How Does Broken Bone Heal?

  • The fractured bone heals by different stages.
    healing starts by hematoma and ends by remodeling
    this Educational video describing the mechanisms and stages of fracture healing by secondary bone healing. it will explain both types of bone healing.
    Primary bone healing
    Cutting cone mechanism
    There is no motion at the fracture site. Seen in rigid fixation of fractures such as in plate fixation. Minimal callus formation.
    What is a cutting cone?
    Osteoclasts at the front remove the dead bone. Osteoblasts lay down new bone at the site of the old bone. Osteoclast eat up or dissolve the bone tissue with their powerful enzyme systems. Osteoblasts lay down new bone stimulated by hormones and growth factors.
    Cutting cones are formed that cross the fracture site. This is a slow process often taking months to years to complete.
    Contact healing
    Contact between the fracture ends allows healing to begin immediately.
    Gap healing
    •Larger gaps do not heal very well.
    •Could be filled with fibrous tissue.
    Secondary bone healing
    Endochondral mechanism
    •Seen in flexible fixation of fractures such as fractures stabilized by rods and casts. Occurs with abundant callus.
    •Undergoes endochondral ossification.
    •Chondrocytes hypertrophy and degenerates then calcify followed by vascular invasion and ossification.
    Stages of fracture healing
    1-Hematoma
    2-Inflammation
    3-Soft callus
    4-Hard callus: woven bone = hard callus
    5-Remodeling: remodeling is a process by which immature or woven bone is converted to mature or lamellar bone. The meduallry cavity is reconstituted and bone is reconstructed in response to stress. There is more remodeling potential in children.

    Typing this with a wrist brace on my left hand, God is really amazing. The natural ability of the human body to heal and restore itself is really just a miracle. Stay safe guys, and I hope everyone here makes a full recovery!
    3 Weeks later Update: My wrist is pretty much fully recovered! (It was just a sprain, but still impressive). I wish everybody the best of luck on healing!

      Seeing this way later I have a question, so can you tell me what happened as the weeks went on like did you transistion too a walking boot or not and how long did it take to fully heal? Plus my last question is after it healed did you get right back in normal activities?

    Fell getting off canal boat, fractured humerus. Hope you all who have fractures heal fast with as little pain as possible. The pain I have is draining, causing disturbed sleep. I'm going to buy a tens machine in the hope that it will lessen the pain. Love and best wishes from England ❤

    Best explanation I've had.
    Even from my orthopedic doctor.
    I fell down the stairs and fractured my left upper fibula and my right ankle has a severe sprain.
    Needless to say its been a challenge.

    Hey Dr Nabil, Just recently fractured the outer bone of my thumb (distal phalanx) on a table saw. The cut goes right diagonally through the pad and leaving a gap in the bone about 15mm deep and 5mm wide. Nonetheless the bone is still sort of one piece, just connecting to the bottom bit on one side. I am now wondering whether bone is generally capable of growing back over "big" gaps like that. Is it a matter of time, personal healing ability of the body or does it need some luck? I'd greatly appreciate your response! Thanks Simon

    I had bone lengthening procedure on a mal union of a fractured femur. Success. If you have a fracture have total confidence your bone will heal. It can't be stopped.

    I have a tibial spiral fracture. I have been in a full cast for 8 weeks and a walking boot for 3. I have recently noticed a hard bump behind my calf, close to the site of the fracture. Is this callus forming?

    Great vid! Thanks! I believe I fractured my distal humerus when I dislocated my elbow but it was never treated as a fracture so now I have a protrusion along my lateral epicondyle. Just figured this out 18 years later 😂

      Bro no way! I fractured my proximal humerus, but due to bad health care I guess they never treated me with a surgery just a cast, leading to a protrusion along my shoulder capsule, it’s nuts how bones heal without the right support

      @@coricabala they did the same to me, I got a very bad displaced fracture on my humerus they just put a cast ... I'm currently on week 3, praying it heals fine ... are you good now?

    I got a metarcapal fracture recently, after a few days of using slab cast and swelling reduces, I noticed some deformity on the fracture, but the x ray shows otherwise before, is there any problem with my hand..

