  • Dr Kaser presents his research into using the drug modafinil to help improve decision-making in patients with depression.
    Depression is typically associated with issues such as low mood, sleep disturbances and appetite problems. But some of the most persistent and long-lasting issues facing patients with depression are cognitive problems involving concentration, memory and decision-making. These cognitive problems do not respond to traditional anti-depressant treatment, leaving patients with a clear unmet need.
    Originally licensed for use in narcolepsy, the drug modafinil modulates dopamine and norepinephrine levels areas of the brain involved in learning and decision making. In this study, Dr Kaser used a reinforcement learning task to show that modafinil improved patients' ability to explore options when making a decision - and so leads to improved decision-making outcomes. This suggests that modafinil could be a valuable tool for improving cognition in patients with depression.
    Dr Muzaffer Kaser is an NIHR Clinical Lecturer at the Department of Psychiatry, University of Cambridge.

    This presentation, 'Computational approach to reinforcement learning in patients with remitted depression: results from a double-blind, randomised, placebo-controlled study' was runner-up in the Post-Doctoral Plenary Competition at CATAC 2018.

    Modafinil is the best anti-depressant I ever took...wasnt even supposed to be one..Also gets rid of all anxiety...amazing stuff

      @George P Im not a doctor this is not medical advice but 100 first day, you will not sleep, but 1 week later u will build mental tolerance and want to naturally goto 200...beyond 400 diminishing returns

      George P 100mg is fine for me, but I also read that doctors prescribe 400-600 mg. It’s all trial and error, you’ll have to find the right dose for you. I also noticed, that it works better when ingested with food.

      George P also could be that it’s not the drug for you. It doesn’t work for everyone and some people get toxic liver problems from it

    I've suffered from chronic depression for years. I started taking this drug to help me to study and get more work done, but shockingly I began to become happier. Granted I still have ups and downs but I second guess myself a lot less, and resolve conflict a lot faster. The major draw back is when it wears off. I don't exactly experience a crash, but I basically go back to normal and suddenly realize just how miserable I am, it's pretty horrible, it's like working at a mansion, and then having to go home to a card board box

      ya it sucks. its not working any more. have to stop it for at least 2 day's. i took 600mg and still have no effect. getting sick of it. been on it for years. its better than nuvigil

      Try psilocibin mushrooms, a fairly high dose (3 - 4 grams of dried mushrooms). They are awesome for depression.

    Haven't taken any modafanil for a while, but my depression and anxieyhas been getting worse. Tried lots of things to get me out of the slump; no more weed, early morning treadmill sessions, etc but no improvement
    Took one modafanil today and I'm back to my normal self; I feel great. Anxiety completely gone as the day progressed 🎉

      I've been taking anti depressants for like 6 months now with some changes in meds as well as going to therapy
      I got prescribed modafinil because I thought I had ADHD and couldn't focus fuck all if my life depended on it
      I did my first dose yesterday of 100mg
      Felt like 10 percent better I think
      I did 200 mg today and it does feel different
      I think I might stick to 200mg for a couple of days to see if it works
      I hope this med will work out
      Has there been any side effect for you?

    I use it from time to time when in a depressive phase/lacking in motivation. I find 200mg gets me kick-started and productive, increasing my self-worth as a result and I'll not need to redose on a daily basis thereafter.

      @@KhaledBu23 The right dosage varies from person to person for what I read in the comments but my experience is that if I take less than 200 mg it kind of backfires and it worsens your anxiety, in the other hand if I take more than 200 mg it just works as well as if I took 200 mgs. To me its been a blessing I've being taking it for years and I have no side effects at all other than being thirsty which at the end is really good because at least in my case don't get hungry and lost Weight to a healthy level. I order them from a website MODAF mod.af/ -- I don't know if it is a prescription drug but if it is they wouldn't give it to me, because they prescribe it for other disorders I think. At least here in the states I don't know how is it where you are at. You will find them from different manufacturers in the website I tried all of them and they work slightly different some you need a bigger dosage and on others a bit less and it does the work, but try them you might like one more than the other. All the freaking nonsense that the brain loops on and on just disappears. It works in minutes if you dissolve the pill under your tong that if you want instant results, the flavor is awful but works better at least for me. Here is other that works really well its called DARK ENERGY its a pre-workout powder I would say it's at the same level as modafinil it just makes you way happier but last less longer.

    During university I had lots of trouble getting focused and motivated, the last year I needed to work while still studying so I started taking Modafinil for dealing with sleep deprivation syntoms; then I started to notice how my performance increased as I was capable of getting focused much more often and easily than before, even with sleep deprivation. Now I suspect I had some level of depression and modafinil aided me a lot.

