  • Today, Dr. B, orthopaedic surgeon, answers one of the most common questions she gets when people visit her office: Should I get a cortisone injection or not?
    People often ask about cortisone when confronted with pain from arthritis, tendonitis, and sometimes gout.
    She explains what cortisone is and what it does for your pain. Dr. B also goes into the disadvantages and even dangers of overuse of a cortisone shot for managing joint pain.

    Good info. I have had good and bad cortisone injections. The doctor really does make a difference.... As Dr. B said though, unless you are doing something about your pain, it is just a temporary bandaid. I spent hours, probably more like days doing research on shoulder fixes. I have been dealing with shoulder pain for 14 months now. I started the Shoulder Pain Solution about two months ago and the last couple of weeks I have been doing better which has been great. I am in for the long haul and working hard at it. Despite 14 months of pain I still havent got a cortisone shot as I am really trying to use it as a last resort. Dry needling is next if I hit a long plateau.... Thanks Coach E and Dr. B for the content. At the very least, it has given me hope and thankfully I am starting to see results.

      Keep going with the program... I started it last June (2022) as I'd had pretty bad pain in my left shoulder, multiple issues with tendonitis, bursitis, etc. but always eschewed getting a cortisone shot. (I'd had two, one in R hip years ago, another in the other hip, a year later) and felt that they did nothing outside of temporary pain relief. At the time, no one in the ortho world was telling me about fixing my imbalances.
      So... after six months doing SPS and regressing where I needed to to keep out of the painful range (which kept shrinking.) In the last week of December 2022, Dr. B. did a "live" (you must be a premium member or have bought into a program) and several of us asked questions about shoulder pain, including myself. With having so much of my full ROM back, but suffering from pretty bad pain at night while trying to sleep, I was wondering if I was making any progress on a deeper level. Dr. B. talked about the options of the cortisone shot, dosing with ibuprofen, and offered suggestions for sleeping props, but also the downsides of these, which only confirmed my decision to not seek a shot. What did I do to help me "get over the hump" of intense pain?
      (I am not a doctor or in the medical field, BTW) -- I had read about a "supplement" which is really an enzyme, already in use in Japan and the EU for scar tissue, post-surgery healing, inflammation, and more. It is not an overnight "pill" like ibuprofen, and it must be taken on a completely empty stomach, but after a week, I absolutely noticed (A) slightly less pain each night, resulting in better sleep quality -- and better healing by inference -- (B) the pain was lessened such that I was able to do some of the SPS routines more effectively and add a few of them back into my SPS routines so that I could build the strength I needed to get past that state of quasi-stagnation that the pain was holding to.
      Fast forward to now, September of 2023 -- I have about 95% of my ROM back and now can more clearly understand and identify my shoulder impingement! I'm seeing the finish line, trust me. Once the pain sheds layers of intensity, more tissue can continue to heal and you can do more things (not more difficult or "harder") to keep making progress. Keep in mind that I'm working diligently to "FIX" the underlying movement and imbalance issues (a great deal of it is how your neuro-muscular "map" is off, too) and have never been so CLOSE!
      Read up on serrapeptase, okay? I also subscribe to ConsumerLab.com (a website run by an MD and his team, subscription only, as they do not accept any ad money) and read their write-up on it, which convinced me it was safe. I feel great, and I truly hope that you and others who may read this keep the faith and find that finish line!

    I've heard this before. Thank you for talking about this. Surgeons don't warn patients about the negatives. Do local anesthetic injections cause the same degeneration?

