    from the same example of water spill, you calculate your risk (likelihood and severity)
    what are the odds that person will step on the water (likelihood/probablity)
    what can happen if he/she steps on water and slips. Can they get severe injuries or just bruises (consequence/ severity)
    then you compare it against your risk matrix to get a risk rating for that particular hazardous scenario

    When we teach Hazards, we usually provide hazard descriptors using a parsing method that includes, Source, Initiating Mechanism and outcome
    For example, there is a water spill on a road/street, a person slips and injures himself when he accidently steps on the water spill.
    There is a chance that…
    … may occur as a result of …
    … which may result in … .
    Source: Water Spill
    Mechanism: Stepping on water and slipping
    Outcome: Injuries
    full sentence will look like this
    There is a chance that a person may step on the water spill and slip due to which he/she may face some injuries.
    One more example
    Worker can get electric shock by touching exposed contacts in electrical panel containing high voltage.
    Source: Exposed contacts in an electrical panel containing high voltage
    Mechanism: Worker inadvertently touching high voltage contacts
    Outcome: Worker got electric shock
    Example Description:
    There is a chance that a worker may inadvertently touch exposed high voltage electrical contacts within the electrical panel which could result in the worker getting electric shock.

      I like the way you have broken the examples down and included the hazard descriptors, often these are bundled in together.

    A harmer thats not in use is not a hazard, its just a an object. It becomes a hazard when its being used or its placed where it can potentially cause harm e.g it can be a tripping hazard, placed at height where it can be a falling object because its when it has the potential to cause harm.
    This applies to the ladder as well. If its parked somewhere and not in use its not a hazard but using it has the potential to cause harm.
    Aeroplane is an hazard when it carries people and either moving or flying
    A black spider is poisonous because it moves and can interact with people thats why in the first place its locked, thats a control

      Exactly 👍 hammer, ladder, knife, vehicle are objects not hazard. Water, Chemicals are not hazards. Their properties depending upon the nature, application of process are in fact the hazards.