      @@notme7310 At some time I indeed took a 200mg pill every day, but that is a bad idea, not because of tolerance which I didn't notice, but because of sleep deprivation accumulation. If you're so much tired you can fall asleep even if you just had taken a pill. I took one pill early at morning and tried to sleep at least a few hours testing whenever possible. Sleep deprivation really fuck up your health, lowering your defenses and triggering some syntomatology similar to immune deficiency like some kinds of dermatitis and opportunistic infections. Also sleep deprivation really affect your mood. Use with moderation because of sleep deprivation consequences

    I suffer from light depression and study law. I started using modafinil to study longer and more focused, but started to realize, how it helps my depression. I can enjoy day to day activities and the pressure in my mind slips away. It works 5-12 hours for me, but I feel kinda anxious and weird when it wores off.

      sounds wrong. it works 2- 4 hours and only short term. how is it working now.

      Happy for you. Depression and law do not mix. Success in law, esp long term success, has much to do with one’s psychology. Because you are constantly being attacked from all sides, being bulletproof is helpful. Good luck.

      Many lawyers are Better lawyers because they are depressed, depressed people often analyze too much. Not every depression looks the same , so a statement like that is not very accurate
      Some people with depression are okay with been attacked and might even attack back better :). I think my depression would make been in front of a computer without people more challenging than arguing law

    Its also shown compelling results for ADHD off label as well. It still has some addictive properties, but SUBSTANTIALLY less than conventional stimulants.
    (Seriously, I know it sounds like a joke, but I think short acting stimulants like coke are a better option for adhd. Give a patient 10 or 12 doses per month, it shouldn't be a daily medication)

    I am Bipolar 11 and suffer more with Depression. I have been depressed now for three Months. Completely disfunctional. I started Modafinil two weeks ago and it was like a Miracle. Although still a bit Depressed. i am completely functional. However i have only been taking 50mgs as i was afraid i could i switch to Mania. I am thinking of going up to 100mgs. Bearing in mind i take other Meds.

    Although it wouldnt effect the immune dysfunction seen in cfsme, it could be a helpful drug for brain fog and cognitive issues they get. Although post exertional malaise could be an issue with these types of meds and maybe lower doses would be more effective. Many are also more sensitive to medications so a low dose woukd be a good starting point.

    I took modaf for a while and here's my take... While taking it, life is good, nice anti-depressant effect, albeit with added anxiety levels though. The day after taking modaf however, I felt hung-over from it. I always thought this was do to a rebound effect in my brain chemistry. Loved the effects and benefits while taking it.... I worked varying shifts at work so it was super helpful.

    Also be interesting to see the testosterone levels of the males in the group that responded to modafinil. Testosterone is known to interact with dopamine pathways. Its possible that male responders could be misdiagnosed with depression but instead be hypogonodal, especially if over 40. It should be a standard test for men over 40yo experiencing depression. It would be better to treat closer to the source then to treat symptoms when possible.

    The one potential bad side effect of modafinil is irritability.
    But this is true for a lot of stimulants for a lot of people....
    Taking low dosages and not every day can help with this.

      @unscripted I had to stop taking modafinil. I get super agitated if I take them a bunch. It depletes something. not for continuous use in my case or at all.

    My Dr won’t prescribe it :( but he’ll write vyvanse and adderall which are horrible for me. I got my hands on modaf and it’s perfect but I can’t get a script :(

    interesting, i've been taking it off and on for about 10 years. originally tired it for adhd since I can't take traditional stimulants. although i don't think it helps much with adhd it does help me to get out of depressive slumps. it's been hard finding a doctor on board with prescribing it though... why prescribe something that works.

    I have been on tricyclics for 50 years. When I try adding modafinil I feel great for 2 days. After that, depression worsens.

    2 days ago started taking modafinil, got the 200mg pills, splitted into 4 parts, first day took 1/4 and after few hours another, so 100mg felt maybe too much for me? Because had a bit of a headache, one moment even got sleepy or similar to need of a after meal nap. Today took only 50mg or 1/4 of pill in the morning n no complains.
    Noticed the antidepresive effects, pleasantly surprised, some say it makes you less social, well I became more social, meaning- talking more, not as shy, less awkward, not as hard to have a conversation and even wanting to talk with people, i guess that's the antidepressant affect

    in my experience, micro-dose is the best. Make a "to-do" list before..take 1/4 ...get the reward feeling..build your discipline and save your tolerance for when you really need it. Then, in odd times take half . I only took a full dose once but gave me strong headaches.

