In America (I'm not American so don't take me for an authority) you have new york pizza which is similar, you have 'Italian' or thin crust pizza which is thin but cooked until the base is crispy and doesn't flop, or you have Chicago pizza which is very, very thick, and if you're Italian you should probably not find out about because you won't be happy.

In a restaurant or pizzeria usually yes, but as you can see these are not American sized pizzas. You can sometimes find pizzerias where pizza is ordered "al metro" which means that can be shared at the table and it can be divided in different types in the same order

However I can confidently say that through all Italy is fine to eat pizza with hands. Some people use only knife and fork, but not the majority.

Neapolitan pizza is not much thicker than Roman one. It seems thicker because there are more ingredients on it. Except the border of course.
