using the Renuback device, recently, with good results. It is not a brace, per se. You wrap it around your lumbar spine and attach the other ends around your knees. It helps your posture ((which in my case is pretty bad).. 15 minutes really helps me. There is no pressure until you attach it to your knees. Do you think this is OK to use? Thanks again.

    I have fairly severe spinal stenosis and stay very active. But I can only walk about a mile before I have to sit. I am most comfortable on my horse, probably because of the pelvic tilt involved. Because I do all these exercises (not cycle though) and do a lot of walking on the spot and yoga, I am probably doing as much as I can. Your video reinforced that. Thank you.

      Hi Sue, It is really important that you see a physiotherapist. The physiotherapist will examine your specific situation and tell you what you can and cannot do. These videos are for educational purposes only. Now that you are more knowledgeable about your condition and the types of exercises you could be doing, it will be much easier for you to discuss with your physiotherapist. You will understand what they are saying, and why they are recommending one exercise and not the other one. I think that patients who are more knowledgeable about their options are much better to adhere to the treatment recommendations. That is the ultimate goal of these videos, that you can discuss with your health care professional and be a partner in the decisions.

    I have osteoarthritis and osteoporosis. These forward folds feel so good. Is there a way to safely do them for those of us with osteoporosis?
    An idea for another video... exercises for those of us with low back issues (spondylosis, stenosis, arthritis, etc) AND osteoporosis.

      Oh yes!! I need that too! I know I hurt sooo bad after attending to my personal Hygiene after a BM. The twisting to Wipe causes such bad pain!! I almost start to cry!! Any suggestions!! Short of using a hot water bottle on my lower spine first... or a heating pad. I also Concentrate hot water on my lower spine in the shower!! Feels soo much better!!
      I’m Super messed up.. the only time I felt better, was when the PT pulled one leg and adjusted it to Learn 1 of my legs was slightly shorter than the other. Her pulling & rotating my leg had me on top Of the world!! But it never stayed. I reverted and resumed the pain! Great post adjustment while I felt like Superwoman!!

      Stenosis... Facet Syndrome... shifted L 4/5 disc in my spine... Fibro ... CFS... IBS- C & D.... most recently C as my lil Sister )am RN for Veterans for 18 yrs died on Dec 18 ( Non-COVID)! peritonitis from
      an incarcerated umbilical hernia! Caused 2X ZmIs! The Sepsis!!
      My Spinal MD would do a Radio Ablation about every 18 mos! It helped sooo much! He returned to Utah to be the Doc for the Para Olympians again! EC MD was my Angel! He soo knew his stuff!! I’m Tempted to Fly to
      SLS, Utah to have him repeat what no other MD has the skills to do. I can Tell who Has their hearts in it to relieve pain! He was THE only one!! My lil Sis & he trained at University of Utah! They just do it better. I can attest to The quality of those who Trained up there! My Hand Surgeon too Dr MP . She rocked as a 💜 MD for the past 32 yrs!! Her pts love/d her!! They still are friends e/ her today !! Plz advise !
      Now I just got word that inspire of OTC 3,000 IUs BID of Vit D... I’m still very deficient in Vit D! Now I’m scare to top