    My first time taking mods yesterday afternoon. 100mg. It made me focus better but i still feel the shadow of my fatigue throughout my body. I stayed well hydrated yesterday but all i ate yesterday was a tiny bit of soup and rice at around 9pm. Was i feeling drowsiness because i didn't eat all day?

    I've talked about my experience with modafinil on my channel.
    As someone who suffers from depression and ADHD I've noticed that the only thing it helps with is allowing me to wake up earlier.
    It was good for keeping me awake but idk if I'd say it helps my depression

      A lot of people who have mild to moderate "depression" don't really have Depression. Speaking as a woman who has lived with severe Depression for decades I find it insulting that the two states are regarded the same by the medical profession. "Feeling quite sad for a few weeks after your boyfriend dumped you? Oh you must have serious Depression!"

    I have dysthymia (chronic low depression) . I am on some thyroid hormones and I am going to start modafinil. no antidepressant anymore for me, they are basically useless for this condition.

      some relief. like a 5% of it. nothing miraculous. we are very far from the real solution of chronic depression. I am coming up with a video about it soon . for me it's a metabolic problem involving oxygen in the brain and sleep disorder. chemical imbalance is bullshit and so it is genetic. and I am sick of it.

      I dont think that it will help you with your migraine, cause the primary side effect is headache. try caffeine for migraine, modafinil good for concentration and mood

    1. 3 g of Kratom twice daily,
    2. 100 mg of modafinil twice daily
    3. 50 mg of ground and dried psilocybin mushrooms once every other day.
    I've been doing this for about 4 years with nearly complete resolution of depression which I had for about 15 years prior. To be accurate, my depression was most severe in my late teens and early 20's and I was even suicidal at many points. The largest relief I got was from forcing myself to be social which alleviated my social anxiety (my main depressive trigger). I still had a great deal of depression though which would affect my work and relationships. The SSRIs, SNRIs, and DNRIs worked a bit but not for long. I'm a pretty experienced and safe psychonaut so I did my own research and set upon finding a regimen. This one has worked for me for a long time and I have no intention of changing it anytime soon.

    I took medafinil (100mg) together with me medication ( amitriptilin 25mg) , and It did help significantly improving me day to day life, I’m just afraid to have to be dependent on yet another drug to make me function normally.

    I looked up Dr Kaser's paper - the dosage used was 200mg which seems very high to me. That's the prescribed dose for narcolepsy. Most people who use it for improved concentration/cognition etc. seem to use about half that dose.

    Modafinil is good to battle the loss of drive and motivation. But it can cause psychosis if you are unlucky, i.e. genetically susceptible to it. I got psychosis from modafinil

    A person taking modafinil will feel is not placeboo unless they give him some stimulant. I wonder how such studies look after the person becomes sleep deprived from the insomnia side effect
    Modafinil restricts certain type of thinking ( creative).
    There is now an antidepressant labeled for cognitive improvements : trintellix
    I love modafinil but I’m yet to discover how to take daily in a successful way
    I only feel it’s magic if I don’t take it for a few days I also recently noticed a higher dose does better for me
    A low dose actually makes me more tired
    With modafinil after not sleeping you feel Tired mentally but can’t sleep

      My experience has been that 200 mg makes me less drowsy that 400 and 300 is were I get the most energy but then can sleep .
      You can play with it until you find your dose and best time of day to take. Recently has some success using it later rather than first thing in am .. most people even have if it doesn’t energize them all day will feel that is not placebo I would think ..
      Also I’m not sure if antidepressants don’t affect how modafinil works I’m sure each antidepressant and person is different . Good but I think if 400 mg doesn’t work try 300 ,200 , 100 before giving up on it

    If modafinil helps you then go ahead and try it, but in my case I became manic, agressive, restless and from time to time even more depressed. The 1st 4 days were a nice boost in concentration and alertness but after i ran out of modafinil (had a 10 x 200mg blister) I noticed how psychotic I was on that pill, throwing punches at the air and crying 2 hours later in the shower for no reason while feeling like I was in a dreamlike state - i took a pill daily but even at 50mg I was showing such symptoms.
    Modafinil was a hell for me, but maybe it might be an opportunity for you. I'm better served by psychedelics in small doses every 2-3 days.

    Modafinil actually calms me down, lifts my depression and makes me feel normal. However, I cannot take it more than once or twice a week, several days apart. I think I have issues with ADHD and dopamine but I've never found a doctor who would bother to test me for these things. Rather, they put me through a course of every single anti-depressant that ever existed with no result. So... I found Modafinil out there on the web and self administer it.
