      Dr. Brosious’ visible authenticity is one of his greatest strengths (in addition to being a brilliant surgeon). One of my greatest luxuries is having him as a business partner, cosurgeon, and friend. Definitely check out his videos as well. He’s a huge evolutionary leap in breaking down stigmas.

    i cant imagine having the patience and determination to become a doctor, literally years of life just dedicated to school and making sure you pass school, exams and retain such important information just to hopefully make it at the end and hopefully still have that determination/love for it

    Kevin and Joshua, thank you. Before watching this episode I was strugling to decide what specialty I should go for, and that's becasue I really liked plastic surgery but in my hometown city (Cali, Colombia, very famous for aesthetic procedures) all plastics surgeons are commited to the aesthetics side of the specialty and for me that's not real medicine, so I felt kinda dissapointed because of that and started to look somewhere else. But then this video appears and Joshua does exactly what I was looking for, complex reconstructions, full anatomy knowledge and he does it not just for the money. Also, like him, I wanted to be a hematologist, but now I'm in love with plastics

      It’s a good point. People (and patients) forget that surgeons, physicians, providers are people also with lives and families. Balancing them can be a sincere challenge.

      Obviously aren’t starving but definitely followed the reward of passion over finance in choosing sub specialties. I tell people a lot to avoid the instinct to massively change lifestyle aggressively after training and maintain the financial freedom to be able to change or walk away from employment that isn’t healthy or meeting it’s potential. Having that latitude is huge…and has been really important to us.

      Not just an ortho surgeon but a fellowship-trained, trauma specialist who also does joints and sports and is an all around bada$$. She’s earned the respect of her colleagues, students/trainees and staff under the most adversarial conditions. And does it as an incredible mom and a wife. It’s sincerely an honor to watch her surprise every single person who had doubt, and to be a part of her life and success.

    hi as a 13 yr old who from a long family of doctors i feel like i needed to become a doctor myself and just thinking about the money but after i have seen your videos i feel like plastic surgery is something very interesting and cool so i wanted to ask how did u reach to their and what is the educational line to becoming a plastic surgeon all the way from secondary school gcse to owning my own clinic institute

    Do you quarrel to get hand cases? Who does hand surgery orthopods or plastics?

      Highly dependent on institute. Many PRS departments include hand trained surgeons who did additional CAQ and hand surgery is within all of our scope and training. Many level one centers have required CAQ to take call. In my experience, if you want hand cases, you can find them…

    Can a plastic surgeon choose whether or not they want to do strictly cosmetic plastic surgery/reconstructive or do they have no say.

      Good question! In the US (and I say that because countries with national health services may differ), we can decide whether to do all cash pay cosmetics, insurance-based reconstruction, or offer cash-pay reconstruction out of network, or a combination. Obviously, aside from a few examples, cosmetic is the simplest and most lucrative. Insurance requires billing and coding infrastructure and pays lower per minute. My partner and I are passionate about reconstruction and have billed a mission-driven practice that has about an 80-20 recon to cosmetic mix. We choose to fight the good fight on the insurance side.

    But if you take the fibula how does patient walk ?

      Great question! Depending on where you read, the fibula is only 5-15% of weight bearing. We leave 4-6cm proximally to protect the peroneal nerve and 4-6 cm distally to protect the integrity of the ankle. Most patients are walking post op day 1 and in a few weeks to months, outside of the avid runner, most won’t even notice the change.

      Missed opportunity to shout out to our incredible multidisciplinary team (which is what I refer to when I say “we”). The night-shift nurses, residents, and staff (and really all involved) are integral to the success of our free tissue transfers and the salvage of threatened flaps. And, in all fairness to me, I am connected via mobile device to our external monitor (that trends flap O2 sats) which I check frequently overnight and have set to alarm for concerning changes…even in bed.
