I find modafinil definitely helps me with motivation...the mechanism might be psychological though. I will often build a huge wall for myself that makes starting the task very hard. With modafinil I will often just start the task without thinking about it, then I start just ripping through the tasks like I imagine a normal, well motivated person might do. This is with 5mg or less.

5mg??? Are you sure that’s the dose you take? I just bought some that 100mg each pill, and recommend dose varies from 100mg to 400mg maximum.
I might be meaning 50mg come to think of it. Yeah they might be 200s I get. Either way I split the pill into 4 or even 5/6 and get by without being super jittery and hyper.

I came in search of the research because, at 45, I was diagnosed with ADHD only two years ago, but my doctors also suspect I have post-TBI/post-Covid narcolepsy (not yet diagnosed), so my psychiatrist who has been prescribing Adderall suggested that this might be a better alternative. I’ve heard some scary things about it, so it’s nice to hear from someone who takes it that it’s helpful!
Modafinil helps my depression, in combination with an SSRI, more than anything in all of the years I’ve tried to find something that would work. I’m past menopause and I’m so so grateful for Modafinil.

It's a great drug.
Pros: Motivation. Focus. Optimism (or anti-depression).
Cons: Too much focus. Good if doing worthwhile things. Bad if doing trivial things. One must make strong effort to not go down trivial "rabbit holes". e.g. researching every type of light bulb on the planet.
Loses effectiveness over time (weeks, months, etc). Best to take it on days when you really need it, so it remains powerful.
Withdrawal: For me, if I miss a day, I am extremely tired. I am sensitive that way. The backlash is so bad I can hardly move. This is why I want to get off it.
Bottom line: Highly effective wonder drug. But those sensitive to withdrawal effect, should not get on it.

@@Hknewyorker 200mgs daily. Been cutting the small edge off pill so recently about 175mg. Plan was to cut it down to zero over the next few months. But I went up to 200mg again cuz I gotta get my messy house in order.

@@KytexEdits I use sandpaper and skid down the pill a certain amount of strokes. Increase number of skids slowly in weekly increments. Example: 5 skids first week. 7 skids 2nd week. Etc.
Five skids on roughest sandpaper removes about 3% of pill. It's going to take me about 4 or 5 months to get off it this way. And things are going fine. Thanks for asking. Good luck. You take it?
I’m genuinely someone who’s sleepy all day mostly and especially after meals. It has started to impact my life a lot and I used to feel unmotivated to do stuff also. I’m a healthy individual but I guess that’s the only good thing I do. But I INSTANTLY feel happy after having modafinil and I just take 25mg as anything above it is feel too energetic and not worth it. I’m wayy less sleepy and i get a lot of stuff done also. So it has genuinely been a life changer for me. I also take it 2-3x a week only. I’m genuinely stunned how things are so difficult to get at so many places and people need to consider insurance, prescription, etc. In India I genuinely believe things are the easiest. If you feel you want it, just go and ask. Drugs are mostly cheap and if they’re generic? Dirt cheap. Modafinil is one expensive one and it costs $3 for a strip of 10.

i agree...I didnt take my vyvanse yet and im finding I have to replay some parts of the video due to his monotone voice not really keeping my attention
One thing is certain. If I am sleepless and I have to perform and go to work. Modafinil will do the trick. I might even feel good and motivated. But when I try it as a nootropic or for any other reasons. I just feel bad. It only raises my heart rate. It doesn't really create anxiety in a similar fashion to cafein. Yet being too wired on modafinil is not enjoyable. It is still a stimulant.
I like having it in my medecine cabinet for when I need it once or twice a year.

I haven't yet tried modafinil, but would like to and probably will in the near future.
I've been experimenting with caffeine plus L-theanine, along with some other components in combination. At their best, they are giving me a new lease on life, new appreciation and enjoyment of life. It is as if I had been living in a condition of low-level depression for years. I could function, but not really enjoy life. These combinations, at their best, are really turning things around.

