    Most relevant care plans for immobility would be :assess for pressure ulcer using braden score.
    Elevate the lower limbs to improve venous return

    hi violet ur first problem is about breathing and ur aim is for the patient to have normal respi rate but in your intervention you did not address how to help her with her breathing like positioning, deep breathing, oxygen..etc. ..

      Pt can use the call bell, pt understood reason for referral, pt verbalizes understanding of treatment plan, future past tense, hope it helps
