    New to practice nurse right to the ICU. I wish we could pick a “major” in nursing school so we could focus on what floor we wanted to go to. Spent a whole semester on PEDS and Mother/ baby- which was a waste bc I’ll never take care of either. We learn so little about ICU in school and it would have been such an advantage if I was exposed to more.

      Haha I felt the same way. Totally could do without PEDS and especially L&D!
      I think critical care is quickly glazed over because you could truly spend the whole 2 years just covering it. Just to plant some seeds and then really learning more once you start working and orienting there.

    Thank you so SO much. These lessons are a lifesaver. I'm a nurse and over 50, just started working in cardiology ICU. Coming from quite a different background I feel like a confounded nursing student sometimes...you know, the usual: little time to learn and fit in, a lot of work to do, few, often tired colleagues who already know everything (and think you do, too, heh)... I don't know how I would manage without the help of clear, no-fuss lessons like yours.

    This channel was a godsend during the Covid crisis as many of us new icu nurses needed a better understanding of how to care for these patients with a lack of a true nursing orientation. I still find myself reviewing these videos to continue to improve my practice.

    Lots of detail and excellent review. Often times we loose this perspective so it's great to revisit what the wave represents and of course can we rely on the information.

    The beginning of systole is the closure of the mitral/tricuspid valves (S1) and the commencement of isovolumetric contraction. However, it makes sense to think of systole as the opening of the AV when thinking about "systolic" blood pressure.

    I Love Love Love your channel. My only request would be that you make longer video quizzes or more video quizzes. I've found the video quizzes to be very helpful in forcing me to recall the information. Just a suggestion. Thank you so much for all that you do.

    Over damp is the same trace if your vamp chamber is full of blood (I.e. when sampling for ABG) ...
    Makes sense because there is lesser energy transfer as the fluid is thicker ... just need to see a square waveform of blood filled blood chamber next time for my own academic reasons . Nice upload . More! Please!

    I ain't a med stude, I just needed to understand the essence of waveform.. I have a palpitating problem and my neuro said the oximeter and ECG are irrelevant to detect wether I need to take my med.. when I feel the palpitation I should take the med.. So each time I use the oximeter I watch the waveform haha! The question is how can the waveform tell if there is something wrong? When the pulse rate seems to be normal (88-96) so here I am being led here.. thank u for the lesson, sir.. big help..

    dicrotic notch is due to closure of aortic valve not AV valve

      AV is shorthand for aortic valve, not the A/V (atrioventricular) valves I think you are assuming.

    What about placement of the transducer at level of nipple? Any tips for this? I've used a physical level when patient is stable and during trauma or an MI, just eyeball it.

    On the square test, if you see a perfect square wave, and a single oscillation, but the oscillation is much taller than the square wave... What does it indicate?

    My facility uses heparin/pavarine infusing over an IV pump at 1-3mL/hr rather than a NS pressure bag. Is there a way to preform a proper square wave test in this case?

    I just started icu after doing Ed for 5 years. And I have never felt so dumb and I feel so dumb in ICU, didn’t realize how much I do not know.. any resources that you guys suggest? Iam nervous but I know I can do it

      I'd recommend getting the AACN Essentials of Critical Care. Don't over do it though and just take time to learn a little after each shift. It can be a whole new world and you already will be good at recognizing normal and abnormals and just lean on those around you as you learn what to do in those situations. I've got a lot of great videos on this channel too that you would probably find helpful. Just take it a day at a time and know that its a process. Everyone feels that same way starting out. You got this!

    hey u Critical Care Dudes. ... is there any way to calculcate the stroke volumen / cardiac output via the Monitor/Aterial Curve ?

      Not sure about by hand (although Im sure you can), but there are algorithms in devices like the FlowTrac that do that.