This would be considered as "personal testimony" or "anecdotal," but I would say that Modafinil is "comparable" to Adderall in my opinion. Both medications have their benefits and drawbacks, but a few things I want to note are that Modafinil subjectively feels like it lasts longer, which could be explained by a longer half-life than Adderall? I have been taking Modafinil for roughly about a month now, but at the same time I do take days off from it where I take Adderall, mostly because I'm experimenting with both to see which one produces a more robust effect on symptoms.
Although different people can catabolize, or break down, medications at rates that differ from the average, modafinil's half life is generally longer than Adderall's. That is one reason why many doctors prefer to prescribe long acting versions of amphetamine (Adderall XR, Vyvanse, Mydayis) rather that immediate release Adderall.
Modafinal and nothing else. Tried the others for many years. Wish i could of got prescribed this stuff first.

Hey, some time has passed so I was hoping that you can share more about how you felt while taking it compared to Adderal. Was it good enough for your focus and did it have any bad side effects? I'm aware that the effect of medications vary from person to person but there is a chance that I might be prescribed Modafinil and there is so little information about it compared to the standard ADHD drugs that I'd be happy with every information that I can get :))

@@cooldriver7236 I liked Modafinil quite a bit, it helped with my fatigue a lot I think. I would take it 30 min before I had to get out of bed and I would be able to get up and do stuff. I still felt a bit scatter brained so I switched to Vyvanse and I LOVE IT, I still have my moments but otherwise I have felt so much better. It makes me feel normal and productive

I tried it for the first time today. I still felt tired, I just lost the ability to fall asleep and was mega focused (likely because of what I was doing) but still. Currently 4:30 am, and I’m still awake, 125mg modafresh.
Any parallels with Guanfacine? It seems both acting on hypothalamus, but it seems Modafinil can cause anxiety while Guanfacine lessens it?
I've never tried the conventional ADHD medications(i hope to to do so in the near future), but i've tried modafinil and also methamphetamine a couple of times. Results were:
- methamphetamine(and once the day after an ecstasy trip), gave me an almost weird state of calm; those two experinces were...just ....fucking weird, can't really describe that calm and peace. Anyway, after snorting that speed, i went home just to try how it feels to work on it, and it was WOW, gave me the focus, determination, and allertness to sit down and work, and even be productive for a couple of hours even tough i was verry close to shitfaced. The work i was mentioning is drawing house plans so it does need some time, and focusing etc.
- modafinil would give me the wake up from the dead effect(always,but i didn't always need it), but none of the motivation, focusing stuff, witch where my main problems to begin with.

After taking modafinil for about a month, I noticed my heart not only racing more but I would feel it different. Then I would start to feel pain in my heart at times. I am a fit person, have been consistent in playing competitive soccer. The pain started to worry me so I got off the medication and after a couple month I stopped feeling the heart pain. Don’t think I will go back although I did appreciate the focus it gave me.
I'm sorry to hear that the modafinil didn't seem tolerable for you. Modafinil should be cleared from your body within several days, so perhaps something else was going on that contributed to these cardiac sensations.

Great video! Can you elaborate a little bit on differences in efficacy between Modafinil and Methylphenidate, especially in effect size, both in studies you've seen or your personal experience? I know there's not much information/research, but you seem to be the most well-informed professional on Modafinil and ADHD. Thanks!
Among individuals I have worked with, I have seen a higher percentage who could feel/tell that methylphenidate was working for their ADHD symptoms than who have said that about modafinil. How much of this is due to increasing body tension rather than purely brain effects is hard to sort out. In studies using psychological measurements of ADHD symptoms, methylphenidate looks a little better than modafinil, but they are in the same ballpark. Certainly some individuals feel they respond much better to modafinil than to stimulants, but this is clearly a minority of those with ADHD.
I read comments, so many positives reviews, and just can't understand why it is ineffective to me. Is it something wrong with my receptors? Don't feel anything at all, maybe with some caffeine just helps a little but nothing relevant.
My own clinical experience, which I hope that I conveyed in the video, is that while I have seen some people with ADHD who had good responses to modafinil, the more common response has been mild to no apparent benefit.
I'm considering trying Modafinil right now because of Vyvanse side effect. I'm losing hair so much it's making me depressed. I'm assuming since Modafinil is NOT same type of stimulant so I really really hope it helps me with inattention without hair loss. I think I can deal with other side effects as long as it doesn't cause hair falling out so much. My fingers are crossing.
Many medications other than stimulants have an uncommon side effect of contributing to hair loss. I hope that you can avoid this on modafinil.
Can you explain why Cymbalta is used for ADHD as a non-stimulant? To me it doesn’t make sense based on how I know the drug works.