    Hi, Dr. I just want to clarify about the soft callus formation. Are you saying that wearing a cast prevents faster formation of Callus? My ortho surgeon asked me to weat a cast for 8 weeks. By watching your video, I am confused if the cast would make the formation of callus slower.

      Same here. How are you now. Is there swelling on toes. How much weeks rest does doctor recommended. I had ankle orif surgery with 1plate and 2 screws. Its depressing

      @@Ath-et9xm Three weeks after the surgery now. It's getting better, I guess. No major pain but a little swelling on the foot when I leave it unelevated. I have my next appointment with the doctor in the sixth week after the surgery to check if the bones heal and to know if I can start walking again.
      Yes, it's surely depressing. It affects everyday life a lot but what can I do now is to stay calm and rest a lot.

      it depends on the type of fracture for example there are types such as...comminuted,greenstick etc..out of which comminuted is kinda scary if the fracture is not huge ...it will take max one month...or it its huge then maximum 2-2.5 months

    This explains why after 6-8 weeks post fracture healing, on resuming activity some people again refracture in the same spot, because the fracture healed is not solid enough.

    Diaphyseal tibia and fibula fracture. My orthopedist mentioned that it could take over six months to heal. I am now in my third month and still have pain, especially on exertion and the pain is always worse in the evening and especially at night when I have to get up to pee. Was operated with a 345 mm long medullary nail. Can you get permanent scars after it heals? Taking ASA as needed, ie about once a day at the moment. Especially at night when the pain is worst, I have to walk with crutches to the toilet to urinate. Furthermore, I still have a swelling on the diseased leg. My doctor said the swelling and slight redness is an inflammatory reaction to the surgery itself. Could it be true? Mvh /Kenneth, from Sweden

    I train Muay Thai and I’m watching this because my legs are constantly swelling due to the kicks I take or throwing kicks against someone’s hard defense. I usually just wait a few days to get the color back in my legs before I start going hard again. Im wondering what type of damage could be going on inside my legs and if I can really train them to be as hard as stone

    So I have a medial malleolus break with slight displacement I didn’t get my cast on for 2 weeks because I was in the hospital with pneumonia! And I was already on blood thinners I’m young and generally very healthy other then what’s happened recently anyways I opted out of surgery by no means will I get plates and screws in my body . I hope it heals with this cast they put on ! Thank you so much for this video very helpful ! Also another reason I didn’t want surgery is because 6 months ago on the same foot I got really bad nerve damage and drop foot . And it had significantly got better with PT although I still have lots of sensation loss I didn’t want surgery to mess me up again . Any thoughts ? thanks !

    In other words, your body creates its own bone graft to mend the broken bone.

    I broke my fibula broke so my ankle and I have like two bumps also pimple like is that what this is or should I be worried? They aren’t big and it doesn’t look or feel is if it actually a pimple

    I broke 2 ribs on my right side. 4 weeks later i can literally punch myself in my side and it doesnt hurt. Guess it healed prerty good

    I got broken femur and its 10month ago after operation. I got IM rod fixation. I feel a little bit pain above my knees. My fracture is in the center of femur when I walk and not stable. Is it normal? Will do an xray on june. What stage do you think my fracture?

    What do you mean bone affect by stress if I have a broken toe and it’s remodeling I’m not supposed to walk on it ??

      @@modelotime3608 word I did too I was hit by a Jeep 2 months ago and I’ll be able to walk in a week you’ll get through it I know it’s tough

    Hi, I'm 50 this yr.. I fell from height at work and had facture at 6 places.. both side hip, both thigh, both knee.. 6 implants. It's going 2 yrs in 3 months, all are healing well even removed the implants in my knee.. BUT, both my thigh (femur) were not healing. Did total 5 surgeries on both my femur.. first subsequent surgery was due to implants slippage, second was bone graft to help healing (failed), third was putting the fractured bones nearer to help healing ( cause tremendous pain when walking, failed), fourth was to switch out old implants for so called bigger implants to help healing, (cause even more pain at more places, again failed).. I did physical therapy thru out the yrs but always disappointed with the x-ray results.. Does anyone knows if my femur bones will fully heal even after such long time?? Or anything at all that might help my recovery.. Thank!
