He could be done with it every time when he leaves. He just gets out and starts all over again. He's done it dozens if not hundreds of times.
So, how is he refusing? In what way is he coherent enough to make a medical decision for himself? They could have helped him into a transport but they didn't have enough motivation to even try.

He would just leave. I had a friend who od'd. Called an ambulance. They took him to the hospital and he was back out and od'd again 3 hours later.
