  • In this video Doctor Furlan explains that the brain is capable of interpreting some sensations as painful when they are actually not doing any harm or causing injury. This is a phenomenon similar to optical or auditory illusions. Some examples of painful illusions are phantom limb pain, allodynia and hyperalgesia.
    Dr. Furlan is a pain specialist in Toronto Canada. She is a physiatrist (specialist in Physical Medicine & Rehabilitation). She holds a medical degree from the University of Sao Paulo and a Ph.D. degree from the University of Toronto. She has 30 years of experience helping people with chronic pain to get better quality of life.

    Good video. That was a screw through the boot not a nail. A screw will tear you up much worse than a nail. In 1977, I had full blown Guillian-Barre Syndrome and was on a ventilator. I was in agony. A psychiatrist(Dr. Brock) was brought in to talk to me. I had to use a letter board(I could only blink) to communicate. After evaluating me(and adding me to a study he was doing) he taught me self hypnosis. With practice I was able to escape the pain for hours at a time. He helped my survival and recovery significantly. My wife's home town MD used hypnosis with his patients and it helped my wife a lot. She was very accident prone as a child. It seems like much more can be done with studying the brain. Good Luck, Rick

    I try to ignore my pain, caused by RA, Lupus and fibromyalgia, by working through it. Physiotherapy seems to help.

    Great video !! I love all the content in your channel. Please could you also post more content about the pain created by anticipation for example undergoing some tests etc. I used to feel dizzy going to the hospital even for some minor surgeries. But over a period of time, I have conditioned my mind to breathe and take in the pain or divert my attention.

    Good content about illusion about pain .yes brain reflects pain signals more ...phantom limb pain explanation is good ...fear avoidance cycle ...is fine...education and movement training are options i am trying here & mirror therapy for phantom.pain ....my hearty congratulations for you dr.andrea got top dr award in.pain management ..

      ​@@DrAndreaFurlan Sounds good but what about world's worst pain trigeminal neuralgia where one can not talk, laugh, eat , can you explain how can we train brain to ignore this bloody pain, when ur fk with our pain then ur centrally sensitization will cut you out then ur auto pilot mode to avoid pain will hide in rabbit hole , kiss your fathers rotten onion balls to lessen pain

    Does this apply to altered sensations? Can the brain interpret pain as numbness or burning?

      Yes. But there is a difference between numbness caused by a nerve damage and numbness caused by brain plasticity.

    Is this the basis of desensitization therapies? I have been participating in those with my physical therapists for my CRPS for almost a year now. The pain is still there....all I am learning is to anticipate that it will happen. I don't feel it any more or less painful and I continue to move and massage those parts in spite of this ,simply because I fear not being able to move more than I fear the pain itself. The pain level is the same, some new additional pain and symptoms from it exist now though. So now I have developed intense headaches from concentrating so much on doing the activity. I also now have my muscle spasms back and my nausea and dizziness is worse. I do share this with my therapists, but they praise my good job of moving and how proud they are I can do this in spite of the pain which still exists. As a person who had scoliosis prior to CRPS I could do so much ignoring the pain ,as long as I kept moving, but could do much more with that condition than with CRPS. So I did ask for my muscle relaxers back to deal with this and try some other things like wet cold compression headbands for headaches, and cimetidine for stomach pain. The stomach pain elevates and then will bring headache up from intense headache to horrible migraine. I also was recently prescribed InfraRX low dose heat pad to help with healing. I go back to MD/DO in May and my Ortho in a few weeks. I also have another dietician appt in April, I am on full elimination diet. Low to no salt, no sugar, no gluten, low fiber, low red meat, and only the yogurt and a few white cheeses my body can tolerate, basically if the food has a potential to cause inflammation it is removed. O.T.C. do not work for relief anymore. My hives and acne are 10x worse. I take Benadryl when things swell or hives travel to multiple places. That is high in sodium ,but must have it. I use hydrocortisone cream too. I am open to any suggestions you or the community can share!
    I do wonder how long does this phase last typically? Is there a chance the pain can go away or does it always remain? Should it take another year? I was diagnosed CRPS in June 2021 officially by a specialist, but my physical therapist first diagnosed me in Feb 2021,and we constantly work on this since then.
    p.s. the illusion games were so much fun! I got each one correct, didn't miss any=), just wondering still if I'm aware of illusions and my brain is based in reality then why can the pain fool me? Its alot of questions, I know, I'm sorry. I thank all who can read and share their experiences and especially Dr. Furlan for posting these helpful videos to offer support to those in the chronic pain community!       Hi Linda, you raise very important points. Brain plasticity is what we use to desensitization therapies. Pain sensations are more complicated, they involve emotions, mood, memory, stress levels (stress hormones), and social context. They are more complex than optical or auditory illusions.

