ACL Tear Stretches & Exercises - Ask Doctor Jo

  • The Anterior Cruciate Ligament (ACL) helps keep the knee stable, and prevents it from sliding forward. When you have a Torn ACL, your knee can become painful and unstable.     If you have an ACL surgery, make sure you follow your doctor and physical therapist protocols because there are several different ways to do the surgery, and they require different rehab during the recovery.
    The first exercise is going to be a heel slide. Sit on the ground or your bed with the leg you want to exercise straight out in front of you. You can bend the other one up to a comfortable level or leave it all the way down. Slide your leg up keeping your foot flat on the ground. Go as far as you comfortably can, and then slide back down. Start with 10 of these, and work your way up to 20-25.
    Now you will do a quad set. Put your leg you want to exercise out in front of you. You want to squeeze or activate the quad muscle by pushing your knee down into the ground. If you want to roll up a small towel to place underneath your knee for a target, you can. Also, pulling your toes up towards you will help straighten out the knee and contract the muscles. Hold this for 3-5 seconds and relax. Start with ten of these and work your way up to 20-25.
    Then you will do an ankle pump. You can prop your ankle up on something if you want, but keep your heel free to move around. I am using a roll, but you can just hang your foot off of the bed or stool. You are going to pull your toes up at your ankle, and then push down like you are pushing on a pedal. This is dorsiflexion and plantarflexion. It works as a natural pump to get the fluid and inflammation out of the leg, so you can do this several times a day.
    Next you will do a short arc quad (SAQ). For this, you will use a foam roll or a rolled up beach towel. You want something big so you have a good bend in your knee. Prop your leg over the roll with your knee on the roll. Try to keep your knee on the roll the whole time, and then lift your foot in the air until your leg is straight. If you pull your toes towards you at the end, this will help activate all the muscles in the leg. Try not to go fast because then you are only using momentum. Go slow and controlled with your movements. Start of with 10 of these, and then work up to 20-25.
    Now you will do a straight leg raise (SLR). Lie down on your back, and bend one knee up. Straighten out the leg you want to exercise. You can prop up on your elbows or sit up, but this will make it harder. Pull your toes toward you to help lock out the knee. Just lift your leg to the height of your bended knee. There is no need to kick your leg way up in the air, and it actually takes away from the exercise. Make sure you are slow and controlled with this exercise. Going fast and using momentum will not be effective. If you get to 20-25 and it is easy, then you can add weights if you have them.
    Finally, you will do some knee props. You can use a roll to prop up your leg, or you can sit in a chair, and have something of equal height in front of you. It can be another chair, an ottoman, etc. Place your heel on the other chair, and try to straighten out your knee as much as possible. If this is not enough stretch, you can add weights to it. A sack of potatoes works well, but sometimes it is too much weight to start with on your leg. Get a plastic bag, and add some soup or vegetable cans to it. Slide your foot through the loop of the bag, and place it just above your knee. Try to hold it there for 5-10 minutes and relax as much as you can. Over time, you can add more cans of soup.

      I also hear this cracking sound and feel discomfort while walking. My doctor told me to do some these mentioned exercises for six months. It has been a month and I'm starting to feel better.
      My suggestion is you go see a doctor first because no one can give a solution without examining the patient.

      Hi,yeah i have already consulted the doctor he suggested some kind of same execrice as the above one. My question is will it recover completely if u do this only by exercise? Are do we need any medication? Since mine is sever when i stand more time n walk much i get sever pain on hole leg.

      @@vidyaranib8517 As I am undergoing this therapy I can't tell you from own experience. But a footballer from my university had this this ACL injury and sincerely did the therapies 20 a set; four five times a day for six months and then continued for a year! He started playing, jumping etc as if he never had any problem. So, yeah I hope you'll recover fully if you take this seriously.

      Hello mam..... I m a cricketer nd while bowling my right leg got twisted.... Doctor recommended me MRI.... Nd my MRI reports conclude that my ACL is ill defined, bulked no tear in meniscus
      I feel pain sometimes but i can walk easily
      It hurts when i jump nd land on right leg
      Pls pls pls reply mam how can i recover

      I've got a complete ACL tear going the non surgery route so far my only issues are with squatting and it doesn't seem to be getting better unfortunately. Pain just does not want to go away. 5 months post injury

    Just tore mine a few days ago. I go on vacation in a few weeks and I have been doing this before using the option of surgery and I can somewhat see a lot of progress. Just today I was able to get my leg almost straight because of this. Thank you, Doctor Jo.

      @@sherainepayne9172 it’s much better! I ended up scheduling the surgery after my vacation but like two days before my surgery, my knee was almost completely straight again and fine! Every once in awhile, I get nerve pain in my knee and I’ll get a sudden pain that away eventually if I run up some stairs. But it’s a lot better!

