The most interesting thing to me is that there was a ligament that no one knew about?? How? What else do we not know about anatomically? It is actually a tad scary.

Hello Dr. Kopelman. I find your videos very informative, thank you. I'm in my early 50's and always had the hollowness/dark circles under my eyes.
I've been able to cover them with concealer all these years but I'm at the point now I can't stand looking at them. I'm really skeptical getting tear trough fillers as I hear negatives about it but after watching this video, I may reconsidered. If only your office is located in SoCal. Any advice on who qualifies on performing tear trough fillers? Besides a MD, I've heard a RN and a PA can also perform the procedure. Of course the cost will depends on who one goes to I assume.
Great question. So I would always first encourage you to go to someone who really knows facial anatomy especially anatomy around the eyes. Second going to someone who has many years of experience and knows how to manage potential complications is key because injecting under the eyes is a very sensitive area that you don’t want done wrong. Typically I always suggest an oculoplastic surgeon or facial surgeon is the ideal person to inject. Nurses and PAs can inject the tear trough but finding one that has the expertise and knows what to do when complications arise is critical. This is not a procedure I recommend price shopping for - go to the best person.
@@drkopelman Thank you for your feedback Dr. Kopelman. Do you mind sharing your thoughts on microneedling with PRP/PRF under the eyes? Because I was skeptical with tear trough fillers, I just did microneedling with PRP, first session, and the result is very minimal. I have my 2nd appointment in 6 weeks. Would microneedling with PRF provide better results? Would you be open to do a video on this topic?
@@cattiennguyen5946 it’s not an ideal procedure to use micro needling under the eyelids. I recommend laser resurfacing. Again your skin tone needs to be considered which laser is most ideal.
I got it done 5 years ago, still looks great! Totally changed how I do my makeup. Here in Texas we have specialists that do under eyes and it’s an extra $250 for their specialty ❤

Dr Kopelman, I have a tear trough issue as I have deep set eyes and I am in my early 50s. A cosmetic part of a plastic surgery clinic literally saw me for 5 minutes and suggested 1 use 4 vials of Prohfilo and some other fillers. Does this not sound completely wrong and over the top and perhaps unprofessional? I am healthy and my issue are minor. It’s with age, I now see some bags under the eyes and that’s it.
It is difficult to advise you what to do without seeing you but in general under your eyelid hollows are best treated conservatively-one to two vials at most
But fillers can migrate is what I heard. Is this correct?
Fillers can migrate but I don't believe the the issues with filling the tear troughs are primarily due to migration.I believe injection techniques are most important

I am sorry you had a complication from eye fillers. The truth is it boils down to going to someone who has expertise. Not everyone is competent to inject the lower eyelid.

My dermatologist won't fill tear troughs either........
Understandable because filling tear trough requires a high level of expertise and not everyone doctor feels comfortable doing it. I have been filling under the eyes for over 20 years and really have seen amazing results for my patients.
So bottom line,as I see it, is you need to find an experienced, ethical injector who will analyze your needs and anatomy to help you make an informed decision.