    • @erinboynton8460
      @erinboynton8460 Před 8 měsíci +2

      Local anaesthetics do not cause significant tissue damage if injected into the right place- using local anaesthetics for pain control is a bandaid but will not decrease inflammation or promote tissue regeneration. The key however is to get to the root cause of your pain and change how you move. :)

    Can i ask for a general consensus on its use for Mortons Neuroma. Common wisdom is it 'shrinks them', but are we at risk of increasing the likelihood of plantar plate degeneration

    • @erinboynton8460
      @erinboynton8460 Před 8 měsíci

      If an experienced practitioner does the injection the plantar plate should not be at risk. I always ask-why is the nerve being compressed so that a neuroma is forming??? Fix the root cause, the injection could cause the nerve to decrease in size but if you don’t fix why it was being subjected to micro trauma the it can come back. So check out ROM coach Strong feet 1 and 2 to address your foot foundation.

    Was advised for my knee pain to go with PRP instead of cortosone. Had 3 injections in both for a cost of £1200. It did nothing to help and consequently after 3 months i was shown to then have torn meniscus in both knees, how i dont know!
    I then needed to have 2 full meniscus surgeries and was advised to have more injections as a supplement ( it was some kind of nutrition injection). That cost another £500 for those injections.
    9 months on from surgery i am way worst than before my first injections, which is about 13 months ago.

    4 weeks ago I had a total knee replacement. The surgeon told me that he found evidence of Cortisone injected into the joint. I can‘t remember to ever have received Cortisone shots, it must have been more than 40 years ago as I still played football intensively. The surgeon showed a picture of my open knee with kneecap removed already. The Cortisone looked similar to white salt crystals. Maybe these crystals contributed to accelerated wear down of the cartilage and my grade 4 osteoarthritis? Never know, but too late anyhow.

    Dr B, I had three spinal injections for tingling. One a month for 3 months. 2 years later follow up MRI found (new finding) bone marrow edema L3-L4, L5-S1 bone marrow ischemia. The spinal doctor never mentioned this. I believe this may be due to the injections after listening to ur video. No confidence in most doctors anymore. Next follow up they wanted to do more injections and I said no. It’s a sad state of affairs. If you have any suggestions please let me know. Thank you again! I would be more than willing to pay for your services.

    I know you’re not a doctor but i just want to ask what you would do in this situation as you are an athlete yourself.
    Some context for you: I play semi professional football; i had an mri scan recently and it came back saying I have an osteochondral defect in my knee which is essentially a hole in the cartilage (it causes me patellafemoral pain aswell & I’ve had the problem for 3yrs now(im 20yrs) I’d say the pain can somewhat be played on but it makes the knee flare up from 2/10 pain whilst running up to a 4 or 5/10 pain.
    The said I have to wait for surgery and I’m not sure how long exactly could be years and by then my career would be over.
    What would you honestly do in my situation ? Continue playing on it then hopefully get surgery on future or stop playing completely and wait until surgery to return to sport?
    Also do you think peptide’s would be a good idea for the defect in my knee cartilage?

    • @erinboynton8460
      @erinboynton8460 Před 8 měsíci +1

      Well- actually I am a doctor:) an orthopaedic surgeon. I cannot say without examining you- if you were my patient I would advise you to look for the root cause of the defect- you may have had some trauma- but it could also be overuse and damage on the joint surface caused by a stiff weak ankle or a stiff weak hip. I would advise checking out some of our knee pain videos and the Knee Pain Solution program . Peptides are a bandaid as is surgery to fix the lesion- if the lesion heals- but you keep moving the same way, it will just come back. If you change how you move and therefore load the knee- then the lesion could potentially partially heal and be asymptomatic. Good luck, and discuss this with your team.

    • @chidiwidi4148
      @chidiwidi4148 Před 8 měsíci

      @@erinboynton8460 firstly, thank you for such a detailed response, I really appreciate it :) if I were to implement the correct exercises in order to restore correct movement/gait how long would you reckon it’d take for the lesion to become asymptomatic? Lastly, do you have your own private clinic by any chance, as I’d be happy to pay for a consultation?

    • @PrecisionMovementCoach
      @PrecisionMovementCoach  Před 8 měsíci +1

      It's hard to say how long it will take for you to remold your body as everyone responds to the exercises differently. You should notice changes in 2 - 4 weeks of consistent practice.