      @@DrAndreaFurlan Thank you for this , believe me it helps a lot! So I think even the fear of not being able to move and channeling it as my motivator is still a motivation of fear....and fear is the emotion that keeps the pain? And perhaps even good stress is still stress? Like excitement and anticipation of a new opportunity is positive ,but for me maybe even happy and exciting times can raise hormone levels that can result in pain? I'm thinking as I type, but do not know this for sure....is there a place to research such topics?
      I also know I have other triggers, I am at the mercy of the weather....especially barometric pressure causing very high winds and summer humidity. Those 2 I can't control, even staying indoors and controlling the environment does not give relief.
      I do use my micro z for a few consecutive hours if I hear the news report of what time the wild weather will arrive, and take all the pain meds on schedule. The muscle relaxers are as needed, but I take those only on therapy days or when the weather will be stormy. At those times I take the muscle relaxer for the entire day or as long as the storm is with us. My Dietician even suggested I do not fuel the fire so when the storm comes I go to a liquid diet. I tried that for the 1st time in March and it did help quite a bit.
      I also practice meditation and visualization to get some control, but mostly it works for me at night when there is less noise and disruptions. I breathe with my hands on my belly,focus on controlling it until it mimics a sleeping persons breathing rhythm,that helps me go to deeper meditation on days I can't sleep or to sleep when I can. Also I try to detach from my thoughts by hooking the words& sentences to a balloon that floats away or a peace dove that will carry them off into the heavens....like I said it works mostly at night. So not practical for the day time. I still can't work with all these problems and inability to bend or kneel, my jobs require those movements. Those are my goal for therapy and we work on it always, just very slow progress.

      Proper lifestyle management sleep yoga meditation Tai chi along with pain education helped me to take control of my pain ...i have my channel on pain.. just started.. as a pain physio
      Even severe pain in my case can heal then you can surely heal.
      . just find out the basic causes of your paying like anxiety trauma..etc and you can heal

      @@DrAndreaFurlan As an update,my therapy has been placed on hold (hopefully temporary), and I am still struggling. My dietician can not be reached, and my MD/DO appt had to be pushed back by 2 weeks. I finally decided to try cryotherapy since I knew the physical therapy was being contested(along with all my other benefits) by workers comp. My ortho , therapists, and lawyer are trying to rectify this. I am temporarily using a line of credit to pay for my cryotherapy sessions. However; I do feel this modality is very helpful. It gives me 1 to 2 days of relief and eases some of the migraine stomach, and pain associated with the weather. I do not know how long I can pay for such an expensive modality,but this certainly eases most inflammation so I can do something for a day or 2.
      I highly encourage those with pain to try such a therapy!
      As a physiotherapist have you ever seen positive benefits from cryotherapy? either whole body or localized? And does it work long term? My former physical therapist told me of this and it seemed the least invasive option. So with all else at risk of loss I tried it and it seems to work. I hope the long term benefits outweigh the risks.

    Is this similar to Central Sensitivity? Part of Central Sensitivity is having an injury that has healed but you still have pain. I have CS and had an ankle “injury” but nothing showed on X-ray or mri and it took 3 years to heal. I now have chronic tail bone pain with nothing showing on X-ray. It’s been 7 months and nothing has helped. It is very difficult to deal with and I struggle every day.

    Dr pls answer me your illusion of pain can it help and stop worlds worst or humams worst psin trigeminal neuralgia, willit be trained to stop it ? If you say yes then i wish you get this pain n you show it n motivate us and if you say then your all this illusion lectures r fpr baby type of pain not suicide worst pain like trigeminal neuralgia

    Where does one begin with illusionary pain? I've been to many doctors and through surgeries without relief. This has turned into bad feeling toward doctors.

      Hi Jean, I'm sorry to hear about that. Each person is different. Not all pains are illusions (nociplastic pain), but when they are, it is really hard to identify and treat them.

      Hi @Ray7234 Stay away from doctors who don't know anything about neuroscience of pain. They keep selling products, injections, pills, etc that do not treat the root cause of chronic pain. The field of neuroscience has advanced in the past decades and it shows that there are modifications in the brain as a consequence of pain. These changes are like "memory" as their mechanism includes LTP (long-term potentiation). In my 30 years as a pain doctor I've seen many people who were able to reduce the pains sensations by working with brain retraining.