    I found this video too late but I wanted to comment and say that doing these exercises and focusing on mobility made post-surgery SO MUCH EASIER!! Especially being able to fully straighten my leg. PT would’ve been so much more painful if I wasn’t able to fully extend my leg to 0 degree pre-surgery. So anyone who is searching, these exercises are it!! If anything I would add some calf raises and hip strengthening. Thank you for putting this info on the internet!! 😊

    Thank you so much .. exactly one month ago I had injury. My ACL, PCL,MCL completely torn and fracture tibia near my knee cap.. it all happened cause my adult son who has autism, he and his brother both fell down on my knee during one of his tantrum episode. 2 adults more than 300 lb fell down on my knee and I heard 3 snap sound..ouch 😢
    waiting for my knee surgery.. anyway I will follow these exercises. Thank you again.😊 I wish everyone goodluck..stay safe..

      @@saggitarius1000 unfortunately there is no way for a fully torn ACL to heal on its own because the blood flow is cut off when it tears.

    Thank you so much, I've injured my kneecap over a year and a half ago, getting close to two years. The physio therapist that I had before was amazing, she gave me some of these exact excerises. I was pushed too hard by everyone around me, however, so I've been stuck with a knee that is very difficult to work with. It's hard to bend, buckles a lot randomly when I walk, etc. The stress of school plus the stress of constantly being reminded that I can't do the things I loved anymore (likr badminton) because I can't run or anything gets to me so bad, that I stopped doing excersises and lost motivation to do them. But I'm starting to slowly work on bettering myself and reminding those around me that sometimes being too pushy makes things worse rather than better, and that even if my progress is slow, it's still progress. ;)

    trying not to be a wimp but this is actually making me cry. super helpful. torn my acl recently and the pain and has been out of this world. i was able to do a couple of these and it felt great to get through them.

    Hello everyone! I have a question. It's been 21 months since I had a meniscus crack coupled with "tension on the ACL". Granted, I got 2 different doctors' opinion, but both relied on the fact that my ACL was damaged. No sign of tear though. Only recovery that I did was sit in a cast bed ridden for a month, after I started exercising more and even playing football again (with an elastic knee brace and only playing in goal, so limited running around). I even started going to the gym and working my legs quite a lot, benching a max of 140kg with no pain (I have skinny legs to begin with so that's a great number for me). Ever since quarantine started in March and I stopped going to the gym, I started experiencing great pain in my ACL. I felt (and sometimes still feel) very insecure about the injured knee when walking (like it's about to collapse at any point). I took some recommended pills and the pain started fading away after, but every time I run or walk for extended periods of time, I get a lot of pain there. I cannot currently visit a doctor for a better opinion. Do these exercises work for my rehab process? Thank you everyone, sorry for the long message. Good luck to everybody with the rehabilitation!

    Thanks Dr. Jo! I’m in physical therapy now for a partial/near full ACL tear. I’ve added these exercises to my plan. I have so much anxiety about possibly needing surgery if I don’t fully heal. Do people ever walk / run / do physical activity normal again with just physical therapy? Thanks for this awesome video and information. ❤️

      If you are able, I would always try a month or two of physical therapy first, and then re-assess where you are at. Even if you end up needing surgery, physical therapy can help get you as strong and flexible as possible to help make your recovery much easier (so it's not a waste of time as some might think). Your physical therapist should be able to help you with figuring out what is best.

    I just had my first day working on landscaping, and my knee gave way, and I got a tone of pain, I carried on working. Bad idea, still got the pain now for 2 days, should I go doctors?

    Hi Dr Jo, thanks for a useful physio and your background explanations of the muscle physiology. It all helps to do the right thing by my knee :-). 1.5 weeks ago, when skiing, I torn my right ACL and partially the posterior horn of my right medial meniscus. First week, no walking, pain, swelling, now I walk almost normally, can do all exercises, but still cannot put full load on the injured leg. I may have had partial tear in both ligaments since my childhood (skiing :-). Had MRI, doc said, no operation, as I am 50. Maybe later, if it bothers me.
    My question is: will the scar tissue replace the ACL and how strong will it be approx? Can blood flow, massage and the physio exercises increase chances of ACL re-growth / meniscus re-join? Also, when bending, the knee goes through a dull "click" - a bit painful, at approx 75% of the full bend. Will these clicks disappear too?

      You're welcome. Scar tissue does help strengthen the ACL because the ACL itself (a ligament) can not heal. But that doesn't necessarily mean you need surgery. Yes, all the above you mentioned should help. I would really recommend going to see a physio in person though for the best treatment. Unfortunately, I don't know the other answers since I can't evaluate you.

