  • We all have read about the extensor apparatus on the dorsum of the finger. And we do need to know about it well, to be able to tackle any problem on the finger. But sometimes, it tends to be confusing.
    Watch this video to see how simple it can be!
    The formation of the central tendon, the terminal tendon, the sagittal band, the transverse and oblique lamina, the Retinacular Ligaments, and so on!
    What were confusing names, explained in a simple manner in this video!

    Gosh I hate my life. I fractured my 5 digit (pinky) at the base of the proximal phalanx a year ago. The surgeon was an idiot and only told me to buddy tape it. It healed with a malrotation malunion and hyperextension. I’m a guitar player and it has made it painful to play. Giving me discomfort in my wrist as it compensates. I’m so depressed and frustrated. Most surgeons have told me to just live with it since it’s all healed. I found two that are willing to do it. But obviously there is major risk. And as I learn about this area specifically I can see why. It’s insane to think to have to go in through the dorsal approach with a saw and cut my bone and correct the rotation and hypertension. Or at least enough to help without doing much more damage. I’m afraid it would make my hand worse and not be able to play guitar at all. I hate my life. Wish they would’ve just pinned it with reduction from the beginning. So sad.

      @ Dan Gennaro, it is unfortunate that you had the fracture and all the sequelae, which are so common. As you would have seen, the apparatus of the extensors is so delicate and balanced on the finger. Any small disturbance can result is changes in the movement patterns.

      @@GKHandSurgery thank u for reply! Do you think I should do surgery to fix my hyperextension and malrotation? I would like in the long run it would help with the discomfort I feel on my PIP joint. I wonder if aligning it more straight would help with the disconfort it places on my wrist or pip knuckle. I know there will be healing. But the surgeon I found told me 80% chance it’s better 20% worse. If I could send you a few pictures that would be a huge blessing sir. God bless you

    Sir my right hand ring finger not fully expand. It is fracturd 2 months ago. Surgery is done. plate is fixed for broker bone. In midlephalanx. Now my finger not nit fully expand. Doctor wil not talk clearly with that. If leteral bands damage can we re construct after 6 month? Pls answer me sir.

    Landsmeer ligaments mean TRL, ORL. Right sir?!

      Only the Oblique Retinacular Ligament was originally described by Landsmeer. Now some authors use the name Landsmeer for both the TRL and ORL.