A patient has a diagnosis "possible autism" as well as "non verbal learning disorder". The most pronounced symptom, from birth, is lack of initiatives, lack of socialization and communication, i.e. passivity. Can this help?
I'm not aware of much evidence that modafinil, other than helping with possible co-occurring ADHD symptoms, provides much benefit for autism spectrum conditions.

You should research it more but I do know there is some interaction between the two. Lower dose of propanolol might be needed
200mg modafinil improves my energy, fatigue, drowiness, wakefullnes without any side effects even headache but no significant effects on Focus. Still it's hard to focus and done projects, books, exams and learning math. Some years ago I had better learning ability.
Vyvanse is amazing for focus and being calm but it makes me so sleepy. I will sleep during the day if I take vyvanse.
I hope to find a drug with power of vyvanse to focus and modafinil for wakefulness and fatigue

Thank you for this enlightening lesson of modafinil! I was recently prescribed this alongside Prozac to help with my tendency to become tired and irritable when the effects of Prozac wear off in the afternoon. So far it’s been amazingly for keeping me mindful and patient and I haven’t experienced a downturn in my energy in the afternoon. It’s also good to know that there are few side effects. I actually experienced the skin problem you spoke about years ago when I was taking nsaids for an extended amount due to illness, so I’m a bit worried about that. However I know what to look for when that begins, so I’ll be able to navigate that if it arises.
One question: when I picked up my prescription I asked the pharmacist about possible side effects. Both he and my psychologist warned me of dizzy spells and asked me to be aware of this especially when driving. Is this something that happens in your experience?
Anyone can have any side effect from a given drug. I've never had anyone who had vertigo with modafinil, and only rarely did people report being light headed or spacey. But I'm puzzled with the Prozac wearing off in the afternoons - the halflives or fluoxetine and norfluoxetine are in the 10 day-two week range, so there should be very little fluctuation in blood levels of that during the course of the day. Might something else be contributing to this circadian pattern?

Modafanil is a controlled substance in the US (although at a lower "schedule" level than stimulants) so you would need to be working with a prescribing physician. Other countries may regulate it somewhat differently.

Yes, for some people it clearly does more than just keep them awake.

Interesting! I haven’t a clue about long-term effects. But I had nearly the opposite effect as aphasia (with Armodafinil, actually).
My ADHD is such that I have a serious deficit in working memory, and going from thought to speech is one way my communication breaks down. So normally I don’t bother speaking up, or my speech is halting, or I completely lose my train of thought.
With Armodafinil, there was much better flow. There was great improvement in fluidity of speech and thought.
But the inhibition was not there, so I would say anything that came to mind - more like a typical hyperactive-type ADHD-er. 😂
Anyway, I am not arguing against the aphasia idea. Just saying that it can affect different people in different ways, depending on the deficiencies (or lack thereof) in each persons neurocognitive systems.

Hello, doctor. Could you please advise why modafinil might not be working? I weigh 52 kg and have taken both 100 and 200 mg, but there has been no result at all. What could be wrong? Could it be related to the fact that I am also taking Yasmin?
I can't offer specific advice without a doctor - patient relationship, but in general, if someone is not having any side effects or beneficial effects, it is usually reasonable to go up, by steps, to the largest recommended dose, which would be 400 mg. Some people don't notice much benefit from this agent.

I'm not aware of modafinil being studied for those with schizophrenia. Lethargy and lack of motivation to initiation action can be big problems for many people with schizophrenia, so conceivably modafinil could help with that, with less risk of worsening psychosis than amphetamine would.

Hello May I please ask why won’t any of my doctors prescribe this to me anymore? They just give the line about it’s a controlled substance. I used to take it responsibly and no doctor will give it to me ! May I please ad 99.9 percent of my doctors don’t even realize it when they read off what medications I’m taking 😅 now all I do is try to buy it online but it’s clearly not as good! I would think that most psychiatrists familiar with ADHD would be more likely than general psychiatrists or general practitioners to prescribe modafinil. Particularly when you can describe benefits from taking it in the past, without problems, that should be convincing for many. You may even suggest that they listen to this video if they have questions about its appropriateness.