      Yes i had surgery (ligamys surgery), maybe thats not available for you its really a new way.,
      I should do surgery if i were you, the hamstring graft is your best option I guess!
      With me its going really well, its a tough route back, but you have to WORK HARD and keep going, now after 8 months (after surgery) Im feeling great, my knee feels great, so its really patience..
      Im a soccer player (football) but im not yet playing fysical contact, but my sprinting and power are great now and I feel like I can play..
      Note:
      WHAT A DIFFERNCE FROM 6 to 8 MONTHS so dont get too frustrated, the mental part is tough man..

      U will only fill pain for 10days or so and after that u won't fill pain but yea u gotta take care of that Leg for Few months

    Thank you. I find your videos are very helpful. I have torn my acl 3 weeks ago, my doctor want me to do therapy for 6 weeks before I can get surgery. Oh it’s hurts so bad when I try to straight my leg. We surely take things for granted...

    I've injured my right knee. Not sure what I did. Knee locks up and snaps continuously. Will see an orthopedic doctor but thinking maybe meniscus or acl tear. Pain is only during weight bearing. Doing these exercises to strengthen. Wear a brace with support side bars. Helps a little still locks up ( like popping knuckles).

    Hi Jo, I tore my acl 6 months ago and was told I wont need surgery and should strengthen the muscles..
    However after doing recommended exercises and wall sits my knee hurts and feels sore when moving, bending.
    Is this normal when doing strengthening exercises? Do people do fine without or Would you recommend an operation generally?

      Not really. People can do okay without surgery, but if it is a large or complete tear, your knee will not feel stable.

    I had ACL surgery nearly two years ago and I am still having knee pain in a regular basis. I am no longer an athlete, but I still enjoy physical activity and weight training on a regular basis. Any advice on how to reduce or eliminate this pain?? Thank you!

      Yeah me too.. Its been a year of my surgery. But still there is pain and i feel very unstable. Also i can not stand for a couple of minutes too. How to overcome this pain?

    can acl tear can be cured without surgery? i have torn my ligament of right knee. and its been about 3 yrs. actually i still play football but sometimes due to disbalance it again starts paining and dislocating. cant it be ok without surgery? ans me plz

      A completely torn ACL will not heal on it's own.

      I'm having the same problem , I did the mri and doc told me it's partial acl tear, also he said it's possible to strengthen your quads really good , especially your vastus muscles which are holding the knee in place.

    I have ACL Tear injury😞I thought it’s just a minor injury that will heal by itself but it’s not. But unfortunately I don’t have enough money to endergo surgery .☹️😭

      JohnSheen Paceno I’m in the same boat it’s been about a year now. You can definitely do stuff without an acl. I would just take life as you go and not get caught up with the knee injury. Trust me if I could have one thing it would be my knee back. I’m very active and just learned the harsh truth that I can’t play sports to my full potential anymore and it sucks. I even picked up skateboarding to try something new and that bombed cause I blew my knee again. Don’t let your knee get in the way of your success it’s just something that is very common and sucks when you can’t repair it cause you feel worthless. Trust me man you aren’t alone and we’re all here for each other dog. Stay strong brother, prove people wrong my friend.✊

      My best advice is to work as much as you can to get the money and get the surgery you cannot do much things when you have an ACL year from that point things will go down if you try to exercise and keep on running. I know not having the money must be hard but getting back your knee is probably worth it. I had an ACL tear during August.. had my surgery on September and ever since then I has that mindset to over come your challenges and trust me you will have challenges for your rehab. Good luck and be careful.

    hi I have got knee injury when I was playing football it was two month and have ago so I met Adoctor last week and sad U need MRI ....After MRI .we meet Again and He said U need To do one month on Phiso therpia so now How kind of Exercise I Can do???As well my problem is Acl tears is broken in the middle Thanks Am waiting For Ur Answer

      Without getting to evaluate you, I really don't know what would be best. I would recommend going to see a physio therapist in person, so they can do an evaluation, and give you the best exercises for your needs. Good luck.

    WOW I cant believe my doctors have not addressed any of this to me. I go in for surgery on the 19th. I limped in their office four different times, you think they would have asked if I was doing any kind of rehab. Toward the end of my last visit when we scheduled my surgery, I asked about any kind of work outs. He said biking is ok, walking around is ok, and that was it.
    They should have realized I have been babying my knee, based on my limping into the office each visit, due to lack of knowledge and recent shock, and suggested I start doing all these rehab workouts. Thank you Doctor Jo, not only did you address all my pains, but you have given me the confidence I need to start rehab now. I am doing these things today and looking forward to a strong knee, for my surgery
