at 21:00 in your video, that large flat washer with the tab, that tab slides on the axle groove and goes under the first nut. The second nut, you asked how nothing is preventing it from opening again, well, you supposed to bend that washer in between the nuts forward over the last nut, you do that with a flat screwdriver, then the last nut will never turn for sure!
Man I’ll be honest with you I would just keep going deeper till I see the problem With that much play it should be obvious once you hit it Looks like something is bad in that pivot point Probably dirt and grit has worn something out in there Sorry I couldn’t help more But let me know what you found Thanks for watching my video