Straterra and modafinil goes well together? Im on stratera 60 mg and it works wonders for my mental fogg but it Gets my blood presure up and besides that i get erectile disfuntion. I wanna decrese stratera to 40 mgs and add some modafinil.
There aren't any likely problematic interactions involved with using them together, and I have certainly seen people taking the combination for ADHD.
What about armodafinil??
Armodafinil is the right-handed stereoisomer of modafinil (so 100 mg of modafinil contains 50 mg of armodafinil and 50 mg of relatively inactive esmodafinil). So Armodafinil is roughly twice as strong, and tends to have a longer half life than modafinil, but seems about as likely to help with ADHD, and has similar side effects.
Hello doctor. My physician has prescribed me straterra 40 mg and to counter the severe drowsiness I feel, he has prescribed modafinil. Can they be taken together according to you?

Hello doctor! I have a 6 year old boy diagnosed with ADHD. I tried meds like atomoxetine and methlphenidate. However, there were so many side effects with these traditional adhd medicine with my 6 year old, that I had to stop these drugs. what are your thoughts on modafinil on children with adhd who does not respond well to existing meds?
Step up, suck it up, and DEAL with your child the way they are. ADHD is WAY over diagnosed and is largely a made up disease that cannot be tested for biologically but is subjective.
Your child has a very young, sensitive, developing brain. You are playing with it giving it powerful pharmaceutical drugs that will alter it's development and change them for LIFE, something they have no understanding of nor can they give informed consent to.
I beg you to let your kid be a kid and deal with behavior in non pharmacological ways. I don't know hardly anyone, and I know so many, who was diagnosed and put on ADHD drugs as a kid that feels they were helped by them long term... Mostly they are not normal people many going hard into the recreational drug route and addiction issues, many life issues and permanently damaged brain chemistry/development.
Too many parents give their kids drugs, consciously or subconsciously, for selfish reasons. THEY don't want to deal with their kids, or with a school, etc, and think a pill can "fix" them and make THEIR lives easier. Your kid doesn't need fixing. They're themselves, they're developing, they need engagement and stimulation. Especially if they're a boy, the school system is NOT designed for or well suited to the way young boys generally are and best learn. This is well known. Please just research ADHD over diagnosis and the problem with the modern school system and boys.
It could save your kids life. You wouldn't give your kid a little meth... Then you shouldn't give them Adderall either. And these other drugs, just because they're not amphetamine or traditional stimulants, doesn't mean they are safe. People got by for thousands of years with far less psychological issues before any of these drugs existed, remember that. Be patient and let them learn how to conduct themselves naturally in the world, or else they'll always be seeking a crutch and blaming external factors for the way life just is.

It's as if the only reason to make and market this less harmful, much safer drug is to Jack up the price and screw over the very people who could benefit greatly from it. Let them eat stimulants. For years in the US insurance companies restricted access because it was so expensive, but within a few years of going generic, that changed here. I have had some patients who had success ordering from online Canadian pharmacies when they got not obtain it in an affordable way locally.
Based on your clinical experience, for a patient not working well with Vyvanse and other stims and not well with Strattera, would you typically go for Cymbalta or Modafinil?
I’ve been on Ritalin LA for a few months, not exactly for ADHD, but because I have extreme sleepiness symptoms during a work day (and even on weekends). It worked for some weeks but then I think my body just adapted to it and I continue to have extreme sleepiness every single day.
My psychiatrist then told me to stop Ritalin and prescribed me Modafinil, 100 mg. per day.
Been taking it for three days now and can’t feel any diference at all, if so, it’s for the worst.
Been seeing some videos about Vyvanse (Elvanse in EU), and thinking that probably that will be more suitable for my symptoms.
I’m in a crossroad.

My thought is it’s too low a dosage. Most generic brands come in 200mg tablets

If modafinil isnt a stimulant how come it raises my bp and heart rate and give me anxiety? It doesnt give me energy like adderall nor caffeine but it makes my heart pump more than these. And it is considered a CNS stimulant is this true?

