I can't use mouse for tendinites problem. I do use Pen Tablet. Is there a kind of mode for that on configurations...?

If you use a pen tablet, it might be worth seeing if you're able to map the middle mouse button to one of the buttons on the pen (assuming it has more than two buttons for left and right click). Another option is to use the setting to use Alt+Right Click as an alternative for middle mouse click (available in the preferences window, and I believe turned on by default).

does the basic trackpad on mac work for the necessary operations?

Although it is possible in a pinch to use, it's not recommended -- TouchDesigner's really intended to be used with a three button mouse w/ trackwheel. Without dedicated buttons and a scroll wheel it can be difficult to navigate.
That said, if you need to use the trackpad a quick tip is that Alt+Right-Click can be used as an alternative to Middle Click. Hope that helps!

Unfortunately no, certain keyboard shortcuts can be changed but not the mouse navigation! Some users have mentioned that they use software like Logitech G Hub for remapping controls for the TouchDesigner app specifically (see forum.derivative.ca/t/other-software-you-use-with-td/265462/9) , but YMMV as it's not officially supported.