I hope that it helps. I usually encourage starting low and titrating up quickly if one doesn't notice benefits at the lower doses (100 mg).

I've been diagnosed with ASD (ADOS), I'm on the waiting list (will be about 3 years) for an ADHD assessment. (It's been diagnosed before but I need a new one in order to access meds).
Currently I'm on .5 MG alprazo (prescription is 1.5/day) but I work with lowest effective dose as much as I can. I'm about to start moda. I'm wondering 2 things. In terms of cardiac issues (which I will track myself BP / Heart Rate) what are the main indicators of cardiac risk when taking this medicine.
Secondly, how much do you know (or could you point to research) around modafinal as an aid for executive disfunction in ASD. In I'm a non English speaking country so having this kinds of discussions with my doctor are difficult.
Modafinil usually does not have direct negative impacts on heart rate or blood pressure or cardiac rhythms. For maybe 10% of individuals, in the first few days it can trigger mild panic like symptoms, (which do increase HR and BP) but those usually go away pretty quickly. I'm not familiar with any studies looking at modafinil specifically in an ASD group.
@@DrJohnKruse I started today. I can't say I noticed anything amazing around focus and attention. But I did notice Improvements with other things. Definitely some anxiety for about 2 hours and after that it was fine. Very interestingly my sensory issues where better. I need to look more into the research on that end. I certainly wasn't expecting that.

Narcotic is a poorly defined term. Some use it to mean any psychoactive drug that can be misused. By that definition it would be a narcotic.
Hello doctor. I’ve just been prescribed modafinil for adhd. I know that it can take 30-60 minutes to take effect for wakefulness. I’m wondering if it helps with the adhd symptoms in that same time frame or does it take longer. I can’t seem to find the article now but I read it can take 2-3 weeks on modafinil before you see improvements. Is this correct?
I typically take Vyvanse, but used to take Modafinil with it years ago for sleep deprivation due to insomnia and sleep apnea as it was hard to stay awake during the day. I still have insomnia but no longer suffering from sleep deprivation and I have a Cpap which helps with my sleep quality. Now I just take Vyvanse but if I’m out of Vyvanse Modafinil helps to make me feel alert and my brain can focus. It’s not as good as Vyvanse but with decent sleep it works until I can get Vyvanse refilled.
Do you have any views on Adrafinil? I took it regularly a few years ago with good results. My understanding was that it predated Modafinil and it was converted to Modafinil in the liver. Would I have any different response to Modafinil? There were some concerns voiced about possible risks of liver damage. Thank you.
Yes, adrafinil is a pro-drug that is metabolized to modafinil. It was never approved by the FDA for the US market. Most research suggests that most people would have similar responses to modfanil and adrafinil. I haven't been able to find much information about how common liver problems were.

Metabolites made off the original drug are a possibility to your allergic reactions. When you go further down the line you eliminate some of those metabolites. I had to do some research on this myself one time. Like I'm allergic to morphine sulfate injectable. I've never seen anything like it on my veins turn blue and black within 5 seconds of it going into my arm and it works its way up like a zombie movie. I can take other things that metabolize into morphine and not have any effect at all. But there are many different metabolites of the drug morphine and I imagine it's the same way with what you're talking about. That's just one avenue of possible reasons for allergies I know several. Like for example there's over 600 different derivatives and types of testosterone alone. You change one molecule here and one molecule there and you change the response to the drug the formal kinetics and the action even the allergic response sometimes. Fun and complicated stuff huh!!

I was on adderall for several years, absolutely hated the side effects so I took myself off cold turkey about 2 years ago . However ever since then I have been SO ridiculously tired all the time . So I was considering something like modafinal , hoping the side effects would be less and I could still feel awake . I haven’t been to the doctor since I got off adderall and I recently moved across the country. How should I go about getting pa prescription? And do you think it could help me?
In this day and age, health care often begins with your insurance company (or local MediCaid or Obamacare Plans. They should be able to find a psychiatrist for you who can help evaluate if modafinal or other medication is a reasonable next step for you.
